1 20+ Childcare Support 2015/2016 Registration Form This childcare registration form must be completed by both the student and childcare provider before an application can be made for childcare support. Doncaster College will only work with childcare providers who have completed this registration form and all questions must be completed. Unfortunately, we are unable to accept incomplete registration forms so please seek advice using the contact details below if you are unsure of the information required. Before submitting this registration form please see the checklist at the end of this document to ensure you have included all of the evidence requested and agree with our Terms and Conditions. If you wish to contact the Financial Support Team please or telephone / Application process for Students All applications for childcare support are subject to the process below Student must complete a 19+ Bursary Fund Application, and provide all necessary supporting documents/evidence to support their application. Student and childcare provider must complete a 20+ Childcare Support 2015/2016 Registration Form and the student must return it to Financial Support Team along with their application form. Student must provide a copy of their Childcare Contract again along with their application form. Please be informed that during our busy periods applications can take up to 10 weeks to be processed. If your application for support is unsuccessful you and your childcare provider will be notified in writing of this and you will be liable for any cost incurred. 2. Student Responsibilities Please read the following very carefully I am responsible for my contract of childcare with my childcare provider and acknowledge that if my application for support is not successful or support is withdrawn due to non-attendance that I will be liable for any costs incurred. I will only use childcare for my timetabled College sessions and acknowledge that support will not be awarded for private/home study. I will inform the Financial Support Team of any changes to my household income levels. I will inform the Financial Support Team when I am not attending College (non-attendance may cause your application form and support to be suspended whilst the team investigate your absence from College and students must meet the minimum attendance requirement of at least 75% for support to continue). If I leave/withdraw from College I will provide both my childcare provider and the Financial Support Team with written notice (please check your childcare contract for the notice period required if you are using College childcare provision the required notice period is 4 weeks), failure to provide this will result in you being liable for any outstanding childcare costs. 4. Childcare Fees Please read the following very carefully Students will only receive funding towards the cost of childcare for their timetabled hours plus 1 hour travel time each way. Doncaster College are unable to assist with registration fees, deposits or additional activities that incur a cost. Any funding awarded will be paid to the childcare provider directly. If a student is entitled to Government Funded Hours it is advised that these be used for non-college related hours. Doncaster College do not support private/home study. Doncaster College do not support childcare provision that is provided by family members or friends. Due to this support being paid via public funds it is expected that these funds are maximised to their fullest and therefore it is at Doncaster Colleges discretion as to the support awarded.

Please state to whom cheques should be made payable. **Please be aware that payment by cheque will take longer than a BACS payment Please note that for academic year 2015/16, Doncaster College will use the following formula when calculating payments to Childcare Providers: Weekly childcare rate x the number of weeks the student is studying This figure is divided by the number of months within the students programme & this is then the set monthly payment amount For example, if the weekly rate is and the student is studying a programme of 36 weeks (this would equate to 10 months), the monthly payment amount would be: x 36 weeks = 2, / 10 = per month Payments will be made on a monthly basis and will be paid for the duration of the students programme. Payments for the first term (September to December) may be delayed due to the volume of applications received during this period. Whilst we will endeavour to make timely payments to childcare providers during this period, there may be a short delay during the first term.

Term Dates Doncaster College s Term dates for 2015/16 are as follows: Term 1 7 th September th December 2015 Half Term 26 th October th October 2015 Term 2 4 th January th March 2016 Half Term 8 th February th February 2016 Easter break 21 st March st April 2016 Term 3 4 th April st July 2016 Half Term 30 th May rd June 2016 **Please note that childcare support for Breakfast and After School Clubs will be paid Term Time Only. 10. Important Information and Terms & Conditions 1. Until written confirmation has been provided from Doncaster College, childcare support is not guaranteed. 2. The childcare contract is between the student and the childcare provider and therefore the responsibility of childcare fees ultimately lies with the student. Therefore Doncaster College reserves the right to withdraw support if the student does not adhere to their responsibilities as outlined in this document and the 19+ Bursary Fund Procedure. If this does occur, any outstanding fees are the students responsibility. 3. Childcare support will be discontinued on completion of a course or if the student withdraws from College. 4. Doncaster College reserves the right to cease working relationships with childcare providers who are deemed to be rude and abusive to or are uncooperative with College staff. 5. Students who have submitted an application will have been given a receipt. If you wish to find out if a student has submitted an application, please ask to see their receipt or financial.support@don.ac.uk. Confirmation will be by only. Please be aware that applications for childcare support cannot be completed until the students timetable is showing correctly on the College s systems. This can cause delays during the early part of the first term and we ask that you be patient with us during this time.
