6 Terms and Conditions of Hire Please tick each box to confirm you have read and understand each condition of hire then sign the agreement at the bottom of the page. Please remember to include time for setting up and clearing away within your hire time, as it will not be possible to access hired rooms until the agreed time unless previously negotiated. In addition, rooms must be vacated at the agreed time so that they are available for the next user. A charge will be levied for any time used over and above that previously booked. Hirers are responsible for the cost of any damage to the buildings, fixtures, fittings and contents incurred by their group members. MYA is covered by Public Liability Insurance, no liability is accepted by the management for personal accident or injury sustained by another person. HIRERS ARE REQUIRED TO CARRY THEIR OWN PUBLIC LIABILITY INSURANCE. A risk assessment should be undertaken for each new booking [see page 3 ] Hirers are required to pay a non-refundable 25% deposit before using the room. If a cancellation is made at less than 3 days notice then full payment will be required. MYA is designated as a non-smoking building. The hirer is to ensure that those who do smoke do so at the designated smoke area on School Lane. MYA operates a strict no alcohol/drugs policy. Anybody suspected of possession, or being under the influence of either will be asked to leave the building. Failure to leave when requested will result in the Police being contacted. In accordance with Health & Safety Regulations, group leaders are asked to ensure that all members sign in at Reception on arrival. Subsequently all members must sign in/out during breaks so that, in the event of an emergency, this register can be checked. Hirers are reminded that they and their group should be fully conversant with the procedures to be followed in the event of an emergency. At the sound of the fire alarm hirers/groups should immediately vacate the building by the nearest safest exit (do not use the lift) and assemble outside the BBC Radio Merseyside Gift Shop. Group leaders should assist all members of their group to vacate the building. A member of MYA or the Fire Brigade will inform groups when it is safe to re-enter the building. Hirers should remind their groups that we are unable to allow access to the hired rooms without supervision. Please ask group members to arrive on time to avoid waiting and overcrowding in reception. Hirers are asked to note that MYA facilities are not suitable for children under the age of 5 years. Hirers must not use any equipment suspected of being faulty and should report any faults to the reception staff User groups must tidy away all mess and leave rooms in the clean and tidy condition you found it. MYA will not tolerate abusive behaviour towards any if its staff members. Any such behaviour will result in the perpetrator being asked to leave the premises. Failure to leave will result in the Police being contacted. MYA reserves the right to cancel this agreement at the managers discretion I/We agree to abide by the conditions listed on this form, as set out by the management of MYA. Failure to comply with these terms will result in restricted access being ended early at MYA. In this event MYA will not be liable for any charges or losses imposed upon the user. 7 MYA LIMITED FACILITY HIRE PAYMENT DETAILS PLEASE NOTE: Your booking cannot be confirmed until we have received your completed booking form together with a 50% non refundable deposit. Full payment is required at least 10 working days before your room hire/buffet. Failure to do this will result in room hire/buffet being cancelled. In the event of a cancellation by the hirer, we require a full 7 working days notice of such cancellation. Failure to provide 7 working days notice will result in the hirer being charged the full cost of the booking. In the event that there are any changes to buffet requirements we require 48 hours notice. However we can offer the space as a conference facility and it has a seating capacity of up to 100 if required. 9 How To find Us BY RAIL The easiest way is by rail. We are situated a short walk away from both Lime Street Station and Central Station. From Lime Street, leave via the exits close to the platforms 7-9. Turn right, and then take the next left. Follow road along to Ranleigh Street (Lewis s on corner). Turn right, past Central Station on your left (If coming from Central, when leaving station, via left). Follow road down onto Hanover Street and you will find MYA situated on the right hand side of the road. FROM THE M6/M62 Take the M62 off the M6 and stay on the M62 until the very end. Follow the signs for City Centre. When at Coppers Hill, follow to end of road, turn left, continue onto Renshaw Street, (Rapid Hardware is on your left). At traffic lights turn onto Berry Street, take second right onto Wood Street. Continue all the way down and turn left onto Hanover Street. MYA, The Door is on the right hand side of the road. CAR PARKING There are numerous car parks situated within working distance.
