Barth, Ph.D. Overview Dr. Barth has been named a Fellow of the National Academy of Neuropsychology, for having made a significant contribution to science and practice. Dr. Barth practices, writes, and teaches in the following areas (as well as others): Chronic Pain (e.g. Complex regional pain syndrome - the concept that replaced reflex sympathetic dystrophy, Headache, Back pain, etc.) Brain injury / Brain Impairment Mental Illness (e.g., PTSD, depression, substance abuse/dependence, etc.) Dr. Barth has been asked to provide faculty, writing, and editorial review duties for many academies, governmental agencies, and healthcare publishers including: American Medical Association American Psychological Association American Academy of Orthopaedic Surgeons European Union of Medicine in Assurance and Social Security American Academy of Disability Evaluating Physicians North American Spine Society American Academy of Neurology American College of Occupational and Environmental Medicine National Association of Workers' Compensation Judiciary International Association of Industrial Accident Boards and Commissions Official Disability Guidelines Treatment in Workers Compensation Occupational and Environmental Medicine Press Several state and provincial governmental workers compensation agencies (e.g. Alabama, Alberta, Georgia, Mississippi, Tennessee). The American Medical Association has repeatedly asked Dr. Barth to contribute to AMA publications and programs, including the Guides to the Evaluation of Permanent Impairment, the Guides to the Evaluation of Disease and Injury Causation, and continuing education for the AMA House of Delegates. 2 the primary source for interpretation of the Guides to the Evaluation of Permanent Impairment. Dr. Barth has also worked in curriculum planning for continuing medical education programs, including chairing the planning committee for the Annual Scientific Session of the American Academy of Disability Evaluating Physicians, and planning multiple years of the Health Science program for the Workers Compensation Educational Conference. His work history has included serving as Neuropsychology Director for a rehabilitation hospital, serving as Director of Brain Injury Rehabilitation for an outpatient facility, and independent practice of neuropsychology. He has also dedicated more than a decade of volunteer work to Hospice of Chattanooga, serving as Director, Chairman of the Board, Compliance and Ethics Committee Chair, and Research Committee Chair for the largest provider of pain management and end-of-life care in Chattanooga. His full-time education culminated with post-doctoral fellowship training in the Harvard Medical School system at Massachusetts General Hospital, following pre-doctoral internship training also in the Harvard Medical School system at MGH, and completion of the Ph.D. program at the University of South Florida. PAIN: Additional Examples of Dr. Barth s professional activities of direct relevance to complaints of pain Contributor for the American Medical Association s Guides to the Evaluation of Permanent Impairment 6 th Edition, specifically in regard to chronic pain presentations. For example, Dr. Barth was asked by the AMA s editors to contribute to discussions of Complex Regional Pain Syndrome (CRPS) that were incorporated into the AMA s Guides. Significant portions of the CRPS discussions that were eventually accepted for inclusion in the Guides were written directly by Dr. Barth. Dr. Barth was asked to be the Final reviewer for the American Medical Association s Guides to the Evaluation of Permanent Impairment 6 th Edition, for the Pain chapter, the Mental and Behavioral Disorders chapter, and the Central and Peripheral Nervous System chapter (all of which address issues of pain). Author of at least 12 articles that were invited by and published by the American Medical Association, regarding pain (e.g. CRPS, chronic pain, back pain, headache, fibromyalgia). Faculty for the American Medical Association s House of Delegates, regarding pain (e.g. CRPS, chronic pain, back pain, fibromyalgia). Six years, and continuing, as faculty for the American Academy of Orthopaedic Surgeons, lecturing numerous times on issues involving pain (e.g. CRPS, chronic pain, back pain, headache, fibromyalgia). For example, Dr. Barth was asked by the Academy s 2

3 medical director to create a near-comprehensive discussion of the differential diagnostic protocol for claims of CRPS. The product of that work was a continuing medical education program for the Academy s membership (orthopedic surgeons). Dr. Barth has subsequently lectured, on the subject of CRPS, for this Academy, on at least seven occasions (as well as lecturing many additional times on other pain issues). Three years as curriculum planner for the American Academy of Disability Evaluating Physicians, including planning the 2010 Annual Scientific Session which focused exclusively on pain management, and planning numerous other continuing medical education programs that were focused on pain. Nine years, and continuing, as faculty for the American Academy of Disability Evaluating Physicians, lecturing numerous times on pain (e.g. CRPS, chronic pain, back pain, headache, fibromyalgia) and pain management. Invited author for the Journal of Hand Surgery (publication of the American Society for Surgery of the Hand) on issues of disproportionate pain and disability, and complex regional pain syndrome (CRPS). Twice serving as faculty for the European Union of Medicine in Assurance and Social Security, lecturing on pain (e.g. CRPS, chronic pain, back pain, fibromyalgia).. Repeatedly lecturing for state and provincial governments (e.g. Alabama, Alberta, Georgia, Mississippi, Tennessee), regarding pain (e.g. CRPS, chronic pain, back pain, headache, fibromyalgia, carpal tunnel syndrome). Faculty for the North American Spine Society, addressing pain in the context of workers compensation (e.g. back pain, neck pain, CRPS, chronic pain). Faculty for the National Association of Workers' Compensation Judiciary, teaching workers compensation judges about the science of pain (back pain, carpal tunnel syndrome, CRPS, chronic pain). Dr. Barth has also been invited to write on the subjects of CRPS and other presentations of pain for Occupational and Environmental Medicine Report. The American Academy of Neurology (AAN) asked Dr. Barth to represent that Academy in the review process for: the American College of Occupational and Environmental Medicine s effort to update their guidelines regarding CRPS and other chronic pain presentations; the American College of Occupational and Environmental Medicine s (ACOEM) Guidelines regarding low back pain; guidelines for the use of opioids (narcotics) for chronic non-cancer pain; practice parameters for recurrent headache, etc. The American College of Occupational and Environmental Medicine has involved Dr. Barth in the creation of diagnostic and treatment guidelines for claims of CRPS, and in the development of guidelines for preventing disability from any type of pain. 3

