1 St. Joseph Health System-Sonoma County STAFF POSITION DESCRIPTION Position Title: Licensed Vocational Nurse I Exemption Status: Non-Exempt EEO Classification: Professionals Job Code: Department: Patient Care Services ARU Acute Rehab Reports to: Nurse Manager Last Update: 08/00; 12/05; 04/07 Job Summary: The LVN is responsible for providing interpersonal, technical and manual nursing care, within the scope of practice as outlined by the California Board of Nursing, to patients under the direction of a designated Registered Nurse. Demonstrates competence in community orientation and stewardship. Essential Functions: Describe the 5-10 duties / responsibilities that must be performed by this position. 1. Assists RN with accurate collection of patient data to include identification of normal/abnormal parameters based on developmental age of the patient.
2 2. Administers all medications and treatments (within scope of practice) adhering to hospital policy and procedures and accepted standards of care and practice and department guidelines. 3. Documents data collected, care and treatment provided in a clear and concise manner. 4. Identifies need for and assists with patient/family education. 5. Communicates effectively with RN, patients, families, and physicians providing necessary information in timely and concise manner. 6. Consistently demonstrates efficient use of time and resources within established budget parameters. Actively participates in identification and implementation of cost saving strategies. 7. Provides patient comfort and privacy during all interactions. 8. Demonstrates accountability for apprising self of hospital and department communication/information. 9. Demonstrates commitment to department objectives and standards. 10. Maintains safe environment by adhering to all hospital and department policy and procedures. Identifies and reports unsafe conditions. Other specific competencies that have been identified by the manager related to changes in departmental operations that focus on current clinical practices and are listed on the Supplemental Competency. Additional Responsibilities: List infrequently performed and non-essential tasks. 1. Utilizes chain of command to communicate issues requiring resolution. 2. Performs other duties as assigned. Knowledge / Skills / Abilities: List the knowledge, skills, and abilities required to perform the essential functions of this position. 1. Able to communicate effectively in English (written and verbal). 2. Effective skills in communication, problem solving, interpersonal relationships, and prioritization. 3. Has the flexibility to adapt to new and challenging situations. 4. Has the ability to think critically. 5. Able to give constructive feedback and teaches staff and patients. 6. Basic computer skills.
3 Age-Related Competencies: Human development knowledge / skills: Demonstrates the basic knowledge and skills necessary to provide age-specific care to patients served, including knowledge of the principles of growth and development and psychosocial characteristics and interventions for the populations served in the department. Age-specific patient needs that employee is required to understand and meet: Demonstrates appropriate communication skills for the patient population served. Additional requirements: Demonstrates knowledge of age-specific community resources. Information Management: Treats all information and data within the scope of the position with appropriate confidentiality and security. Other Requirements (Optional): Minimum Position Qualifications: List the minimum qualifications required for this position: License / Certification: Current, active LVN license in State of California BLS within 60 days of hire Experience: Preferred Position Qualifications: List the preferred qualifications required for this position: Six months experience as an LVN in a health care setting. Environmental Conditions: May include exposure to potentially hazardous materials, infectious agents, contact with patients, public and staff.
