3 providing successful outcomes for adults At tracscare, we are committed to providing successful outcomes for younger adults who may challenge existing service provision. We have over 25 years experience of supporting vulnerable adults with challenging or complex needs who may have experienced difficulties in finding suitable services to meet their needs. Korsakoff s Syndrome Personality Disorder Support for adults with Challenging Needs We have extensive experience of working with clients who may have varied forensic backgrounds often associated with complex needs including mental health and past drug or alcohol abuse. We also work with clients who have previously been detained under section of the Mental Health Act. We are a specialist provider of care and rehabilitation, providing clients with individual care packages and have the experience and understanding to provide positive outcomes for our clients. Our average length of stay for clients with mental health needs is less than two years. Every year we step clients down into more independent living and we publish an annual outcome report showing that the majority of clients progress towards their individual care goals. Care Pathway Acquired Brain Injuries Mental Health Learning Disabilities with Challenging Needs Autistic Spectrum Condition We can provide individuals with appropriate levels of support through various care packages, according to their stage of rehabilitation or level of independence. Each clients care package is person-centred and designed to help them reach their individual goals. We use a clear costing tool, which we share with purchasers and we are always ready to seek commercial solutions to fit with our first class care outcomes.

4 Our support is focussed on a recovery model of care and avoids institutional-based practice. MENTAL HEALTH We are able to support clients with mental health needs in residential care homes, nursed care homes and supported living schemes across England and Wales. What makes us successful? It is generally accepted that individuals with serious mental illness are amongst the most socially excluded people within our society. ACQUIRED BRAIN INJURIES Our services across England and Wales are client focused and dedicated to supporting the complex needs of individuals with an acquired brain injury, who may have a physical disability or who are mobility impaired and may have associated neurobehavioural needs. What makes us successful? We offer a comprehensive ABI assessment We have our own clinical team of Qualified Nurses and Behavioral Advisors We have developed our own Positive Steps and Thinking Ahead groups We provide an individual life skills assessment We provide a focus on neuro-rehabilitation We provide compensatory strategies Meeting Complex Needs We are able to support clients who have a forensic background are stepping down from a Neuro-rehabilitation unit may be under section of the Mental Health Act are moving on from long term intensive rehabilitation All clients who come into our care undergo a comprehensive assessment of their Acquired Brain Injury, to establish the cognitive impairment that they are currently experiencing. The assessment of the clients abilities and deficits allows the care plan to be specifically developed and allows progress to be accurately measured. The development of the clients care plan follows the assessment and focuses on a program of activities that enables the relearning of previous skills and the acquisition of new skills where possible, or focuses on compensatory strategies to reduce the impact of these deficits upon the clients daily living. The aim of the assessment and care plan is always to enable the client to achieve as high a degree of independence as possible. Providing support for adults with acquired brain injuries associated with: Traumatic brain injuries Korsakoff s syndrome Mobility impairments Physical disabilities Early onset dementia Strokes Aneurysms Brain Hemorrhage Encephalitis Substance misuse We successfully run our own positive-steps and thinking ahead groups with a focus on neuro-rehabilitation. These groups are organised especially for clients with a brain injury and use cognitively based therapeutic group work with the aim of providing individual and group exercises which focus on dysexecutive syndrome, memory deficits and social functioning. Clients are encouraged to increase their levels of cognitive functioning whilst having fun at the same time. LEARNING DISABILITIES with Challenging Needs Within our homes there is a wealth of experience and expertise in supporting clients who have a dual diagnosis of mental health or challenging needs associated with their learning disability. Our homes undertake an in-depth assessment and analysis of challenging needs using the B-BAT tool and take a positive approach based on detailed Behaviour Support Planning. Each client is supported with an individual care plan designed to meet their needs. Our care homes offer a homely environment in different styled settings, within the heart of their local communities. This provides the clients with a peaceful environment but close to local facilities. The standard of care for adults with a learning disability is very high within our homes, delivered by a team of dedicated staff. we are committed to a philosophy of doing with rather than doing for clients, and promote the active support model. Our care includes an experienced team of staff, qualified or working towards NVQ level qualifications, a high