Doctor's Chair The unit intended for use in various location of the Emergency Department to use as waiting area chairs The unit shall have aluminium base with castors The unit should have synchro mechanism with armrest The unit should have seat in PVC Upholstery The unit should be in black colour Staff's Chair The unit intended for use in various location of the Emergency Department to use as waiting area chairs The unit shall have aluminium base with castors The unit should have synchro mechanism without armrest The unit should have seat in PVC Upholstery The unit should be in black colour. 2. General Requirements 2.1 Patient's Chair The size of unit shall be approximately 812mm (H) x 550mm (L) x 612mm (W) The internal clear width shall be approximately 525mm The safe loading capacity of the seat shall be approximately about 100 kg excluding the weight of unit itself The safe loading capacity of the back rest shall be approximately about 30 kg excluding the weight of unit itself The frame structure shall be constructed with durable steel able to resist to rust. All joints shall be welded and all exposed surfaces shall be straight and true to line and curve. The entire structure shall be coated with a layer of anti-corrosion paint follow by another 2 finishing coats either power coating or nylon polyester coating to withstand periodic cleaning and typical abuse. - Page 3 of 17 -
4 The unit should not be too heavy The unit should be able to stack up for space saving and easy to transferable The unit also must be sturdy and would not sink once sit upon. 2.2 Waiting Area Chair 2.3 Doctor's Chair The size of unit seat shall be approximately 480mm (H) x 570mm (W) The length of the unit shall be as follow: Two (2) seater 930mm Three (3) seater 1400mm Four (4) seater 1900mm Five (5) seater 2350mm The each seat length shall be approximately 450mm The spacing between each seat shall be approximately 25mm The frame structure shall be constructed with durable steel able to resist to rust. All joints shall be welded and all exposed surfaces shall be straight and true to line and curve. Standard Accessories 6.1 Patient's Chair The standard accessories shall include the following items and other items necessary required for normal operation and usages of the trolley. 9 7. Optional Accessories 7.1 All optional accessories or consumables that are optional shall be listed separately with unit prices clearly indicated. 8. Installation / Commissioning Requirements 8.1 The unit shall be fully assembled in the contractor's factory before delivery to site for commissioning. The Contractor shall arrange with the Hospital, Materials Management Department representative for delivery with ample advance notice. During the commissioning, the Contractor shall carry out the necessary functional tests in accordance to the manufacturer's recommendation. The Contractor shall provide all necessary testing equipment or required for the functional testes at their own expenses. No claim for extra payment will be entertained by the Hospital owing to the Contractor's neglect any unforeseen difficulties for which provision are not made for the commissioning. A Contractor, by the fact of submission of tender, shall be deemed to have accepted all conditions and stipulations of this clause, which shall be binding on the Contractor. 9. Additional Requirements (where applicable) 9.1 The supplied equipment and accessories must be of hospital-grade and shall comply with national and international recognised Standards and applicable Standard Systems. 9.2 The Tenderer shall provide test certificates from an internationally recognised testing body attesting to compliance with recognised standards. If certification for each stated compliance is not provided during submission, it shall be considered as non-compliance to the standard. 9.3 The Tenderer shall provide back-up units during the warranty period while the unit is undergoing corrective repair by the Tenderer. 9.4 The warranty shall cover unlimited breakdown service calls at no additional cost. 9.5 The Tenderer is advice to check for incompleteness and misleading information that may result in disqualification. 9.6 All terms and conditions which are printed on or referenced in the successful Tenderer's Bill of Sale or other sales documentation for the equipment to be purchased are null and void and are not binding to the Company. The Company will dictate all terms and conditions of the sale. 9.7 The Company will be entitled to purchase all replacement parts, components, sub-assemblies and peripheral devices as needed for the maintenance arid repair of each model of equipment purchased from the successful Tenderer at the fair market price. No excessive handling or shipping charges will be applied to these purchases. The successful Tenderer must expedite all - Page 9 of 17 -
10 shipments and not withhold shipments in order to increase equipment downtime to the Company or for any other reasons. 9.8 The Company has the right to use any service representative of his choosing, including in-house, third party or independent contractor. These representatives have the right to repair, install, calibrate, maintain or overhaul all models of equipment purchased from the successful Tender. The Company s representatives shall be afforded the privilege of ordering all necessary repair parts and components from the successful Tenderer for each model of equipment purchased at a fair market price. 9.9 In the event that computer software or external devices are required for the operation, calibration, or repair of the equipment, then the successful Tenderer shall make available to the Company any and all software and hardware at a fair price. Ail subsequent updates for the software must be provided at a fair price. The software may be in the form of ROM type memory, magnetic media, software transmitted via telephone, or any new formats not yet available that may be developed in the future. The Company has the right to use and operate all hardware and software for the purposes of operating, repairing, or calibrating the equipment. The Company has the right to allow her designated service representative to use all software for the repair and calibration of the equipment purchased The real time clock and the calendar of the unit/system should continue to perform and register the actual time, day, month and year in the 21 century. The output information from the interface port should indicate the same accuracy and performance The Company has the right to send its designated service representatives to the the manufacturer s service training school to receive sufficient, any or all, technical training to allow the representative to repair and calibrate the equipment purchased All documentation, software and manuals become the sole property of the Company The Tenderer is expected to successfully commission the Article one month from the date of delivery. Failure of which the Company has the right to return the Article to the Tenderer. No claim for payment will be entertained by the Company. A Tenderer, by the fact of submission of a tender, shall be deemed to have accepted all conditions and stipulations of this clause, which shall be binding on the Tenderer Upon sale or transfer of the equipment purchased within and/or outside of Singapore, the Company shall have the right to transfer any or all hardware, software, documentation and manuals to the new purchaser of the equipment The Tenderer is advice to check for incompleteness and misleading information that may result in disqualification All Tenderers are to comply with all requirements stated in the Company Standard Conditions of Contract SCC Failure to comply with any of the above requirements may result in the rejection of the offer. - Page 10 of 17 -
