We are headquartered in Franklin, Tennessee and have more than 700 therapists operating in 13 states. We provide this management in all rehab venues including Inpatient Rehab Units ( IRU or IRF ), Skilled Nursing Units ( SNU, SNF, or swing beds), outpatient departments, and inpatient acute. 4 Inpatient Rehab Units focus on functional independence Rehab Management in the IRF: Therapy in your IRF requires team collaboration, patient education, and participation while focusing on functional improvement in a medically complex patient population. We understand the importance of the interdisciplinary process, delivering the required intensity of service and regulatory requirements driving service delivery. Focus on Quality Measurement and Outcomes: We work with our customer s IRF nursing team to become fully integrated into our customer s care management process. We collaborate with physicians and care managers to manage utilization and length of stay. The focus on quality measures and outcomes drives the need for therapists to participate in our customer s quality initiatives. Our therapists are involved in specific initiatives to drive clinical outcomes, such as lift teams, fall prevention teams, and transport teams. We partner with our customers to provide a specialized team with dedicated tools. IRFs serve medically fragile patients who require an intense level of physical, occupational, and speech therapy with the prevailing goal of returning to their community with the highest level of independence. We have proven experience in the implementation, expansion, and enhancement of: Census development; Clinical programming; Improved FIM scores; Regulatory compliance; Operational management; and Patient scheduling strategies.
5 Skilled Nursing Units meeting maximum individual needs Rehab Management in Skilled Nursing: partners with customers offering skilled nursing care as a part of the post-acute continuum. We often see that therapy services, care coordination, and nursing are often fragmented. We bring a strategic approach to the SNU, working in concert with the nursing and physician care teams to align interests. Commitment to Meeting Individual Patient Needs: We recognize the transition from home or hospital to a care facility can be difficult. Ensuring that patients are engaged and that their care is coordinated well across settings is our primary focus. The RUG system is not based on patient diagnosis or discharge location, it is based on the amount of time a therapist spends assisting a patient in meeting their individual goals. We train therapists to understand this dynamic and work seamlessly with the nursing team to deliver value-based care. Optimize Business Results: Skilled nursing patients in the hospital environment are most often a higher acuity and medically complex. Therapists working in this environment receive functional training that prepares them to better serve the specific population and understand the service delivery requirements. Accurately assessing the care needs of patients upon admission assures they will be assigned to the most appropriate level of care. Accurate coding of the ADL Index plays an important role with 5-Star Quality Measure scoring and reimbursement. 8 Inpatient Acute promote patient mobility Rehabilitation Management in Acute Care The role of therapy in acute care is to facilitate patient movement through your hospital, easing patient transition to the next care setting, and working to keep patients from readmitting unnecessarily. We strive to accomplish these goals while also winning physician and nursing confidence in our service delivery. We take an active role in supporting initiatives, such as the Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS), which help our hospital customers maintain market share and avoid losing reimbursement.
