1 Cding Guidelines fr REDLOC Curated Frms and Instruments After instruments have been suggested by researchers, reviewed by REDLOC librarians and recmmended fr inclusin by REDLOC members, the frms must then be translated int REDCap frmat. At this time, REDLOC members are invlved in this prcess n a rtating basis. In rder t assist in frms translatin and encurage cnsistency thrughut the instruments, REDLOC has develped the fllwing guidelines fr library frms develpers. Instrument Name If pssible, use full instrument name, abbreviated name and versin number. If the name is t lng, be as descriptive as pssible while still staying within the size limitatin. Fr example, the full instrument name is Infant Behavir Questinnaire Revised - (IBQ-R). But Pelvic Organ Prlapse/Urinary Incntinence Sexual Functin Questinnaire (PISQ) is PISQ. Variable Names Variable/field names shuld begin with a brief representatin f the instrument. Fr example, the field label patient in the Agitated Behavir Scale is cded as variable name abs_patient. Variable/field names fr numbered questins shuld be cded with instrument abbreviatin and the questin number, such as abs_1. Cding Values fr Categrical Fields If variable cdes are prvided in the riginal instrument dcumentatin, use as described. Example: BRFSS 2009 Sectin 20: HIV-AIDS Questin 20.1 respnses 1, Yes 2, N 7, Dn't knw / Nt sure 9, Refused If variable cdes are nt prvided, here are sme suggestins: Binary respnses shuld be cded 0/1. Examples: N=0, Yes=1 False=0, True=1 Male=0, Female=1 Respnse sets with mre than tw ptins shuld begin with 1.

2 Cde respnses that represent an unknwn type value with a number that is bviusly ut f nrmal range fr any variable cde r text respnse that makes sense r is missing. Example: Has the patient tested his/her bld sugar levels every day this week? Yes N Dn t knw Refused Missing Might be cded as: 1, Yes 0, N 7777, Dn t knw 8888, Refused 9999, Missing Reverse Cding If questins have reverse scring, cde the selectins accrding t the value unless therwise specified. Here is an example, where #4 is cded nrmally and #5 is reversed: 4. 3 A nte must be included in the instrument header that says the equivalent f Reverse cding has already been applied fr scring purpses. Reverse cding breaks up a matrix. Text Fields If questins have text field respnses, use field validatin whenever pssible (date, phne, , integer, number, r ther available field types). Example: (field label) Tday s Date (pssible respnse) 3/20/12 (Validatin? Date(M-D-Y)) If number r integer validatin is applicable, define the minimum/maximum range as much as pssible. Example fr data cllected in a study n children 6-12: (field label ) Age f child: (pssible respnse) 12 (Validatin? Integer /Minimum: 6 Maximum: 12) Frm Layut Include instructins in the header when available. Example fr PedMIDAS instrument: Headache Disability: The fllwing questins try t assess hw much the headaches are affecting day-tday activity. Yur answers shuld be based n the last three mnths. There are n "right" r "wrng" answers s please put dwn yur best guess. Structure REDCap frm t resemble the riginal instrument as clsely as pssible. Fr respnse area, take advantage f Custm Alignment ptins such as Left/Hrizntal (LH) (applied belw); Left/Vertical (LV); Right Vertical (RV); Right/Hrizntal (RH). 4 Slight wrding changes may be implemented if needed because f the dcument frmat change frm paper t electrnic. Example: Circle the crrect respnse becmes Chse the crrect respnse. Simple HTML may be implemented in field labels t make it mre clsely resemble the riginal instrument. It may als be used t clarify r highlight infrmatin as lng as the intent is nt affected. Examples: <u>text</u> fr underline, <i>text</i> fr italicize, <div><span style='clr:red;'>text</div> t clr red and highlight the interviewer instructins. Fr data entry frms, drpdwn fields are preferred t radi buttn fields, but use yur judgment as t what is mst suitable. Fr surveys, use radi buttns fr shrt lists, drpdwns fr lnger lists. Again, use yur judgment as t what is mst apprpriate fr the frm. Use the Descriptive Text field type t attach scring infrmatin r stats package cding if available. See Interpersnal Reactivity Index instrument. Mark Identifiers. T be HIPAA-cmpliant there are 18 pieces f infrmatin that must be marked as Identifiers in a REDCap data dictinary. 1. Name 2. Example: BRFSS has questins that wuld branch based n answers frm different frms; but since the library prvides each frm individually, the branching lgic wuld nt wrk/be applicable.
