PLEASE NOTE - IMPORTANT INFORMATION: - The Local Government Act and the City of Perth Local Laws do not permit obstructions, materials, the erection of any structures or hoardings on footpaths, road reserve, City Parks and Reserves; or any interference with or alteration to any fittings or facilities located thereon; unless the approval of the City is first obtained. In this regard, applicants seeking the City s approval for any activities in the road reserve, associated with a construction site, are required to submit a Construction Management Plan. The builder is requested to instruct truck drivers making deliveries to site to avoid routes which involve roundabouts or other obstructions to minimise damage to the road reserve and associated infrastructure. (Provide drawings or sketches where required - refer to attached typical example relevant application forms can be found on the City of Perth website at Road or Footpath Closures:- Will the footpath, road, laneway or verge be obstructed by workmen, materials, trucks, cranes or the like, for any period during the works? If yes, an application is required for a road/footpath obstruction permit. Pedestrian and Vehicular Access:- Subject to the satisfaction of the City, adequate measures shall be taken by the applicant to maintain pedestrian and vehicular access to the road reserve and to ensure the City s facilities are safeguarded. Storage of Materials and Equipment On-Site: Will any materials be stored off site? If yes you will need to apply to the City for a hoarding and/or gantry licence. Indicate on a site plan where materials are to be stored and extent of any hoardings, gantries or scaffolding. Provisions for Parking:-. Provide details of parking arrangements for worksite personal. te: any illegal parking on verges, footpaths or in restricted areas will incur a penalty. Provide details of parking arrangements. Workzones:-. Is it anticipated that parking bays or other areas in the road reserve will be required for the exclusive use of the worksite during the construction? If yes, an application is required for a Workzone Permit. Conditions of Footpath and Road Reserve:-. The builder is responsible for the condition of the road reserve, including footpaths, kerbs, stormwater grates and landscaping, during the works. A works bond deposit fee must be paid on application for a building licence. The applicant will be responsible for any costs relating to reinstatement of the footpath and road reserve. The deposit fee is an estimate only and actual costs will be charged. Once the works bond has been paid contact the City s Construction Supervisor (on ), prior to the works commencing, if you consider that a pre works inspection of the existing condition of the road reserve needs to be recorded for future reference at completion of the works. If the City is not notified it will be assumed that the footpath and road reserve are in an acceptable condition for the purposes of refund of works bond. Dewatering:-. Is it proposed to dewater the site? If yes, a site assessment and investigation is required to determine whether or not Acid Sulfate Soils (ASS) are present. Details of the investigation will need to be attached along with the application for disposal of ground water. Where ASS is present appropriate management measures will need to be approved by the City and the Department of Environmental Protection Water and Rivers Commission. NOTE: Ground water table monitoring will be required for all major de-watering operations. 3

4 Tower Crane Provide details of proposed methods for controlling sand & dust. Is it proposed to use a tower crane on site If yes, the builder must notify the Civil Aviation Safety Authority (CASA) and comply with their requirements in relation to crane lighting. CASA contacted ( Wash down Areas for Trucks:-. (Assessment by Environmental Health Officer) Where will excess concrete or other materials be washed down out of delivery trucks? Has a place been allocated on site? It is NOT permissible to wash down into the stormwater system. Provide details of wash down area in space provided below. Day Time Works twithstanding the requirement for a ise Management Plan (NMP) to address after hours work, large scale demolition and construction projects/sites are also required to submit a NMP for works conducted during the hours of 7am to 7pm. A NMP should be provided for demolition work and for potentially noisy building construction activities such as piling, compacting and concrete pouring processes. Is it anticipated that construction noise may cause nuisance or disturbance to any noise sensitive premises in the close proximity to the development? If yes to either questions, a ise Management Plan will need to be submitted to the Council for approval at least seven (7) days prior to the construction work commencing. Storage & Disposal of Rubbish:- (Assessment by Environmental Health Officer) How is rubbish and waste to be contained on site and removed from site? Please refer to the attached Information Sheet on ise Management Plans Details of works hours and noise management te:- Allowing litter, sand and other materials to spread from the site is an offence. Provide details on the site plan. Control of Sand and Dust;- (Assessment by Environmental Health Officer) What methods are proposed to control the drift of sand and dust from the site? 4

