3 Introduction The Rosedale Centre is a purpose built single storey short term residential rehabilitation and assessment Centre with 44 bedrooms, owned by Stockton-on-Tees Borough Council. The Centre has four units, providing Rehabilitation (Oaks and Laurels) and Assessment (Willows and Poplars). Clients are referred from local hospitals and from the community via GP s, Social Workers and Therapy staff. Aims and Objectives To value clients and meet their individual needs with respect and dignity at all times, offering advocacy if the need arises. To achieve a level of independent living appropriate to each client s needs and desires, maintaining skills and offering a quality of life, which maximises freedom of choice. Activities and social events will be made available and clients will have the choice to participate or not. Families and friends will be made welcome and encouraged to be involved. We enable both existing and new links within the community. Client s have the right to make a complaint and have this investigated in a professional and supportive manner. We liaise effectively and appropriately with all health and other allied professionals to ensure clients receive a comprehensive support services. We take regard of the safety of each individual client but recognise that calculated risks are part of normal every day life. 3
4 Accommodation Each unit consists of: A lounge where clients consume their meals, combined with a seating area for them to watch television, listen to music or to chat with fellow clients. A small kitchen with basic equipment, where clients and their visitors can make refreshments. A small lounge with a television where social visits can be held. Two individual toilets and one within the bathroom. Single occupancy rooms, which have a hand washbasin, wardrobe, a single bed, a bedside cabinet, a portable television and a lounge chair. A commode chair can be provided if the client requires this. Overhead lighting is provided, which can be turned off by the resident when in bed by a pull cord and all rooms have a bedside lamp. All rooms can be locked from inside and client s are encouraged to hold their own key. Range of Needs Individually assessed needs are met on the following units within the Rosedale Centre: Rehabilitation Units - Oaks and Laurels Laurels has 10 beds and Oaks has 12 beds, offering a rehabilitation service free of charge to the client. Clients must be a resident living in the Borough of Stockton-on-Tees. Social Care staff work alongside health workers (Occupational Therapists and Physiotherapists). Clients are referred to this service by social workers, GP s and hospital staff and then assessed by Rosedale s Therapy team to see if the unit can meet their assessed needs; if so the client can be admitted to the unit for up to six weeks but the average stay is approximately 17 days. 4
5 Clients will receive an individual therapy plan and before discharge a review meeting will be held to discuss community support (if needed). If required the therapist will complete an environmental home visit to see if the client requires any additional equipment/aids to support them in their home. Therapy staff complete a meet and greet service when clients are discharged home to ensure the discharge process goes smoothly. Assessment Units - Willows and Poplars Poplars has 10 beds and Willows has 12 beds offering an assessment service. Clients are admitted from the local hospitals after they have been assessed as medically fit for discharge by hospital medical staff. Admission can also be from the community via an assessment from a GP, Community Matron, District Nurse, Rapid Response Nurse or a member from the Community Therapy Team, if this admission would prevent a hospital admission. This is a free service to the client for up to 6 weeks. The clients must be a resident living in the Borough of Stockton-on-Tees. The process of assessment is that after two weeks the management, therapy team and the unit support staff will complete a summary of needs for the social worker. All assessment clients will be reviewed no later than four weeks after admission to plan the next phase of care. Appropriate people will be invited to the review meeting e.g. family, social worker, occupational therapist and district nurse. Options will be discussed with reference to the summary of needs recommendations, which are: Return home with support of family and care package. Rehabilitation in Intermediate Care (residential or community). Move on to permanent care. Rosedale staff do not provide nursing care needs, however if required these needs are met by District Nurses from the residents chosen GP s surgery or by Specialist Nurses i.e. Diabetic Nurse, Stoma Nurse, etc. 5
6 All units are staffed individually, promoting continuity of care. Each unit has an allocated unit manager, who has meetings with staff and one to one supervisions, which provides a tool for monitoring, to ensure good quality care is being delivered. The duty manager is responsible for all units on a daily basis. The registered manager has the overall responsibility for the services provided at the Rosedale Centre, staffing issues and health and safety of the environment. Registered Manager Rosedale Centre s Registered Manager is Mrs. Jean Spedding, who has been employed by the Local Authorities Adult Services department at the Rosedale Centre since October Jean has experience in both direct care work and management of care and has been the centre manager since October Jean is a qualified State Enrolled Nurse, has a National Vocational Qualification level 4 in care and Registered Managers Award Level 4. She also has D32/33 Assessors Award and D34 Internal Verifiers Award qualifications and has both assisted and encouraged her staff to reach the national requirements of NVQ level 2 in care, to enhance their practical and knowledge skills to deliver a quality standard of care to all individuals within the Rosedale Centre. Jean has also completed the Management Development Programme and has Level 5 Managing Medication Systems. Jean keeps up to date with mandatory training courses and attends any other courses relevant to her role. Registered Provider Stockton-on-Tees Borough Council has a vast experience of providing a personal care service in a residential setting and within the community. The council operate to high standards developed from legislation, national guidance, local policies and procedures. 6
