Zanker reserves the right to charge a 1.5% or a minimum of $2.00 per month finance charge on outstanding balances. 3. A signature of the owner, all partners or corporate officer is required along with their social security number(s), printed or typed name(s) and title(s) on the application. 4. Zanker reserves the right to charge a reasonable attorney s fee for the collection process should your account be place in litigation. 5. Zanker only accepts check, cashier s check or money order for account payment. General Information 1. When corresponding with Zanker, always reference your account number. 2. When remitting payment, always reference the invoice number and invoice amounts. If this is not done, then the payment will be applied to the older invoices on your account including any applicable finance charges. 3. Regarding discrepancies on invoices, call the office prior to remittance. Short paid invoices will be considered outstanding unless prior approval for any credits has been obtained. 4. Any accounts that are 30 days past due will be placed on CASH ONLY status. Zanker will only transact business when accompanied by cash, credit card or company check when your account is on this status. Any special pricing will also be discontinued. In order to ensure a more accurate billing, include a list of drivers and/or sub-contractors that will be signing on your account. Provide an updated list if there are any changes. 4. Lead base paint including all materials painted with lead base paint. Mercury, or any materials containing mercury, such as fluorescent lamps and tubes, mercury-vapor lamps and ballasts, mercury switches, electronic devices, etc. Hazardous materials including but not limited to PCB s, radioactive waste, treated railroad ties, source separated pressure treated wood. Any materials that cannot be accepted at a Class III landfill, or any materials burning or smoldering or classified as wet, hazardous, or designated. Asbestos Before it was banned from use, asbestos was used in various applications. The Bay Area Air Quality Management District Regulations and information on identifying and managing asbestos can be obtained from the BAAQMD web site or through direct contact with BAAQMD staff. Asbestos can be found in various construction/demolition materials. Some examples include, but are not limited to acoustic ceilings, taping mud on sheet rock, plaster, flooring, heating ducts, stucco, flue pipes, pipe lagging, and roofing. To determine the presence of asbestos, a sample should be taken to a laboratory and evaluated using appropriate testing procedures. The California State Contractors Board requires that a contractor have a valid, current contractors license and when dealing with asbestos, a valid certificate for asbestos abatement work. Lead Paint For any structure built before 1978, there is the potential that the paint on that structure contains lead. Before acceptance of construction and demolition debris from structures built before 1979, the contractor or hauler must demonstrate, if requested by Zanker staff, that the material either 1) contains no lead paint, or 2) has been suitably tested and determined acceptable for disposal at a Class IIII landfill. 6

7 Items Containing Mercury No mercury or items containing mercury are accepted by Zanker s facilities. These items are considered hazardous waste and must be managed accordingly. Examples of mercury containing items that are not accepted are: Fluorescent tubes, including mercury vapor tubes. Compact fluorescents, including low mercury lamps. Metal halide lamps, such as floodlights for large indoor and outdoor areas and gymnasiums. Sodium lamps, such as those sometimes used as security lighting and outdoor floodlights. Mercury vapor lamps, such as those sometimes used for street lighting. Electronic Ballasts containing mercury. Mercury switches. By signing this Customer Attestation you are attesting that there is no asbestos, lead based paints, mercury or any hazardous or designated materials that cannot be accepted at a Class III landfill, in the materials being delivered to Zanker s facilities.
