2 Bringing healthcare home Access Homehealth (Access) has been supporting people across the country since 1927 when the bush nurses provided homecare services. In those days much of New Zealand was rural and our founders vision was to bring quality home-based care to all parts of the country. Today we still uphold that principle, with highly-skilled staff supporting over 16,000 people in their own homes nationwide. Sitting at the forefront of the homecare sector, we work with the Ministry of Health, District Health Boards and ACC to provide a full range of services. 3 We re always here for you If you ever find it a challenge coping on your own or relying on family and friends, it s good to know that someone is here to offer you a hand. Whether you require nursing, personal care, or even just some help around the house, Access is dedicated to giving you the care and support you need to live independently in your community. We start by getting to know you When you get in touch, the first thing we talk about is you. Often the people we work with are either: disabled rehabilitating following an injury or accident recovering from an illness or hospitalisation over the age of 65 suffering from a long-term condition, or in the final stages of life. Once we have a good understanding of your needs, we draw on our skills and experience to ensure you receive the support that works best for you, your family and whānau. Providing care and support for independent living We always put you first. This involves doing whatever we can to help you achieve your goals, while not forgetting the importance of basic things like good timekeeping and a friendly face. It also involves recognising your cultural and ethnic needs, as well as respecting your privacy and dignity; ensuring that you never feel uncomfortable or embarrassed. 3

4 A full range of services delivered to your home Our skilled team of nurses and support workers provides a range of services, including: Personal care You can depend on us for everything from dressing, bathing and grooming to helping you to get in and out of bed. Nursing services Our fully-qualified nurses can assist you with wound care, medication management, continence management and other duties. We can also support you while you get back on your feet after surgery or an illness. Other clinical services Our care extends to other clinical services such as physiotherapy, occupational therapy, speech therapy and social therapy. Household assistance We can help make you comfortable in your own home by assisting with chores such as cleaning, cooking, shopping and childcare. 4

5 Socialisation As well as helping you to maintain your independence at home, we offer you the support you need to continue doing the things you love. Carer relief We can provide temporary in-home care to give you and your primary carer a break. Palliative care Our palliative team helps people who are at the end of their lives to die with dignity in their own homes, in accordance with their wishes. Restorative support Specialised services that support people to live life to the fullest, by helping identify goals, and be as functional and independent as possible. 5

6 How do I get started? It s easy. All you need to do is get in touch. We ll do the rest. Step 1 Get in touch You can do this in one of three ways: talk to your GP and they can refer you for home-based healthcare if you have an ACC contact manager or case manager, ask them about putting you in touch with Access call us directly on 0800 AT HOME ( ) and we ll talk about the best way to access the care you need. If you re not entitled to funded care through the Ministry of Health, a District Health Board or ACC, we can talk about other options, including private care. Step 2 We ll visit you After we re appointed as your provider, one of our community nurses will visit you at your home to get to know you and assess your situation. You re welcome to nominate a family or whānau member or friend to help you talk with us. 6

7 Step 3 We ll put together a Support Plan Once we have a good understanding of what your needs are, we ll work alongside your GP and/or other health agencies to determine the steps required to help you maximise your independence. We call the result your Support Plan. Step 4 We ll appoint a support worker to you We ll match you with the support worker who is most suited to meet your needs (and lives within reasonable distance from your home). If required as part of your Support Plan, we ll also match you with a nurse and/or any health workers you may need. Step 5 Maximise your independence Your support worker will get to know you, care for you, and work with you to help you plan and achieve your goals. At the same time, you can rely on the support of our nationwide Access network you can talk to us any time between 7am and 10pm, 7 days a week on 0800 AT HOME ( ). 7

8 How are these services funded? When you are referred for home healthcare and support you will also be assessed for public assistance from one of the following sources: aged care services are managed and funded in New Zealand by District Health Boards (DHBs) on behalf of the Ministry of Health Disability Support Services (DSS), part of the Ministry of Health, is responsible for the planning and funding of disability support services in New Zealand community nursing services (also called district nursing) are administered in New Zealand by DHBs and ACC the Accident Compensation Corporation (ACC) provides support if you ve been injured by helping you get back to work and everyday life as soon as possible. If you are not eligible for public assistance, there are other options, such as: you can pay privately you may be eligible for care to be paid for by another agency e.g. another Government department, a charity or a health foundation or society if you hold private medical insurance, your insurer may pay for home-based rehabilitation services and support. 8

10 ACC home-based recovery services Access is one of only four national providers of Home and Community Support Services for ACC, which includes personal care and household assistance. We also provide a variety of home-based nursing services for ACC clients including: wound care stomal therapy and catheter care continence management medication management and oversight How do I get started? It s easy. Once you have advised ACC you have nominated Access to provide your care, they will provide us with your details, and we will do the rest. 10
