3 An integrated care facility designed to meet the needs of individuals and families through: Adult and Children s Behavioral Health Care recovery options for severe and persistent mental illness, chemical dependency and co-occurring disorders available to children and adults Primary Care support for managing chronic medical illnesses such as, diabetes, obesity, heart disease and other challenges to healthy living Wellness Support smoking cessation, nutrition/ dietary programs and physical fitness coaching to improve common chronic disease conditions On-site Health Professionals therapists, physicians, nurses, dieticians, exercise and tobacco cessation specialists, case workers 3

5 Available Services Wellness Services-programming, coaching and services provided by professional staff and specially trained peers to help people understand and successfully address chronic disease conditions associated with smoking, poor dietary choices and lack of exercise Telemedicine-increased prescriber capacity through telepsychiatry that allows psychiatric professionals to provide services anywhere in the city from a fixed location Mental Health First Aid-community mental health education available in English and Spanish that helps non-professionals identify, understand and respond to signs of mental illnesses and chemical dependency disorders; certified trainers will offer training certifications at the facility Peer Support Specialists-people who have experienced mental illness and are professionally trained to serve as a resource for adults to promote recovery Promotoras-Latino community members with specialized training who provide health education in community settings to increase access to services and ensure effective treatment; they are also liaisons between the community and health and human service organizations 5

Two community training rooms equipped with state-of-the-art technology will be available for the community to use for meetings, events and educational purposes. What medical professionals will be available in the facility?
