2 This Request for Quotations ( the RFQ ) issued by the Town of Milk River (the Town ) is an invitation to submit non binding offers for the provision of one (1) new Refuse Collection Truck as further described in this document. The respondent selected pursuant to this RFQ process will be informed in writing. For the purposes of this procurement process, the Town Contact shall be: Samuel Beliveau; publicworks@milkriver.ca or (403) Submission Instructions Respondents are requested to submit their Quotation Form (Appendix A) in the manner prescribed herein by the Submission Date: Town of Milk River 240 Main Street Milk River, Alberta T0K1M0 Or publicworks@milkriver.ca

3 All submissions are to be prominently marked with the competition title with the full legal name and return address of the proponent, and with the Submission Date. Respondents to Review RFQ Respondents shall promptly examine all of the documents comprising this RFQ and (a) shall report any errors, omissions or ambiguities and (b) may direct questions or seek additional information in writing by e mail or fax on or before the Deadline for Questions to the Contact. The Town is under no obligation to provide additional information but may do so at its sole discretion. It is the responsibility of the respondent to seek clarification from the Contact on any matter it considers to be unclear. The Town shall not be responsible for any misunderstanding on the part of the respondent concerning this RFQ or its process. All New Information to Respondents by Way of Addenda This RFQ may be amended only by an addendum in accordance with this section. If the Town, for any reason, determines that it is necessary to provide additional information relating to this RFQ, such information will be communicated to all respondents by addenda. Each addendum shall form an integral part of this RFQ. Such addenda may contain important information, including significant changes to this RFQ. Respondents are responsible for obtaining all addenda issued by the Town. In responding to this RFQ, and to be eligible for consideration, each respondent must submit a completed and signed Quotation Form that, among other things, acknowledges its acceptance of the RFQ Terms of Reference and Governing Law as contained hereunder: (a) (b) (c) (d) (e) (g) this RFQ process is not intended to create a formal legally binding bidding process and shall not give rise to the legal rights or duties applied to a formal Contract A binding bidding process or any other legal obligations arising out of any tendering process contract or collateral contract, and instead shall be governed by the common law applicable to direct commercial negotiations; neither party shall have the right to make any breach of contract, tort or other claims against the other with respect to the award of a contract, the failure to award a contract or the failure to honour a quotation; the respondent will bear its own costs associated with, or incurred in, the preparation and presentation of its response, including, if applicable, costs incurred for interviews or demonstrations; when evaluating quotations, the T o w n may request further information from the respondents or third parties in order to verify, clarify or supplement the information provided in the respondent s submission, and the Town may revisit and re evaluate the respondent s submission or ranking on the basis of any such information; the Town may consider the respondent s past performance on previous contracts or any other relevant information taken into account by the Town when determining the acceptability of a respondent; the respondent consents to the Town s collection of the information as contemplated under this RFQ for the uses contemplated under this RFQ; (h) the Town may elect not to consider a respondent whose quotation contains misrepresentations or any other inaccurate, misleading or incomplete information; (i) the Town may prohibit a respondent from participating in a procurement process based on poor past performance or based on inappropriate conduct in a prior procurement process, and such inappropriate conduct shall include but not be limited to (i) the submission of quotations Page 3 of 6