staffing ratio in each home and a professional service with the client s needs, wishes and aspirations at the forefront of the care provided. Staff training is specific to each home, but based upon positive approaches to learning disabilities. Each Home is supported by a tracscare Behaviour Advisor who is involved in the development of the Reactive Management or Behaviour Support plan and supports staff to work positively when challenges arise. For more information on our support for adults with a learning disability, please call us on or visit AUTISTIC SPECTRUM Condition An Autistic Spectrum Condition is a genetic disorder where there are three defining characteristics, (the Triad of Impairment) Severe communication impairment Lack of imagination Limited ability to form social relationships Aspergers Syndrome is an Autistic Spectrum Condition with similar characteristics, which may include lack of physical coordination. It is generally considered to be at the higher end of the spectrum, particularly in terms of verbal communication and social functioning. For more information on our support for adults with an autistic spectrum condition, please call us on or visit Supporting clients with autism Our understanding of the triad of impairment enables us to fully understand autism, and how to deliver the best support to clients. Our homes offer a homely environment in different styled settings, far away from the noise and distraction of city life but well within the heart of their local communities, to provide the clients with a peaceful environment but close to local facilities and social interaction. Clients are provided with an Autistic Spectrum Condition sensitive environment, designed, to make them feel more comfortable. Facilities include sensory gardens, Snoezelen rooms, activity areas, and peace and quiet. Client care plans are centred on the Triad of Impairment. We utilise a specific ASC Assessment and Sensory tool to assess the impact of the triad on the client s daily functioning. This is Key to providing the right care to clients. Clients receive a structured routine based on their individual assessment. This includes autism focused activity programmes, behaviour therapy and reflexology support. Interventions are aimed to recognize and minimise anxiety related to the triad of impairments. All of our homes specialising in Autistic Spectrum Condition employ well trained staff with the knowledge and skills to manage adults manage adults and deliver the best care possible.

7 Nursing support for adults leaving hospital or secure accommodation. NURSE-LED THERAPY Forming a key part of tracscare s continuum of care, our nursing registered support can provide clients with a rehabilitative route towards residential care where possible, an opportunity to receive an extended assessment period as part of their transition to residential care or more independent living, support during relapse, therapeutic individual or group intervention, and long term support if required. Client care plans are based on a comprehensive assessment of individual needs and are bespoke to each client. A named Primary Nurse is responsible for the development of the individual nursing care plan in conjunction with the clients commissioning care team. The Primary Nurse is also responsible for regular Nursing Reports which communicate clinical information to the care team in relation to the client s mental health and general progress. Our Commitment to Recovery - the Rehabilitative Route Staff encourage individuals to see themselves as capable of recovery by providing them with the required support to improve all elements of their life which impact upon their mental well being. With a focus on mental health relapse prevention, risk management, independent lifeskills promotion and clinical therapeutic intervention, nurse-led therapy helps individuals to move towards reduced support and progression towards less support intensive environments. Continuing Care Nurse Support Our nurse-led therapy is suitable for: Clients leaving medium or low secure accommodation Clients relapsing from a residential care environment Clients on a care pathway to residential care Schizophrenia Affective or bi-polar disorders Anxiety disorder Obsessive / compulsive disorders Depression Suicidal ideation Self-harm or para-suicidal behaviours Personality disorders Eating disorders Mental health needs associated with substance misuse Addiction problems Mental health needs with a forensic background For clients with more enduring mental health needs, our nurse-led therapy provides nursing support over long term placements. With its focus on minimising the effects of mental health symptoms on the individual, relapse management, maintenance of life skills, and self advocacy, our nurse-led therapy aims to enhance the quality of life of those individuals it supports. Residential Care We are experienced providers of residential care and rehabilitation for adults with various challenging needs associated with their mental health needs, acquired brain injury, learning disability or autistic spectrum disorder. We are able to provide individual residential care packages based on individual clients needs, to help them progress and reach their potential. We aim to provide structure to individuals daily lives and encourage integration into their local community, participation of home activities and hobbies as well as maintaining relationships with family and friends. Our person-centred support considers an individuals