3 Instructions for the trainee Your answers to the assessment tasks need to show the assessor that you have a full understanding of the topic. The assessor may require you to discuss your responses. This assessment is open book, which means that you can use any information you wish when you are completing this assessment. This information may include learning materials, books, the internet and your organisation s policies and procedures. Your answers can be written or spoken. When you write your answer, use pen in the space provided. Initial any alterations you make. Please write your name on any additional pages and attach them to the assessment. When you answer verbally, the assessor will make full notes of your answer in the space provided in this assessment or may use a digital voice recorder. You will be asked to demonstrate practical tasks. Your assessor or verifier will observe you and complete an observation form. The assessor/verifier may require you to perform the task on more than one occasion. You need to show that your work complies with your organisation s policies and procedures. If you require assistance with any aspect of the assessment, please contact your assessor. Definitions of terms The word trainee in this assessment refers to the person being assessed. Other terms that may be used are candidate, student or employee. Person refers to a person accessing services in a health or disability setting in either a residential care facility or in a private home the person s own or a friend s, group s or family member s. Other terms used for the person being supported after brain injury include client, consumer, individual, resident, service user, tūroro or tangata whai ora. Life roles may include but are not limited to family/whānau relationships, social, cultural, spiritual, economic, community, vocational, educational and civic. Natural supports refer to any assistance, relationships or interactions provided to a person after brain injury by family/whānau, friends, peers, co-workers or community volunteers. A rehabilitation plan is a detailed programme of education, interventions, activities, therapies and treatments which aims to restore a person to his or her highest levels of cognitive, physical, psychological and social functionality. ICF framework refers to the International Classification of Functioning, Disability, and Health. WHO refers to the World Health Organization. Workplace assessor or assessor means the person who will assess the trainee. A verifier is a workplace supervisor or manager who understands the assessment, works closely with the trainee, and can confirm that the trainee has performed the task competently. Abbreviations ER refers to the evidence requirements of the unit standard the evidence (descriptions, explanations, documents etc) that you must provide and/or the actions that you must do or demonstrate. 21 Task Three Contribute to the development and/or review of a rehabilitation plan and implement one rehabilitation plan (demonstration) ERs , This task has two parts. A. You are required to contribute to the development and/or review of the rehabilitation plans of two different people who have experienced brain injury ie you may contribute to the development of a rehabilitation plan, the review of a rehabilitation plan, or both. Your work must be based upon a strengths-based approach, and the plan must include clients goals, progress towards achieving these goals, risks and barriers to progress. B. You are required to implement one rehabilitation plan of a person who has experienced brain injury. You must cover all aspects of the rehabilitation process and must include activity, activity limitation, participation, and participation restriction. It is expected that you will carry out this activity in your workplace with a person who has experienced brain injury (or with more than one person if necessary, to demonstrate all aspects of your competence in this task). As in all your work, it is important that: You comply with the Code of Rights, and any relevant New Zealand Standards when carrying out this activity. Your support for people should be aimed at maintaining, improving and/or restoring their independence and/or interdependence; utilising their existing strengths; and where possible, utilising the resources of the local community. You respect the cultural beliefs and values of the people you are supporting. You work within the people s service plans. You work within your organisation s policies and procedures. You will be observed demonstrating this task and your observer will complete the observation form. You might be observed by the assessor. Or, your manager, supervisor or team leader may sign the form to confirm that they have observed you carrying out this type of work in your day-to-day work.
