1 JAMAICA COLLEGE Application for Entry to 6 th Form Academic Year PREAMBLE Sixth Form studies require an elevated level of personal commitment and endeavour to ensure success. The school policy regarding 6 th Form studies includes ongoing reviews of attainment, both social and academic throughout the school year. If a student is not meeting the required standards he will be so advised in the presence of his parent/guardian. Goals for improvement will be set and thereafter monitored. The school will review the students tenure in 6 th Form if he fails to meet the required standards. ACADEMIC REQUIREMENTS 1. Class attendance is compulsory. 2. Assignments MUST be completed to required standard and on time. 3. Students are expected to maintain a minimum average of 70%. 4. Recommended texts should be acquired at the beginning of the course of study. 5. Resource Centres such as the computer lab and the library should be used to facilitate completion of assignments. SOCIAL REQUIREMENTS 1. Proper school uniform MUST be worn at all times. 2. Students MUST attend school by 7:30 a.m. daily. 3. All school rules for conduct and behaviour MUST be adhered to with no exceptions. 4. Excellent behaviour and conduct MUST be demonstrated to all members of the school community, including students in 1 st 5th Forms and to colleagues in 6 th form. 5. Participation in clubs and society activities is mandatory. 6. Community Service where and when required MUST be completed. PREPARATION FOR TERTIARY STUDIES 1. Applications to a local university MUST be made before January of the senior year. 2. All students MUST apply for entry to a university. 3. Applications for scholarships require the highest level of attainment in academic and social activities throughout the Upper and Lower Sixth Form years.

2 CRITERIA FOR ENTRY TO JAMAICA COLLEGE 6 TH FORM 1. Applicants should be under 18 years old at the point of submitting the application. 2. A minimum of 5 CSEC subjects preferably at one sitting Applicants MUST have passed English Language and Mathematics. 3. Applicants MUST have a pass at CSEC Grade 1 or 2 in subjects to be pursued at the CAPE level. 4. Applicants will be required to do three CAPE level subjects in addition to Communication Studies and Caribbean Studies in the subject groupings offered by the school. 5. Before the application is accepted, Jamaica College students MUST settle any outstanding bills and school property returned (e.g. rental books). 6. Jamaica College students are required to submit the application to the Dean of Student Affairs by July 10 th. Other applicants should do so by August 12 th. 7. Upon receiving your acceptance letter, all applicants MUST pay the total School Fee before starting classes in September. The application MUST be accompanied by a hand written PERSONAL STATEMENT of at least 150 words outlining your reasons for entry to 6 th Form. Also, you are required to submit an outline of your long term goals and objectives. 8. Successful applicants will be contacted to attend an interview; parents/guardian should be in attendance as well. If you are advised of an interview please ensure that all of your relevant documents are available (the original and a copy). 9. Jamaica College reserves the right to deny any application. The Principal s decision is final. 10.
