I was adjunct faculty with Polk State College for 10 years and adjunct faculty with Hillsborough Community College for 3 years. I have served as a preceptor for many students at the ADN level to the DNP level. I served as the Stroke Program Coordinator with a focus on community and population health for 4 years. Over the course of my career this far I ve held positions such as Patient Care Clinical Team Leader, Charge Nurse, and Staff Nurse. I am a certified CPR trainer and provider. I have a diverse healthcare background in Disease Specific regulatory compliance, quality improvement, practice standards, HIT integration, patient safety, staff development and inter- professional collaboration. As a leader, I have the necessary skills to operationalize complex processes of patient care delivery, implement quality and safety improvement strategies to improve patient care experiences as a healthcare systems leader. I led the stroke team to excellence in key strategies to improve patient safety and clinical practice by reviewing; analyzing and interpretation of data for Target Stroke and Stroke get with the Guidelines benchmarks. In collaborated with HIT to develop and implement a new swallowing screen practice change policy at LRH on paper as well as in the electronic health record. This was one of the stroke program s focus areas for quality improvement. I was a reviewer for the Core Curriculum for Medical- Surgical Nursing, 5 th Edition and have worked many years as a medical surgical nurse education specialist at Lakeland Regional Health. My goals and aspirations are to facilitate learning and guide nursing students development to impact best patient health outcomes leading to a health community and population. Currently I am the Instructor for the Foundational Course, the Capstone and I am clinical faculty for the BSN psych rotation. I build, assess and revise curriculum, plan courses, included technology for blended learning. I collaborate with my dean, peers and other content experts as needed. I assess the needs of the learners and plan strategies to meet learning needs. Working with other instructors I plan future class schedules keeping in mind the needs of the students. I work one on one with students in the position of Advisor assisting to plan for degree completion ensuring they are on course for graduation. Nursing Practice, Lakeland Regional Health, Lakeland, FL Stroke Program Coordinator Department of Professional Nurse Practice Summary of job duties The role of the Stroke Program Coordinator include: Preparing the organization for the Primary Stroke Center Accreditation and Bi- annual Joint Commission Certification and Mid- cycle Joint Commission Survey, Coordinator and facilitation of the Stroke Program Steering Committee meeting, which is an interdisciplinary administrative group that includes AVP, s ARNP, Physicians, Directors, Managers and direct care staff where the stroke program data is shared and opportunities for improvement and action step decisions are discussed and made. I abstract some of the data, review and follow up on core measures and stroke data as mandated by the joint commission on care of the stroke patient. I also present stroke data at the Neurology Service Meeting and provide feedback to the physicians related to stroke patient door to needle responses and stroke

4 alert responses. This is a large piece of the role of stroke program coordinator. I coordinate and often facilitate the Stroke Victors and Caregivers Support Group monthly. This community group meeting engages a diverse group of speakers and facilitators who are able to touch and connect with the stroke victors and their families building trust and caring relations. I provide ongoing needs assessments and education for the stroke program core group of nurses on B4 as well as nurses and rad techs in radiology and other areas of the hospital such as ICU s, and the ED that touch stroke and care of the stroke patient. A part of my role is assessing acute stroke patients and making recommendations for patient and caregiver education and post discharge care. I m a coordinator and organizers of community education and health fairs on stroke prevention measures and stroke risk factors; Coordinator of grand- rounds with medical director of the stroke program, lightening rounds and daily rounding on patients for the stroke unit, working collaboratively with the ED, EMS and community facilities to provide evidence based practice for best patient outcomes, provide and advocate for preventative stroke education for the community at the local, regional and national levels. Collaborate with the stroke center medical director to develop review and revise policies for the stroke center and stroke center patient care. Develop and provide annual budget for stroke program financial needs. And, other duties and jobs as assigned Education Specialist Department of Hospital Education The role of Education Specialist include; Educator for Cardiac Center, Chest Pain Center support and program development, Training Center Coordinator for Certified BLS instructors and BLS classes. I am the program coordinator for new products, policy changes, and procedures. The Education Specialist is responsible for in- house marketing and information dissemination as well as external marketing of program through CE- broker. I am a consultant for the development of an education Skills Lab for new hires and current staff. I develop curriculum, construct and facilitate continuing education programs in the hospital, community and statewide program coordination. In Collaboration with additional Educator provide annual mandatory education development for medical surgical