OAEC Summer Camp offers unique educational programs that stimulate cultural exploration through fun-filled recreational activities. From Chinese language, art, dance, musical instrument, theater arts to Kung Fu. Our program is one that will fill your children s summer with excitement while promoting a deeper understanding of Chinese culture and art. Through the OAEC, your children s skill level will be greatly enhanced, and this amazing program will motivate learning and building self esteem, aiding your children on their journey to self-fulfillment. They will have a great time with peers and make lasting friendships while doing it. Our summer camp is tailored to all students with or without prior Chinese culture exposure. Our experienced teachers will use creative and unique educational techniques to make this summer camp a fun learning experience that our students will never forget. Chinese Poem Reading: Will introduce to students some classical pieces which are widely known in China. Chinese Language: Will focus on written and oral practices on vocabularies, sentences and dialogues. Chinese Theater: Students will learn and practice rhymes, tongue-twisters, poems, ballads, and etc. Center Time for Chinese: Students will sit in a circle and practice more Chinese by playing games and participating in role play activities. 4 Website: Tel Summer Camp 2014 Contract Form June 9-27 By signing below, I understand this contract is between the Oriental Arts Education Center and the person submitting the registration for the Summer Camp days/hours of Monday-Thursday 2:00 PM- 6:00 PM, and Friday 7:15 AM- 6:00 PM for the dates of June I agree to the contents of this registration packet and agree to the following for the Summer Camp Disregard of this contract and/or the contents in the registration packet will result in immediate dismissal from the camp. I understand that I am enrolling only for Summer 2014 ONLY. CLASS TIMES: I understand that I may register for one week at a time. PAYMENTS: (Please initial each item below on the fine) Payments MUST BE SUBMITTED BY CHECK OR MONEY ORDER ONLY made out to OAEC. Credit Card and Cash is not accepted. The entire payment for each week I sign up for is due upon registration. I will be assessed a one-time registration fee of $15.00 if I submit my registration by April 11 and a one-time registration fee of $25.00 if I submit my registration after April 11. I will be assessed a $35.00 check return fee for each returned check and all future payments will be accepted only via Money Order. I also understand that I must pay the returned check in the form of 2 money orders: 1 money order to replace the returned check and a 2 nd Money Order for the check return fee. Cancellation Policy: There is no proration and no refunds on a weekly basis. SIGNING CHILDREN OUT: ONLY individuals listed on the Authorization Form will be allowed to pick up your children. Failure to sign the student out from the school will be regarded as an Incident and may result in suspension from the program. ONLY adults 18 years or over are authorized to pick children. SHOWINg PHOTO ID IS MANDATORY UPON REQUEST. LATE PICK UP: If my child is picked up after the registered class time ends, I understand I will be assessed a late pickup fee of $2.00/minute. After the 3 rd late pick-up I understand my registration may be cancelled and I will not be refunded. The official time for our program will gauged by the clock located in Pick UP Location in the school. Please Note: If late pick-up fees are not paid at the time of pick up and no later than the following morning at the time of drop off, your registration will be cancelled and will not be refunded. (All pick up fees must be paid by Check or Money Order only) TRANSPORTATION: Students will not be allowed to leave the campus without the supervision of an Authorized Adult listed on the form. Children will not be allowed to walk home unsupervised. OAEC buses will pick up at MCLIMS parking lot by 2:15 PM on Monday to Thursday and return 5:30 PM at MCLIMS parking lot. On Fridays,, OAEC buses will pick up students by 7:15 AM in the morning and will be transport back to MCLIMS parking lot by 5:30 PM on Friday. BEHAVIOR EXPECTATIONS: In order to ensure the safety of all students note: Services will be suspended should disrespectful or unsafe behavior or conduct be directed towards the school premises, a student, a staff member, either by a parent of a student or student enrolled in the Summer Camp. A Zero Tolerance Policy will be enforced. CLASSROOM POLICY: All girls must bring pink leotards and white tights and pink ballet shoes. All Students must bring and prepare a recorder every day. FIELD TRIP: All students must wear camp uniform for child safety. SPECIAL NEEDS: We do not provide staff trained to deal with special needs (i.e. medical, behavioral, etc.) All information published in this registration packet applies to this contract. In addition, I understand and agree to adhere to any guidelines amended to this contract and that I will be notified in writing by the school officials. I understand that my address and/or phone number(s) may be given to the Summer Camp Curriculum Director, OAEC, should it be necessary to contact me. 5 Website: Tel BUS RELEASE PERMISSION FORM TO WHOM IT MAY CONCERN: I hereby grant permission for: Student Name to ride on the OAEC buses from MCLIMS parking lot by 2:15 pm on Monday through Thursday and return at 5:30 pm to the MCLIMS parking lot. On Fridays, OAEC buses will pick up students by 7:15 am in the morning and will transport back to MCLIMS parking lot by 5:30 pm Friday. I understand that this is a school-sponsored trip and all school rules and guidelines apply. I understand that all students participating in this trip will be responsible in conduct to the driver and to the teachers or adult sponsors at all times. It is further understood that students are required to go and return from this event on the transportation provided, unless prior arrangements have been made.
