2 A BIT ABOUT US Trafford is a metropolitan borough of Greater Manchester with an estimated population of about 211,800 in It covers 41 square miles THT Create 46m social value annually through Landlord with 9000 Homes &18000 customers Developing 600 homes over 3 years Range of care and support services including telecare, meals, home care Invest 750k annually in the community Trafford Council spending will be 167m Since 2010 found 96m savings with a further 20m required next year 51% - Children, Families & Wellbeing services 22% - Economic Growth, Environment & Infrastructure 16% will be used on council-wide budgets 11% will be used on Transformation & Resources Focus on economic growth and investment libraries, 5000 homes in Carrington, investment in town centres

3 WHAT S IT LIKE IN OLD TRAFFORD? Old Trafford and the electoral wards comprising Old Trafford are in the top 10% of most deprived areas in England Lower life expectancy in both men and women High levels of childhood poverty and obesity High prevalence within the BME population of developing long term conditions such as diabetes and heart disease High incidence of lung alcohol and smoking cancer associated with Poor uptake of screening, disease prevention and early diagnosis with significantly higher than average levels of referrals to secondary care, non-elective (emergency) admissions and prescribing rates which have significant impacts upon Primary Care resources

5 OLD TRAFFORD VISION In 2008 The Council and The Trust decided to work in partnership to develop the Old Trafford Masterplan which aimed to "create a vibrant, sustainable community, with quality housing, excellent environment, and good connections to employment opportunities together with improved retail and community facilities"

8 WHAT DOES TRAFFORD S CCG WANT? Consistently achieve local and national standards Increased delivery from primary and community services Reduce gap in health outcomes between most and least deprived communities in Trafford Ensure a financially sustainable health economy TCCG has, as a Level 4 Co-Commissioner, committed to develop and improve integrated pathways of care based upon a hub model and which the Shrewsbury Street Village Project will form the north hub. The Shrewsbury Street Village Project is being viewed as the key element to the delivery of TCCG strategies within the north of the Borough and Old Trafford.

9 WHAT DOES PUBLIC HEALTH WANT IN TRAFFORD? Promote individual lifestyle changes by offering a choice of wellbeing services NHS Health Checks, sexual health services, drugs and alcohol advice Engage with and encourage communities which are less likely to access services Ensure individuals and their families are engaged and make informed decisions about participation in national screening programmes and to ensure inequalities are addressed Promote wellbeing by treating individuals holistically in both mental and physical ill health Promote effective self-management for people who experience long term illness Improve the management of co-morbidities by addressing the causes of early mortality