units, ED, OR, Stroke, ICU s as well as urology and gynecological units. I provided for and facilitate house- wide needs assessments with all clinical managers to meet the requirements of The Joint Commission for Stroke, Trauma, Chest Pain Center, CAP and other accrediting and governing bodies Manager of Hospital Education /Human Resources Department My job role as Manager of the Education Department was to oversee the work of a team of 21 interdisciplinary staff members including Registered Nurses, Advance Practice Nurses, Educators, Physical therapist, Respiratory Therapist, Cath lab tech and Training Coordinators. I provided interviews, ongoing staff counseling, annual performance evaluations and appraisals, professional development, leadership and guidance in the development of orientation and a nurse residency program, preceptor program for clinical as well as non- clinical staff. Managed yearly budget and purchasing for department. Consultation and learning plan development for staff and managers. Partnership with managers to complete needs assessments and set learning needs priorities. I was instrumental in the development of on- line education programs. I facilitated training for students entering the hospital setting in collaboration with local colleges of nursing. I served as Education representative for Shared Nursing Governance and took part in the development of the nursing strategic plan. I engaged in implementation of strategies to meet the annual strategic plan and improve HCAP and Gallop scores at LRH and within the department Clinical Learning Specialist- Team Leader As Clinical Learning Specialist and Team Leader a part of my role was to develop strategic ways to move the clinical learning team to a higher level of learning and teaching adults in an environment conducive to adult learning while integrating adult learning styles. I addressed the needs of the clinical learning team to assist in

5 producing a team that delivers quality adult education on a continuum across the institution to all patient populations including urology, nephrology, endocrinology oncology, and gynecology. I facilitated revisions of clinical staff orientation processes which enhance staff satisfaction and retention. I developed the medical surgical certification program and facilitated bringing in a national speaker to present the program. I also took part in writing and developing the PCCN hospital based certification program and module for certificate preparation. I developed and presented multiple Developmental programs for the Learning Team at LRMC as well as other hospital staff who wanted to attend the programs Clinical Learning Facilitators Functioning as a clinical expert in the art and science of medical and surgical patient care, a part of my job role was to meet the needs of staff as Educator of new hires and current staff on surgical unit that included urology, gynecology and general surgeries. Some of the other job task included planning, developing, and implementing new and strategic ways to educate staff. Meeting and negotiating with representatives and vendors from large companies that supply new products and safety devices to the hospital to provide expert feedback and being a voice for bedside nurses to meet the needs of staff for best patient outcomes. Plan, implement oversee, and evaluate house wide education of safety and new products. Other aspects of my role included meeting with managers to discuss new staff member s orientation progress and needs. Planning, development, implementation and evaluation of education programs were a part of my role. I implemented utilization of the theory and process of andragogy when working with preceptors assisting them with skills to allow new staff to learn guiding them as they develop preceptor skills. I was instrumental in providing self- development sessions for supervisors RNs, and PCAs to define role clarity Team Leader/Charge Nurse My job role as team leader included team leader for surgical teams, staffing, scheduling, and facilitator of staff meetings. I provided counseling, conflict resolution, learning plan development, goal setting, mentoring, coaching, role modeling, preceptor, resource person, interim and annual staff evaluations. I was a provider of holistic patient care including stroke, renal failure, and urology pre and post operatively, gynecology, medical surgical and trauma patients. I provided discharge planning and home care education for patients and care givers. I partnered with physicians, physical therapist, social services and dietician for grand rounds and patient rounds for best patient outcomes. I was Responsible for the operation of the unit from the charge nurse prospective Summary of Job duties: Staff Nurse Urology/Endocrinology Unit My job role included caring for patients in collaboration with the urologist and endocrinologist during a time in the hospital where a model for cross training in patient care was taking place. Part of my job role was caring for patients with post urological procedures such as cystoscopy, super public catheters, 3 ways irrigations, leg bags, penial implants, ostomies, self- cauterization, patient and family teaching post operatively. I provided care for patients post TURP, patients with prostate cancer and bladder cancer and 3 way irrigation. I cared for patients post hysterectomies and other gynecological surgeries. I also cared for patients with new onset of diabetes teaching them and their families how to care for diabetics Nurse Intern My role as nurse intern was to follow and shadow a registered nurse to learn and get as much experience with bedside patient care in the clinical setting as possible.