4 containing misrepresentations or any other inaccurate, misleading or incomplete information, (ii) the refusal of the respondent to honour its pricing or other commitments made in its quotation, or (iii) any other conduct, situation or circumstance, as solely determined by the Town, which constitutes a Conflict of Interest; and (j) the Town may cancel this RFQ process at any time. The parties also acknowledge that these terms (a) are included for greater certainty and are intended to be interpreted broadly and separately (with no particular provision intended to limit the scope of any other provision); (b) are non exhaustive (and shall not be construed as intending to limit the pre existing rights of the parties to engage in pre contractual discussions in accordance with the common law governing direct commercial negotiations); and (c) are to be governed by, and interpreted and construed in accordance with, the laws of the province or territory within which the Town is located and the federal laws of Canada applicable therein. THE DELIVERABLES The objective of this project is to purchase one (1) new refuse collection truck. The truck is to replace existing refuse collection truck. Schedule 1 Waste and Recycling Automated Sideload Refuse Truck MATERIAL DISCLOSURES Examples of the type of information that should be disclosed as material disclosures include, but are not limited to Prices to include all applicable freight, customs duties, excise taxes, and insurance costs FOB Milk River. Vender to provide a complete, accurate weight distribution diagram, clearly indicating weights and locations of all loads for quotation evaluation purposes The proposed complete unit must be compliant with all applicable Alberta Traffic Safety Act provisions, Canadian Motor Vehicle Safety Standards and current Canadian environmental Protection Act vehicle emissions regulations. current CMVI inspection included with unit out of province inspection RIV register imported vehicle A detailed preventative maintenance schedule to be provided in RFQ The Town of Milk River would like to have Hard copies of parts and service and operating manuals but understands that different methods of manual(s) access is now used. Please specify the method maintenance (service & parts) manuals can be accessed. The supplier is required to provide to the Town an owner s manual for each truck. The submitted RFQ package will include all standard warranties, extended warranties and guarantees covering the materials and workmanship. Supply a line sheet showing specifications. Specs need to include: curb weight, exterior dimensions, interior dimensions, and volumes and capacities, etc. Dealer may supply multiple quotes the Town has no preference for make of chassis or body Cost of options not stated in the specifications to be priced separately Quotation responses need to be complete and accurate. Each quotation specification should have either a YES' or 'NO' response checked off. If NO, please state deviation. Incomplete bids may result in bid rejection. If the access of parts and service manual require fees or purchase costs, please specify. Each unit to be supplied with 3 sets of door and ignition keys Change orders: Cannot start until the change is approved through the appropriate levels within the Town Page 4 of 6

5 The vendor can start work on the requested change once the PO has been adjusted to include all new items and costs and after instructed by the Town. The supplier to provide to the Town all related wiring schematics. Any training will be scheduled through the Town before delivery of unit. Training is to be provided to operators and service staff. The Town reserves the right to consider and award alternate proposals if deemed in the best interest of the Town. The minimum requirements for the completed units shall be detailed in the specification requirements. Specification form must be completed. Bidders may propose changes/alternates/equivalents if they believe a better quality product will result Page 5 of 6

The arm and grabber to be on curbside of truck and must be able to reach and grab a cart at a minimum of 84" (7') B. C. state the type of arm used. i) arm mounted between cab and hopper (i.e. Wayne Curbtender, Heil Rapid Rail, Dadee scorpion etc.) ii) trucks that have horizontal sliding tub or rail grabber with a swing out grabber (pendulum type) will need to demonstrate the ability to dump Carts in lanes the truck is intended to be used in D. E. Lifting mechanism shall be capable of gripping, lifting, raising, and dumping 360 liter refuse carts iv) if a candy cane style lift arm is to be bid because the Town of Milk River is not familiar with this design and have concerns about reliability. The bidder is to provide references and maintenance records of performance for a minimum of 5 years. Arm reach and dump cylinders will be internally cushioned to slow the arm movement at the end of the return of the reach cylinder and the return of the dump cylinder A dash mounted warning light indicating when arm is not stored and audible alarm when arm is not at rest

Safety / Warning Lighting i) LED beacon required for front and rear of vehicle D. Back up (collision prevention) system E. Camera monitoring system F. All trucks will be required to be equipped with camera monitoring systems. please provide information on the various models available for installation onto your truck i) camera system to be high quality a) state make model used ii) flat screen color monitor with high definition picture a) LCD design b) monitor screen minimum 7" size c) monitor easily toggle between camera views iii) 4 colour cameras a) mounted - one on rear of truck, one on the right side of body with outward view of arm (to line arm up with can), one to view the inside of hopper and one under the right side review mirror b) split screen and switching between views G. Auxiliary arm controls i) accessed from right side (curbside) of truck with operator standing on the ground ii) controls for arm grabber activation - dump, cart return to ground 12. A pre-build meeting shall be held via teleconference or in Milk River approximately 2 weeks after award of tender E. Project progress reports i) the winning vendor at point of construction is to provide a production schedule iii) the first report will be at approximately the 50% completion point iv) the second report will be at approximately the 85% completion point vi) the reports are to show where the project is on the production schedule and if on, behind, or ahead of schedule vii) photos and other related information to be included in the as required F. Inability to make these arrangements will negatively impact the evaluation score for operator acceptance, as the Town is not willing to purchase a machine that has not been demoed / viewed by the appropriate Town staff. 22.
