1 Judy W Kelley 6 th Annual Scholarship Application Instructions for Judy W Kelley Scholarship Application MOBILITY Credit Union is pleased to offer two $1,000 scholarship to graduating seniors of the Class of The scholarships will be awarded to students that will be full-time college students enrolled in 12 hours or more. All applications should be mailed directly or returned to MOBILITY Credit Union. The applications must be postmarked ON OR BEFORE May 12, 2015 to: MOBILITY Credit Union Attn: Marketing Department 8384 N. Belt Line Rd Important Application Information: 1. Students must include: a. Two letters of recommendations - One from a school representative - One from a member of the community b. A cumulative copy of their High School transcript 2. The application must be filled out completely (no see resume ). 3. The application must be typed or completed in black ink N Belt Line Rd

2 Judy W Kelley 6 th Annual Scholarship Application Scholarship Scope: To be considered, applicants must be high school seniors planning to attend an accredited four-year college or university, community or junior college, or state licensed trade or technical school. Eligibility: All graduating seniors regardless of race, religion, or sex are eligible for consideration upon submission of an application. All applicants must either be a member of MOBILITY Credit Union with account(s) in good standing or have parents/legal guardians who are members of MOBILITY Credit Union with account(s) in good standing. Award: Scholarship payment of $1,000 will be made payable to the school and presented to the students to use toward college expenses. Award Criteria: Selection will be based on several factors, including: character, community service, scholastic record, financial need and recommendations. Application Procedure: 1. Complete and submit a scholarship application including: a. A letter of recommendation from a member of a community organization b. A letter of recommendation from a school representative 2. The scholarship selection committee will review the submitted applications and select applicants for interviewing. Students selected will be notified by phone or with an appointment date and time. 3. Applicant interview process. 4. Selection committee selects recipients an an alternate. 5. Recipients will be notified by June 2, Scholarship recipients MUST submit the following information to MOBILITY Credit Union by June 12, 2015 to receive their funds: a.
