1 Absentee and early voting information A concern for many patients is not having the chance to vote on Election Day because of a hospital stay. Please remind patients of their options of absentee voting and early voting. If you are registered to vote by Oct. 10, 2008, you can do absentee voting. If you are NOT registered to vote by Oct. 10, you must go to an Absentee One-Stop Voting Site in your county between Oct. 16 and Nov. 1. You may vote ONLY at a One- Stop Absentee Voting Site in your county of registration during the One-Stop Absentee Voting period and NOT on Election Day. Absentee voting Any registered North Carolina voter can vote by absentee ballot. An absentee ballot can be obtained between Sept. 15 and Oct. 28, It must be received by 5 p.m. the day before the election (Nov. 3, 2008). There is no official absentee voter request form for North Carolina. To request an absentee ballot, send a handwritten request to your County Board of Elections by the last Tuesday before the election (Oct. 28, 2008). The request must include: Your name, your residential address, your mail address, your date of birth, and your signature. You must return the completed ballot to your County Board of Elections by 5 p.m. the day before the election (November 3 rd, 2008) in order for it to be counted. If you are unable to make your own request, you may have a near relative do so. (A near relative, as defined by law, is a spouse, brother, sister, parent, grandparent, child, grandchild, mother-in-law, father-in-law, daughter-in-law, son-in-law, stepparent, stepchild, or verifiable legal guardian.) Each request must include the information specified above, and be signed by the voter or the voter s near relative. It is helpful to include the voter s date of birth, and if the request is by the near relative, the near relative s address and relationship to the absentee voter. If you are voting for the first time in North Carolina and you did not meet the ID requirements when you registered to vote, then you must include a COPY of a current and valid photo ID that shows your name OR a current utility bill, bank statement, government check, paycheck or other government document that shows your name and North Carolina address. 6 Absentee One-Stop (Early Voting) One-Stop Absentee Voting (Early Voting) begins the third Thursday before the election (October 16, 2008) and ends the Saturday prior to the election (November 1, 2008). Any North Carolina resident - registered or not - may vote during this period. To use this process, a citizen must: (1) Go to a One Stop Voting Site in your county of residence during the One Stop Absentee Voting period, (2) Fill out a voter registration application, and (3) Provide proof of residency by showing the elections official an appropriate form of identification with the citizen s current name and current address. The new registrant may vote ONLY at a One-Stop Absentee Voting Site in the county of registration during One-Stop Absentee Voting period and not on Election Day. Acceptable forms of identification include: A North Carolina driver s license with current address A utility bill with name and current address (A telephone or mobile phone bill, electric or gas bill, cable television bill, water or sewage bill) A document with name and current address from a local, state, or U.S. government agency, such as: (passport, government-issued photo ID, U.S. military ID, a hunting/fishing/or gun license, a property or other tax bill, automotive or vehicle registration, certified documentation of naturalization, a public housing or Social Service Agency document, a check, invoice, or letter from a government agency, a birth certificate) A birth certificate A student photo ID along with a document from the school showing the student s name and current address A paycheck or paycheck stub from an employer or a W-2 statement A bank statement or bank-issued credit card statement Your registration application cannot be fully processed until you provide proof-ofresidency.

7 Registered Voters: Any registered North Carolina voter may choose to vote in person using One-Stop Absentee Voting, which begins the third Thursday before the election (October 16, 2008) and ends the Saturday prior to the election (November 1, 2008). Simply go to a One-Stop Voting Site and indicate that you wish to vote Absentee One- Stop. You will have to provide your name, home address, and complete an absentee ballot.