5 Access to site:- (Assessment by Planning Section) Will access to site be required through any other properties, ROW s or reserves or will any other properties be used for storage of materials or other associated construction uses? If yes provide details and written authorisation from the affected parties and other landowners. ROW s shall be kept clear of any obstructions and be maintained trafficable at all times. If the property does not already have an adequate cross over then a temporary crossover will need to be constructed to the City s specification. An application is required for approval of a temporary crossover. Details of access to site: - Indicate access on site plan & attach written authorisation from affected property owners. Signs on hoardings, scaffolding workstructures:- (Assessment by Planning Section) Is it proposed to display any signs, other than OH&S signs or the builder s site board, on the street frontages or within view of public areas? If yes provide details of any proposed signs which exceed 1m² in area and will be in position for longer than 3 months. Some signs may require a separate planning application for approval. Underpinning and Ground Anchors:- (Assessment by Building Section) Is there going to be a need to underpin or insert ground anchors under adjacent buildings or under the road reserve. Provide details and written consent of the neighbours for approval. Show details of underpinning on attached sketch and include written consent from adjoining owners. Vibrations and Dilapidation Survey:- (Assessment by Building Section) Will any building operations or earthworks involve the use of equipment that could possibly cause damage by vibration or settlement to the property of an adjoining owner? So that any damage arising from the works can be clearly identified the City requires the builder to arrange for a dilapidation survey to be conducted on all adjacent buildings which may be deleteriously affected by the works. Details of any equipment, which may cause excessive vibrations. Attach dilapidation surveys. APPLICATION FOR PERMITS AND OTHER APPROVALS The Construction Management Plan is a requirement of Planning and Building approval BUT DOES NOT NEGATE the requirement for separate applications and approvals for such items as:- Road and footpath closures /obstructions; Workzones; Gantry and hoardings; Worksbond inspection request; Use of parking bays; Disposal of water from dewatering operations; ise Management Plan; Signs. INDEMNIFICATION Upon submission of the Construction Management Plan, the applicant undertakes to hold the City of Perth indemnified against all claims which may be made against them for damages or otherwise, in respect of any loss, damage, death or injury caused by, or in the course of or arising out of the use of the road reserve and the property of the City of Perth, during all periods when the reserves are in use. 5

7 INFORMATION SHEET NOISE MANAGEMENT PLANS In accordance with Regulation 13 of the Environmental Protection (ise) Regulations 1997:- Construction work being undertaken should comply with the control of environmental noise practices set out in Section 6 of the AS Guide to ise Control on Construction, Maintenance and Demolition Sites; The equipment used for the construction work or demolition work must be the quietest reasonably available; The work must be carried out in accordance with a ise Management Plan (NMP) - (prepared by a suitably qualified acoustic consultant). WORKS OUT OF HOURS If it is anticipated that there will be any requirement to work outside the hours of 7am to 7pm Monday to Saturday, then an application will need to be submitted to Council, for approval of the work, at least seven (7) days prior to the construction work commencing. The application will need to include a suitably prepared ise Management Plan. DAY TIME WORKS twithstanding the requirement for a ise Management Plan (NMP) to address after hours work, large scale demolition and construction projects/sites are also required to submit a NMP for works conducted during the hours of 7am to 7pm. The NMP will need to address construction noise which may cause nuisance or disturbance as a result of the close proximity of any noise sensitive premises and/or the particular scope of the development or the equipment being employed. A NMP should be provided for demolition work and for potentially noisy building construction activities such as piling, compacting and concrete pouring processes. The ise Management Plan needs to meet the requirements listed on the following page. 7

8 MINIMUM REQUIREMENTS FOR A NOISE MANAGEMENT PLAN Have you provided us with all of the following details? Submit procedures to be adopted for monitoring noise emissions i.e. verifying actual noise levels 8. Provide details of complaint response procedure e.g. Provisions to provide notification to identified noise sensitive premises. 9. Detail follow up procedures investigation of ongoing or unresolved noise issues. Include contact details of persons who will be available to receive reports relating to noise issues during work time and after hours. 8
