Because it pairs nicely with your medical insurance but you don t have to have medical insurance for this plan to benefit you. Here s why: Let s say you are driving to work and get in a car accident resulting in an ambulance ride to the emergency room and admission to the hospital for a broken leg. Unum writes a check to you for $2,600. $100 for Ambulance; $150 for ER; $1,600 for broken leg; $750 for hospital admission. And remember, this coverage only cost you $3.78/week. You probably will pay over 2x that for lunch this afternoon. Why would you need Critical Illness insurance? Medical advances in the last 50 years have increased our life expectancy, and it is now much more likely that a person can recover from a critical Illness (like a heart attack, stroke or cancer). But there is still the matter of a recovery period, which can be lengthy (and expensive). Unum s critical illness benefit is a good news plan because it is about survival and recovery, and about having the money so you and your family can financially survive during a period of a critical illness. Unum s critical illness insurance provides a lump sum payment at first diagnosis of a covered critical Illness. It can be used however you choose for the expenses health insurance doesn't cover. A $5,000 benefit for a year-old, non-tobacco user is just $1.53/week. It is often used for medical deductibles, co-payments, out-of network costs, experimental treatments and other hidden indirect costs such as childcare expenses, loss of family member s income from time off, and travel to treatment centers for you and your loved ones. Why would you need Term Life insurance? Because life insurance is a product you buy not for yourself, but to help take care of those who depend on your income. The hardest thing a family faces is the loss of a loved one. Having life insurance won t make the loss any easier, but it can relieve some of the financial burdens during this very difficult time. For a year-old, cost for term life insurance is just $.25/week for a $10,000 benefit. That is just $1.00 a month. Also, as an employee, we are offering you at this enrollment only, the opportunity to buy up to a $100,000 benefit without evidence of good health. What does this mean to you? As long as you are an active employee during the enrollment and when the coverage becomes effective on 1/1, you could have a serious illness such as cancer and you are still eligible for up to a $100,000 benefit. Who s at risk? About one-fourth of all heart-disease-related deaths occur in men ages 35 to The estimated probability of developing cancer during a lifetime is 1 in 2 for men and 1 in 3 for women. 2 Every 26 seconds an American will have a coronary event, such as a heart attack or stroke. 3 Did you know? 7 Get the coverage you need Critical illness insurance is offered to all eligible employees ages 16 to 69 (64 in California) who are actively at work. Choose the benefi t amount that s right for you from $5,000 to $50,000, in $1,000 increments. Three reasons to buy this coverage at work 1. You get affordable rates when you buy this policy through your employer, and the premiums are conveniently deducted from your paycheck. 2. You own the policy so you can keep it even if you leave the company or retire. Unum will bill you directly for the same premium amount. 3. Coverage becomes effective on the fi rst day of the month in which payroll deductions begin. Employer-selected benefit option Your employer may automatically include this option as part of your benefi ts, for an additional premium. Health screening benefit rider This benefi t pays $50 per calendar year per insured individual if a covered health-screening test is performed, including blood tests, chest X-rays, stress tests, mammograms and colonoscopies. Eligibility begins 30 days after the coverage effective date. A full list of more than 20 covered tests will be provided at your enrollment. Employee-elected benefit option Your employer may make this option available to you to purchase, for an additional premium. Enhanced (two payout) plan This plan includes all covered conditions previously listed and can provide an additional lump sum benefi t if diagnosed with cancer and/or carcinoma in situ. Available family coverage Who can have it? Spouse rider Ages 16 to 64 with purchase of employee policy Child rider Available with purchase of employee policy for eligible children, stepchildren and legally adopted children, 14 days through 24 years (who are not married and are dependent on the employee for at least half of their support) What s the benefit amount? From $5,000 to $30,000 in $1,000 increments $5,000 or $10,000 one rider covers all children Policy provisions Reduction of benefits The benefi t amount for the employee and spouse reduces by 50% on the fi rst policy anniversary after the insured individual s 70th birthday, or fi ve years after the policy effective date, whichever is later. Premiums for the policy will not be reduced. If partial benefi ts for coronary artery bypass surgery or carcinoma in situ have been paid prior to the reduction of benefi ts, then the new benefi t amount will be calculated by applying the 50% to the benefi t amount reduced by the prior payout. The additional cancer and carcinoma in situ benefit No benefi ts will be paid for cancer or carcinoma in situ if the date of diagnosis occurs during the fi rst 30 days from the coverage effective date. 10 Get the coverage you need. Group Voluntary Term Life insurance is offered to all eligible employees. Choose the benefi t amount you need in $10,000 increments up to your plan s maximum. Three reasons to buy this coverage at work now 1. You can keep the coverage if you leave the company or retire, provided you aren t seriously ill. You re billed at home for the same premium amount as you would have at work. 2. Once you buy $10,000 of coverage, you can add to it each year without a medical exam. 3. You get more affordable rates for you and your family. Premiums are conveniently deducted from your paycheck. Benefits highlights you ll appreciate: Survivor Financial Counseling service Financial planning and counseling are available to you or your family members at no extra cost if you die or are diagnosed with a terminal illness. Accelerated (early) benefit If you become terminally ill with less than 12 months to live, you may get part of your life benefi t early, up to your plan s maximum. Early payout is deducted from the policy s fi nal death benefi t. Available family coverage (if offered by your employer) Who can have it? Spouse Available with purchase of employee coverage; your spouse may be required to answer a few health questions. What s the benefit amount? In increments of $5,000 up to 100% or 50% of employee benefit amount. This varies by state. How long can they keep it? Your spouse can keep coverage as long as you keep yours, no matter where (or if) you work. Child Available with purchase of employee coverage for eligible children, stepchildren, legally adopted children and grandchildren, ages 14 days through 26 years. Birth to 6 months: $1,000. Six months to age 19 (or 26 if fulltime student): increments of $2,000, up to the lesser of: either 100% or 50% of the employee amount (this varies by state), or $10,000. Ends when employee policy ends or when children turn 19 (or 26 if full-time students). They are guaranteed the right to purchase a new policy that s the lesser of: same amount as the employee or spouse coverage, highest amount available to children, or $10,000.
