3 Christ the King School New Student Application Procedure ADMISSION INFORMATION Parish Verification of Active Membership Form must be submitted directly to your pastor for his signature, for verification of active participation in our faith and in the parish community. Your church will return the signed form directly to Christ the King School. This form must be received by the school prior to applicant screening. Applicants with a signed Parish Verification Form qualify for the reduced Catholic tuition rate. If a pastor will not sign the Parish Verification Form, the students(s) may be accepted but at the full tuition cost. Application Fee: The completed paper application should be delivered directly to Christ the King School. The $ non-refundable fee must be paid online. Checks will not be accepted Birth Certificate: A copy must accompany the completed application. Baptismal Certificate: A copy is required for all applicants and must accompany the completed application. Immunization Record: As required by Georgia law, an up-to-date Georgia Certificate of Immunization Form 3231 and DHR Eye, Ear, and Dental Exam Form 3300 must accompany the completed application. See immunization regulations below: State Law requires all students entering a Georgia school for the first time to have: Diphtheria (4+), Tetanus (4+), Pertussis (4+), Polio (3+), Measles (2), Mumps (2), Rubella (1), Hepatitis B (3), Hepatitis A (2), Varicella (2) or Documented History of Disease or Serology. Students entering 6th grade must have: Hep B (3), Measles (2), Mumps (2), Rubella (1), Varicella (2) or Documented History of Disease or Serology. Request for Release of Records Form: Parents of all applicants must submit the Release of Records form to the current school. Records must be received by Christ the King School via US Postal Service directly from the current school. Student Evaluation Form: The appropriate Student Evaluation Form (Kindergarten, Grades 1-5, Middle School Grades 6-8) must be completed by the applicant s current teacher and mailed directly to Christ the King School. No applicant will be considered for acceptance without a completed Student Evaluation Form. Tuition Assistance: Available to active Catholic families on the basis of need and the availability of funds. In addition, GRACE Scholars provides scholarships to new students from families of all faiths with financial need in grades K-12 for use at approved Catholic schools in the State of Georgia. Please see Financial Policies for further information. Custody Agreement: If applicable a copy of the most current Custody Agreement with the seal of the court is required and must accompany the completed application.

5 Christ the King School FINANCIAL POLICIES A non-refundable application fee of $ per student is due at the time the application is submitted. Payment must be made online at Click on the CKS Application link to find payment instructions. 2. Upon acceptance, a non-refundable student registration fee of $1, is due and paid through FACTS Tuition. $1, is applied toward the accepted student s tuition. Families must register with FACTS.* 3. Tuition includes student fees and textbooks. There are three tuition payment options: Plan A - Annual: Full payment of tuition is due by July 1, There is a $ discount on tuition if paid annually. Plan B Semi-annual: 2 equal payments are due by July 5 or 20, 2015 and January 5 or 20, Plan C Monthly: 10 equal payments beginning July 5 or 20, 2015 and ending April 5 or 20, *All tuition payments are made through the FACTS Tuition Management Program.
