2 Your First Visit The Reception Staff will take your details to enter onto the practice computer system. They will then ask you to complete a Confidential Medical History form. This is to provide us with information which may be relevant to your Dental treatment. It is not normally necessary for us to have access to your previous dental records so please don t worry about them. You will then be shown to the Reception lounge where you may relax in very comfortable surroundings. A member of Staff will collect you and take you to your new Dentist. At the first visit we feel it is more important to get to know you and establish a good relationship therefore unless there is any emergency treatment necessary we confine our first visit to a discussion of your Dental needs (if any), the taking of a history and a thorough Dental examination together with any necessary x-rays. After that you can relax! our team Your health and comfort are our primary concerns. Please let us know of any condition requiring special attention of any changes in your health during treatment. If any problem should arise, at any time, please inform the Dentist, Practice Manager, or an appropriate staff member so we may have the opportunity to resolve the matter. Welcome to The Partner will then discuss with you any treatment options identified during the examination and, if necessary, an appointment will be made. At the following consultation appointment we will explain our findings and recommendations with you. By explaining the options fully, we enable you to make an informed decision about the procedure which is right for you. You will be given a written report and treatment plan, including an estimate of costs. After your treatment is complete we will agree together how often your routine examination and hygienist appointment will be. For most patients this will be every six months. We can make your check-up appointment at your last visit or our fully integrated computer system will generate a recall to be sent to you through the post, or text messaging!

In return we expect from our patients the following: Your cooperation in making and keeping appointments. Should it be necessary to change an appointment, we require at least 24 hours notice. 4 Fee List Dental Care Plan Regular dental appointments(two per year) to provide: clinical examination screening for oral cancer X-rays where clinically necessary Maintenance Plan We also offer a service where members are entitled to two dental appointments and four hygienist appointments per year. Regular Hygienist appointments(two per year) to provide: Scale & Polish Periodontal advice Instruction in preventive techniques to reduce decay and gum disease Assessment of emergencies and dental pain with any temporary treatment required, carried out at the practice during normal surgery hours. Dispensing of emergency antibiotics. All treatment planning for your future dental needs. Production of study models when required. All subsequent remedial work within 12 months of recommended restorative work. 20% reduction off any laboratory based treatments required, ie, crowns, bridges and dentures. 20% reduction off any fillings. 20% reduction off other dental work (extractions, etc) 20% reduction off any additional hygiene work. 10% reduction off oral hygiene products purchased at the surgery. Worldwide trauma insurance to protect against unforeseen accidental damage. Emergency call-out insurance, should you need a dentist in an emergency, anywhere in the world. Full details of the fees for this service are available upon request. Annual Plan As for the Dental Care Plan, but members are entitled to one dental and one hygienist visit per year. Financial Policies Our patients understand that in order to deliver optimum dentistry, we must maintain our office on sound business principles. Therefore, we inform our patients of our financial policies at the very beginning of our relationship to avoid any misunderstandings. We accept major credit cards, cash and personal cheques. Payment for the examination appointment is due at the time the services are rendered. Extended payment plans are available for qualified individuals for subsequent treatment. 6 Partner Profiles Dr Paul Coulthard BChD, MFGDP(UK) RCS GDC No Paul has returned to his home town after qualifying in 1982 from Leeds University. Paul gained top marks in the final examinations, being awarded the Watssman Prize for Clinical Dental Practice. Paul joined Kelvin Lodge in 1987 as an Associate, replacing the founding member, Mr Raymond Stubley, on his retirement. He became a Partner in Paul completed the Diploma of Membership of the Faculty of General Dental Practitioners of the Royal College of Surgeons in 2003 and is a member of the British Dental Association. Paul has recently completed a one year Restorative Dentistry course held in Manchester and also two courses in 2006, and 2008 in advanced cosmetic dentistry. He is a full member of the British Academy of Cosmetic Dentistry. Paul is married to Sally, a research scientist, and they have two boys, Nicholas and Richard. Outside work, Paul s main interest is golf, being an ex-county player and club champion. Paul also enjoys keeping fit at the gym and skiing. Dr Andrew R.Greenwood BDS.DGDP(UK)RCS.Dip.Imp.Dent.RCS(Eng).FFGDP(UK) GDC No Originally from Halifax in West Yorkshire, Andrew qualified in 1986 from Newcastle University. After working as an Associate, he started a single-handed practice in 1993, seeking a new way to practice modern preventative dentistry. Whilst running his own Practice in Gateshead, Andrew provided Orthodontic treatment at Kelvin Lodge until he became a Partner when Mr Gordon Jennings retired in In 1995, he successfully completed the Diploma in General Dental Practice from the Royal College of Surgeons and is also a member of the Association of Dental Implantology and International Team for Implantology. He has been a Clinical Assistant in Orthodontics at Newcastle Dental Hospital and has an active interest in Orthodontics. In 2006, Andrew passed the Diploma in Implant Dentistry from the Royal College of Surgeons and has been invited to return as a tutor on the course. He is currently working towards the Fellowship of the Faculty of General Dental Practitioners, the highest qualification attainable in general dental practice. Andrew is married to Janet, a physiotherapist, and they have a son, James. Outside work his main interests are racing a sports car in the Radical Enduro Series, supporting Medicals Rugby Club and Snowboarding, Windsurfing and Sailing. Dr David Row BDS.DGDP(UK) RCS GDC No David is originally from North Wales. He went to school at Sedbergh in Yorkshire and qualified in 1986 from Newcastle University. After spending a year in London as an Associate, he returned to the North East to work firstly in Killingworth and later in Ashington. David joined Kelvin Lodge in 1994 following the retirement of Mr Michael Atkinson. David enjoys all aspects of general practice and, in 1995, completed the Diploma in General Dental Practice at the Royal College of Surgeons. David is a member of the British Dental Association, the British Society of Periodontology, the Association of Dental Implantology, a member of the North of England Dental Implant Group and has been using implants in restorative dentistry since David s interests include playing golf, skiing, supporting Medicals and Newcastle Falcons Rugby Clubs and Newcastle United. David is married to Georgina, a hygienist who has also worked at Kelvin Lodge. They have a son, Sam, and a daughter, Elizabeth. Dr James Bough BDS GDC No James qualified from Newcastle University with Merit in Clinical Dentistry in 2007, having been awarded the John Hopkins prize for his support for fellow colleagues and joined Kelvin Lodge in James has continued to further his skills and knowledge by completing a large number of postgraduate courses including dental implant courses at the Sheffield Implant Centre and Newcastle Dental Hospital, the Core Course in Conscious Sedation at Newcastle Dental Hospital and the Inman Aligner accreditation. He is currently working towards his final examinations for the Diploma in Restorative Dentistry, which provides the skills and knowledge for advanced treatments for the most complex of cases. In addition to dental qualifications, James has also carried out a number of facial aesthetic courses in order to provide treatments such as wrinkle relaxing injections, dermal fillers and Genuine DermarollerTM therapy and is able to give personalised advice and treatment plans for skin therapy. James is a member of the Faculty of General Dental Practice (UK), The Royal College of Surgeons and the International Team for Implantology (ITI). He is an Honorary Clinical Lecturer at the Newcastle Dental Hospital and lectures to dentists on behalf of the Northern Deanery (the Postgraduate Institute for Dentists), a specialist dental laboratory and Ivoclar Vivadent UK. James lives in Tynemouth with his wife Lindsay and enjoys keeping fit and healthy, with many outdoor hobbies, particularly as the beach is on the doorstep! He is passionate about skiing, food and travelling to visit new places.
