2 Is your license current? 10/31/10 expiration Applicator, Operator, Applicator Business, Dealer and Dealer Business Renewals must be done each year Renewals mailed late summer each year Verify and update any changes to information and return renewal with fee prior to October 31 st

3 Recertification Must be completed every 5 years Credits required Core 8 Category 16 in each category licensed If Core certification is lost Must take Basic Pesticide training course if not attended previously Take prior to retaking the Core exam

5 Do I Need a Business License? Register as a pesticide applicator business if: You hire yourself out to apply pesticides Advertisement of pest control services Fees charged for services even if pesticide use is not directly charged

6 Responsible Applicator At least one person must be certified in all categories of work performed by business Accountable for pesticide use Himself/herself Any persons under direct supervision Available when needed 3 hours-ground transportation

7 Record Keeping The date of application For pesticides having a specific REI listed on the label or labeling, the date of application shall include the hour completed; and The re-entry time and date Place of application includes specific address, municipality and county. The brand or trade name of the pesticide used, the EPA registration #, and the active ingredient(s).

8 Record Keeping The Recipe used to mix the pesticide 1. The total amount of each pesticide concentrate used; 2. The total amount of diluent; 3. The size or area treated when rate per given area is on the label. The full name and license number of the person making the application The site of application

9 COMMERCIAL APPLICATION RECORDS FORM 1. For agricultural applications, the PLACE OF APPLICATION is the name and address of the farm and the specific field or land area and crop that was treated. 2. The brand or trade name of each pesticide used or symbol representing such name, providing the business also keeps a list which clearly correlates the symbol used with full and complete pesticide products names. 47 CONSUMER INFORMATION NOTICE Consumer Information Form is available on the PCP website Under the publications link Dear Customer: APPLICATOR OR BUSINESS NAME ADDRESS PHONE NUMBER As of November 19, 2001, the New Jersey DEP Pesticide Control Program instituted a new program that requires us to provide customers with information about the pesticides we will be using as part of the pest control program for your property. This CONSUMER INFORMATION NOTIFICATION provides you with this information. This CIN lists any label precautions for the pesticide(s) listed below that you should be aware of. You should also be aware that you may request notification of the exact date(s) of any pesticide application, and a copy of the label for each pesticide that will be used. BRAND NAME (CODE) CHEMICAL NAME (CODE) INSTRUCTIONS AND PRECAUTIONS We are also required to provide the following phone numbers for your use in the event you have any health related questions concerning the materials that we will be using: National Pesticide Telecommunications Network (General Questions) 1 (800) New Jersey Poison Information & Education System (Emergencies) 1 (800) New Jersey DEP Pesticide Control Program (This number is for pesticide regulations information, complaints & health referrals.) (609) As the customer, you are entitled by the New Jersey Pesticide Control Regulations to ask us for a copy of the label for each pesticide that will be used on your property, and if you request them, we are required to provide them to you. Furthermore, if this is a contract for termite control, we are required to give you, without your asking for them, a copy of the label of the termiticide(s) we will use and a copy of a summarization of the New Jersey Pesticide Control Program's Regulations regarding proper application of termiticides. Sanitation, as well as physical and biological control measures, should be considered as another part of a good pest control program. Pesticides may be used as another part of a good pest control program. Pesticides are substances used to control living organisms and vary in degree of toxicity. As the customer, I acknowledge that has provided me, in writing, all the information required by the NJ DEP Pesticide Control Program's notification program.