7 Rosedale s Organisational Structure Rosedale s staff team consists: One Registered Manager who works 37 hours (shift pattern to the needs of the service) Three Assistant Managers are contracted to 37 hours per week working a three-week rota consisting of morning and late shifts, between the hours of 7:30am 10pm- = 111 hr Three Support co-ordinators who work alongside the duty manager between the hours of 8am and 9pm Three office clerks 12 night support workers that take over the managerial role from the duty manager between the hours of 10pm and 7.30am. = 280 night care hours 32 day support workers. = day care hours One cook, 31hours and one assistant cook 29hrs = 60 hours Four kitchen assistants = 86 hours Six domestic s = 113 hours Two laundry assistants. = 38 hour Staff Qualifications The management team have the accredited professional qualification required to fulfil their managerial role. All support workers have or are working towards appropriate qualifications at Diploma Level 2 & 3 in Health and Social Care. Staff attend the following mandatory training courses, which they are paid to attend: NVQ levels 2,3,4 or equivalent. Corporate Induction given to all new starters. 7
8 Skills for Care Induction for support staff new starters. Emergency Aid. Safer People Handling (support staff)/moving and handling of objects (ancillary staff). Health and Safety/Fire Training. Food Hygiene Awareness. Level 2 Infection Control. Safeguarding Vulnerable Adults. Child Protection Awareness. Autism Awareness. Deprivation of Liberty awareness. Mental Capacity Act/DOLS awareness. Customer First. Equality and Diversity awareness. Re-ablement training from outside agency. Supervision, Training and Development The Registered Manager supervises the management team monthly. Remaining staff are supervised six times a year including an annual appraisal. The staff groups are shared between the management team for supervisions and appraisals. General staff meetings are held annually and individual unit staff meetings are held regularly by designated unit managers. The Council s training team supplies a training schedule yearly, stating what courses are available. Training and development needs are discussed at supervision sessions and staff are encouraged to apply for courses relevant to their role. Three paid training days per year is available to all staff groups. 8
9 Ethos and Philosophy Our aim at the Rosedale Centre is to provide the highest quality, person-centred care service to our intermediate care clients, taking into account their personal needs and wants. Individuals who use our service will be assessed according to their individual needs and requirements. This helps build a detailed personal profile and formulate an appropriately targeted, ongoing individual care plan and individual risk Assessments are produced to make clear the necessary balance between the client s needs, wants and their individual need for protection. We build our approach by working within the department s core values of Privacy, Dignity, Independence, Choice Rights and Fulfilment. In order to develop and reinforce our ethos and philosophy, we actively seek and encourage ongoing communication with our residents, their families, friends and with other agencies. We are always looking for ways to improve or develop our service and welcome suggestions from all concerned. 9
10 Relatives, Friends and Representatives The Rosedale Centre has designated visiting times due to therapy sessions, which are 1 till 2pm and 5 till 8pm, Monday to Friday. Saturday and Sunday visiting is between 10am and 8pm but we ask visitors to avoid mealtimes, if possible. Each unit has a small lounge available for clients to entertain their visitors and a small fully equipped kitchen where clients and their visitors can make tea or coffee, free of charge. Complaints Procedure The Internal complaints procedure is explained in the leaflet Complaints, Commendations and Complaints, which are displayed within the Rosedale Centre and included in the welcome pack upon admission. Clients and their family are made aware of the procedure upon admission and if they have a need to complain, which cannot be resolved informally by the Rosedale Centre s management team or if the complaint is of a serious nature, this procedure is reiterated and supported. Protection of Clients The safeguarding of clients is paramount whether the abuse is physical, sexual, emotional, institutional or financial and from whatever source. All staff have attended the No Secrets (Safeguarding Adults) training course, to raise their awareness of the various types of abuse and the procedure to follow if they witness or are aware of abuse taking place. The Rosedale Centre endeavours to take necessary steps to minimise opportunities for and to prevent abuse, the departments Whistle-blowing policy is broadly advertised within the home. The Registered Manager has completed Module one Safeguarding Vulnerable Adults at the University of Teesside. 10
11 Fire Precautions The Rosedale Centre has a NO-SMOKING policy in place. Rosedale s alarm system is checked weekly on Wednesday mornings by way of a fire instruction for staff on duty and all fire fighting equipment is checked annually. Fire and smoke detectors are in place throughout the building. The evacuation procedure assembly points are displayed on each unit. Staff attend external training courses held by Health and Safety officers and have 6 monthly in-house fire training from the duty manager. Clients are made aware of fire procedures upon admission and are informed when weekly checks are being held. There is an emergency lighting system in place in case the electricity supply fails. Engineers check this system monthly and any faults found are reported and repaired as soon as possible. Anti-Discrimination Policy The clear policy of Stockton-on-Tees Borough Council (Children, Education & Social Care) and of Rosedale Centre itself is that all clients should be offered equal opportunities regardless of their age, gender, faith, disability, ethnic origin or sexuality. 11
