T4 T5 T6 Thoracic pedicle screws percutaneously open and lumbar L1234 Tomita type total spondylectomy Transpedicular corpectomy open and then L1 2 remove XLIF cage. insert corpectomy cage Open TLIF or PLIF L3-4 Sacrectomy plus iliac bolts insertion Thursday Anterior high thoracic approach either sternotomy or clavicular variation Anterior L5S1 Discectomy and reconstruction using either artificial disc or instrumented cage screws +?- plate Remove device and then proceed to L5 corpectomy with reconstruction using expandable cage plus plate possibly. Preserve pedicles bilaterally Mini Thoracic discectomy Subsequent conversion into corpectomy using specialised retractor syn frame, max access. cage and plate inserted T10 High thoracotomy around scapular to see anatomy and access to T4 - no implants XLIF type procedure for L1/2 Open expand dissection to perform transdiaphragmatic retroperitoneal dissection of thoracolumbar spine and? repair diaphragm No bony resection
