For the guaranteed issue amount, you don t have to answer any medical questions. You don t have to make any complicated decisions. Check the Guaranteed Issue Levels section at the end of this brochure for the guaranteed issue amounts that may apply to you and your family. Voluntary Group Life Insurance 1

3 Apply for an Amount That s Right for You You have options. We understand the guarantee issue amount might not meet your specific needs, so you can apply for higher levels of coverage: Voluntary Group Life is available in units of $10,000, from $10,000 to $300,000. Once you buy coverage for yourself, you can apply for coverage for your spouse, from $10,000 up to the amount you buy on your own life. You can apply for coverage for your eligible dependent children in amounts of $2,000, $5,000, or $10,000. All your children will be insured for the same amount. For all amounts over guaranteed issue, for coverage increases, and during an open enrollment period for member and spouse, you will need to provide satisfactory evidence of insurability. Waiver of Premium for Disability If you re under 60 years of age and become totally disabled, we ll waive your premiums after six months of total disability. At that time, we will refund any premiums you paid after the date your total disability began. Enroll Today and Benefit from Affordable Group Rates! 4 Here s a Summary of the Policy If your Voluntary Group Life application is approved, you will receive a certificate of insurance. Please read it carefully. The master group policy contains the controlling provisions of this insurance plan and neither this material, nor any certificate, may modify those provisions or the insurance in any way. Eligibility If you are an active employee working at least 30 hours each week, you become eligible for Voluntary Group Life on the first day of the calendar month coinciding with or next following 30 days as an employee. Temporary or seasonal employees or full-time members of the armed forces of any country may not become eligible. If sickness, injury, or pregnancy prevent you from working the day before the scheduled effective date of your insurance, your insurance will not become effective until the day after you complete one full day of active work. The same applies for an increase in your insurance amount. Reductions in Insurance There is a graded reduction in the Life Insurance benefit beginning at age 70. When Insurance Ends Your Life insurance will end automatically on the earliest of: the date the last period ends for which you made a premium contribution, the date the group policy terminates, the date your employer s participation under the group policy terminates, Voluntary Group Life Insurance 3

5 the date your employment terminates, and the date you re no longer a member of an eligible class of employees. Continuation of Life Insurance You may continue your Life Insurance if your employment with your employer terminates. To continue your insurance, you cannot be disabled. You must apply in writing and pay the first premium to us within 31 days after the date your employment terminates. The maximum amount you may continue is the amount that was in effect on the date your employment terminated. If you decide not to continue your Life Insurance, you may not continue insurance for your spouse or dependent children. Settlement Options Benefits payable because of your death will be paid to your designated beneficiary. Unless otherwise requested, Standard pays approved Life Insurance claims of $10,000 or more by depositing the insurance proceeds in a personalized, interest-bearing checking account. The beneficiary owns the account and is free to use it as he or she would any other checking account. Smaller claims will be paid in a lump sum.