every need, and aims to help clients fulfil their objectives from daily living skills to long term care objectives. We believe that providing personcentred support avoiding institutional practices gives each person the best opportunities to reach their potential. Our residential care homes are small community-based homes in locations with close proximities to activities such as shopping and local clubs, to give clients opportunities to increase their social interaction and self-confidence. SEMI INDEPENDENT LIVING Individual flatlets within a residential care environment, for clients seeking greater independence Independent living need not mean Isolation; we encourage individuals to explore opportunities for interaction with others and the chance to make new friends within the local community. However, individuals can enjoy the privacy and independence of living in their own home, safe in the knowledge that discreet help is at hand, if needed. A carefully designed support plan reflecting our clients individual needs, enhances their confidence, helps build life skills and ultimately, maximises working opportunities. For clients making the transition towards independence, but not ready to leave residential care, we have developed semi-independent flats. Semi independent living gives clients the chance for increased independence whilst maintaining the safety and security provided from a Care Home registration, with support from staff as required, up to 24 hours a day. Our semi independent living units are registered as a Care Home but comprises of individual flats each with secure access. The flats are furnished to a high standard, with a bedroom, bathroom, lounge and kitchen area, offering higher levels of independence and privacy not normally afforded within residential care. There is also a communal lounge to provide opportunities for socialising. harm, personality disorder, suicide, drugs and alcohol abuse. Semi-independent living is suitable for: Clients ready to increase their independence A transitional period moving from residential care to supported living Clients wanting to increase independent living skills Clients with a history of placement breakdowns in supported living Clients preparing to return home to their families Clients with mental health needs looking to regain social and living skills Clients with acquired brain injuries looking to regain social and living skills Support from an experienced staff team Each tracscare support worker has appropriate experience and is specifically trained. We encourage staff to further develop their skills through an ongoing programme of education and training. We rarely use agency staff but where this is necessary, we ensure they are skilled, experienced and safe to use. Every member of our support team is carefully screened through CRB and ISA checks and by other relevant in-house procedures. We have therefore developed a Supported Living Service that gives clients an opportunity to increase their levels of independence as part of a care pathway, and provides them fully trained staff support when required. Supporting individuals across the UK With service-provision across South and Mid Wales, the West Midlands, East Midlands, South West England and London, our support services have a wide reach for those seeking greater independence. Our services are continuously developing across the UK, and new individual services in other areas can be discussed and developed at anytime. Our packages are flexible, and designed according to each individuals needs, providing support for adults with the complex and challenging needs associated with: Mental health Acquired brain injuries Learning disabilities/difficulties Autistic spectrum condition Supporting individuals with complex and challenging needs Our staff are fully trained in providing support for adults with the complex needs associated with their diagnosis. Our support teams follow approaches developed by Andrew McDonnell including Studio 111, a low arousal and positive approach to managing challenging needs. tracscare s Behaviour Advisors were personally trained by Andrew McDonnell and his clinical team, and this positive approach to client support underpins the company s staff training and care philosophies. This commitment to quality assurance provides additional external auditing of our homes and policies which complements regulatory inspection. As a result of our commitment to quality assurance, tracscare was the first care company to achieve ISO accreditation. Our Commitment to Training & Development We have a commitment to all staff, and as such we are committed to training and developing staff. All tracscare staff receive regular training in line with the organisational values and strategic goals, to provide the necessary skills to undertake their roles. Many of our Home Managers commenced with tracscare as Support Workers, through our unique fastrac training and development programme. Staff receive in-depth training and on-going support on low arousal techniques, which are at the core of the tracscare care philosophy tracscare provides a range of internal training programmes including extensive induction; understanding the clients perspective; clinical induction and more. We also provide external training with NVQ s and all statutory training. One special feature of our training for all employees is positive behaviour management training, provided at induction, which outlines tracscare positive, low arousal approach to managing challenging needs.