4 Teaching for the American Psychological Association on the evaluation of and scientific knowledge base regarding pain. BRAIN INJURY / IMPAIRMENT: Additional Examples of Dr. Barth s professional activities of direct relevance to brain injury and brain impairment: Dr. Barth s work with the American Medical Association Dr. Barth was a contributor and reviewer for the most recent Edition of the American Medical Association s Guides to the Evaluation of Permanent Impairment. For example, Dr. Barth was chosen to be the final reviewer for the Guides chapter which is of greatest relevance to brain injury and other forms of brain impairment, the Central and Peripheral Nervous System chapter. Dr. Barth has also provide continuing medical education for the AMA s House of Delegates, including specifically teaching the Delegates (who are all medical doctors) about impairment claims that are associated brain injury. The American Medical Association has also published Dr. Barth s written discussions of impairment claims that are associated with brain injury. Dr. Barth s work with the American Academy of Disability Evaluating Physicians This Academy chose Dr. Barth to teach medical doctors, nationally and internationally, on the following issues: Brain Injury (specifically including teaching the medical doctors about diagnosis and impairment considerations for such injuries) Postconcussive syndrome Posttraumatic headache Other relevant activities: Dr. Barth has been named a Fellow of the National Academy of Neuropsychology, for having made a significant contribution to science and practice. Dr. Barth has repeatedly served as a clinical guideline reviewer for the American Academy of Neurology, on issues such as dementia / brain impairment. Dr. Barth has a history of serving as Director of Brain Injury Rehabilitation and Director or Neuropsychology for brain injury rehabilitation facilities. 4

5 Dr. Barth has also served as faculty for additional continuing education programs, which addressed brain injury, for governments, healthcare and scientific academies in addition to those mentioned above, and universities/medical schools. All of Dr. Barth s full time graduate and post-graduate education/training programs (e.g. Masters program, Doctoral program, Internship, and Post-Doctoral Fellowship) involved a focus on brain impairment. Teaching for the American Psychological Association on the evaluation of and scientific knowledge base regarding brain injury and brain impairment. MENTAL ILLNESS: Additional Examples of Dr. Barth s professional activities of direct relevance to mental illness: Author of the Mental Illness chapter in the American Medical Association s Guides to the Evaluation of Disease and Injury Causation. Chapter leader for the mental illness chapter of the Official Disability Guidelines Treatment in Workers Compensation, which has reportedly been adopted into the regulations of 20 states and 6 Canadian provinces. Wrote the PTSD discussion in the American Medical Association s Guides Casebook 3rd Edition, specifically addressing the evaluation of impairment associated with PTSD and other forms of mental illness, in accordance with the 4th, 5th, and 6th Editions of the AMA s Guides to the Evaluation of Permanent Impairment. Final reviewer for the American Medical Association s Guides to the Evaluation of Permanent Impairment 6th Edition, for the Mental and Behavioral Disorders chapter. Asked by the American Medical Association s editors to write the first draft of the Errata for the Mental and Behavioral Disorders chapter of the AMA s Guides to the Evaluation of Permanent Impairment 6th Edition. Author of at least eleven articles that were invited by and published by the American Medical Association, many of which addressed issues of mental illness. Faculty for the American Medical Association regarding issues of mental illness. Six years, and continuing, as faculty for the American Academy of Orthopaedic Surgeons, lecturing numerous times on PTSD and other forms of mental illness. Nine years (and continuing) as faculty for the American Academy of Disability Evaluating Physicians, lecturing numerous times on mental illness. Twice serving as faculty for the European Union of Medicine in Assurance and Social 5

A history of being invited to lecture for several state and provincial governments (e.g. Alabama, Alberta, Georgia, Mississippi, and Tennessee) on issues of PTSD and other forms of mental illness. Selected by the Journal of Bone and Joint Surgery to serve as an editorial reviewer regarding issues of PTSD among surgery patients. The American Academy of Neurology asked Dr. Barth to represent it in regard to reviewing the American Psychiatric Association s guidelines for the treatment of Major Depressive Disorder and Bipolar Disorder. Teaching for the American Psychological Association on the evaluation of and scientific knowledge base regarding mental illness. Additional issues which have been prominently addressed in the course of Dr. Barth s practicing, writing, and teaching: Hypertension, Systemic Lupus Erythematosus, disability, maximizing health outcomes within workers compensation and other forms of medical-legal claims, and scientifically credible health care. 6
