Employers have a choice of four coverage levels, as shown below. This is limited indemnity coverage. Some provisions, benefits, exclusions or limitations listed herein may vary by state and are not available in CT, ME, MA, MN, NH, NY, NC, ND, UT and WA. This insurance program is sold in conjunction with group Limited Healthcare program and cannot be sold standalone. 6 these value-added discounts and services are not insurance and are neither underwritten nor provided by Chartis insurers. They provide discounts at certain healthcare providers for medical services. The plan does not make payment directly to the providers of medical services. You are obligated to make payment for services but you will receive a discount from those providers contracted with the plan. Teladoc services are not available in OK.

5 Description of Benefits 7 Physician s Office Visits Benefit pays per visit benefit if an insured person visits a physician s office for treatment of sickness or injury. Physician and Hospital Discounts We offer a national provider network to complement the benefits provided by Chartis. Discounted rates are available at premier physicians, hospitals and medical centers around the country. Health Screening Benefit pays a per test amount when an insured person undergoes specified routine examinations or other preventive testing. Routine Well-Child Benefit pays a per physician s visit amount when an insured dependent child visits a physician and undergoes physical examination and/or appropriate immunizations during the first 12 months following birth. Outpatient Diagnostic X-Ray & Laboratory Benefit pays an outpatient diagnostic x-ray and laboratory benefit when an insured person visits a physician s office or other outpatient setting except an emergency room and undergoes diagnostic x-ray and laboratory tests for treatment of sickness or injury. Hospital Admission Benefit pays a lump sum hospital admission benefit if an insured person is admitted as an inpatient to a hospital for treatment of injury or sickness Hospital Confinement Benefit pays a daily hospital confinement benefit for each day that an insured person is charged for a room as an inpatient when that insured person becomes confined as an inpatient to a hospital for treatment of sickness or injury. Mental Nervous and Substance Abuse confinement covered. Intensive Care Unit Benefit if benefits have become payable for an insured person under the hospital confinement benefit and such insured person becomes confined in an intensive care unit, this benefit pays an additional daily intensive care unit benefit for each day an insured person is confined in and charged for an intensive care unit. Surgical Benefits pays a scheduled surgical benefit when an injured person undergoes a surgical procedure for treatment of sickness or injury. Anesthesia pays an Anesthesia benefit for the administration of anesthesia for which a charge is incurred during a covered surgical procedure. Critical Illness Benefit pays a lump sum benefit upon diagnosis of a specified critical illness after a 30 day waiting period and 12/12 pre-existing condition exclusion. Ambulance pays a benefit if ambulance transportation to a medical facility is required due to a covered accident or sickness. Prescription Drug The prescription insurance program helps employees manage the cost of outpatient prescription drugs with an expanded list of brand formulary and generic formulary medications available at a fixed copayment. Non-formulary medications available at discounted pricing. Accepted at over 60,000 pharmacies nationwide. Accidental Death & Dismemberment Benefit pays a lump sum benefit if an insured person suffers an injury that results in death. Dismemberment benefits are paid according to the schedule as detailed in the Policy. Fractures & Dislocations Benefit pays a scheduled benefit amount if an insured suffers an injury that results in a specific dislocation or fracture. 7 Benefits payable for a covered accident & sickness. 6 Optional Benefits In addition to the Limited Benefit Medical coverage underwritten by the National Union Fire Insurance Company of Pittsburgh, Pa., our program also offers optional benefits to better meet the needs of your clients. Short-Term Disability In the event that an employee cannot work because of an illness or injury, his or her absence can put strain on the employee s source of income as well as business productivity. With the Sickness and Non-Occupational Accident Disability Income Benefit, a weekly income benefit is payable to insureds who become totally disabled as a result of a non-work related sickness or injury. Benefits begin after the end of the elimination period. The weekly benefit is payable for each week that the insured remains disabled up to the maximum benefit amount. This benefit is guaranteed issue, with no pre-existing conditions exclusion. Questions or concerns about the services should be addressed directly to the providers. Prescriptions Over 59,000 local and national pharmacy locations accept the card. Cardholders save up to 65% on medications including nearly all brand and generic prescriptions. With over 60,000 drugs covered by the card, cardholders save on prescriptions not covered in other plans. Everyone in the family is included and there are no annual discount limits and no expiration date. Dental Members save up to 50% on dental fees from nearly 70,000 nationwide dentists providing general and specialized dental services including: preventative, diagnostic and restorative dentistry, oral surgery, endodontics and orthodontics. There are no limits on the services provided. Vision Members receive up to 50% savings when they purchase eyewear or receive eye care at participating OUTLOOK vision providers. Over 11,000 national and regional vision care centers participate in the discount program. Hearing EPIC offers a national alliance of independent ear physicians and audiologists dedicated to high-quality hearing care. Members can save between 35% and 50% on name-brand hearing aids and products to protect and improve hearing. MRI and Imaging Savings up to 70% on MRI, P.E.T. and CT scans at over 3,900 facilities nationwide. Lab Members receive up to 60% savings. Use our online search tool to locate a provider near you and schedule your prepaid tests. Our provider contracts ensure a competitive price on all tests. All savings are visible to the member with online transparent pricing tool. Test results are shared with member within hours. Teladoc Teladoc is the first and largest provider of telehealth medical consultations in the United States, providing over 3 million members 24/7 access to affordable healthcare via phone and video consultations whenever they need it, from wherever they are. 9 Frequently Asked Questions Q: Are pre-existing conditions covered by the Limited Benefit Health Insurance? A: Yes, pre-existing conditions are covered; however, there is a 12-month pre-existing condition limitation on the critical illness benefit only. Q: Is maternity covered by the Limited Benefit Health Insurance? A: Yes, maternity is covered as any other condition. Q: Will insureds receive an ID card? A: Yes, personalized, plastic and wallet-friendly ID cards are sent to each insured person. The fulfillment kit also contains phone numbers, web links and information on how to use all the benefits included in the Flex Shield Plus program. Q: Can the insureds use any doctor or hospital with the Flex Shield Plus plans? A: Yes, they may go to any doctor or hospital. However, they can receive discounts for covered medical visits when they visit a participating provider in the nationwide network included with the plan. Q: Regarding the Limited Benefit Health Insurance benefit, is there a copayment amount? Is there a deductible? A: There are NO copayments or deductibles associated with the Limited Benefit Health Insurance. Q: How do insureds pay for doctor visits or file claims? A: at the time of a visit, they should present their ID cards to providers. The ID card has all the information the provider needs to verify benefits and file claims. The providers may require the full amount due at the time of service if insureds are filing their own claims. There are no claim forms necessary. The insureds or their providers should simply send itemized statements, detailing their medical visits, to the claims address printed on the ID card. Q: What if an insured needs to go to the doctor and either has lost their card or has not received them yet? A: If coverage is in effect and the insured does not have their ID card or has lost their card, contact Harrington Health, administrator for this program at: Harrington Health will provide verification of coverage to the hospital or doctor and the information necessary to process the claim. Q: How do the discount programs work? Do insureds need to file a claim? A: With Flex Shield Plus Value-Added Non-Insurance Services including prescription, vision, dental, etc., there are no claims to file. The discount is provided at the time of service. So it is important that the insureds present their cards during their visits. Q: When can insureds begin to use the discount non-insurance services?
