2 DDES CCG The Durham Dales, Easington and Sedgefield Clinical Commissioning Group (CCG) was established in October 2011 and authorised as a statutory body in February The CCG is made up of 40 member GP practices. It covers a total population of around 272,000 over a large and diverse geographical area. The CCG comprises three localities within County Durham: Durham Dales, Easington and East Durham, and Sedgefield. These localities have worked closely together as commissioners of local healthcare services for many years. Clinical commissioning means that local GPs across our localities will now have greater opportunity to use their knowledge about healthcare to develop services that meet our patients needs. 6 Website Links Patient UK is a comprehensive web directory for UK patient information. It provides excellent links to support groups and voluntary organisations. National Electronic Library for Health (select the virtual Mental Health Library section). This has information for patients and clinicians. The Department of Health site provides access to the most up-to-date government health circulars, papers and reports. The Health Index Database enables you to search for a private specialist, nursing home or hospital. It holds discussions and is searchable by health community, e.g. depression or sleep disorders. The official NHS website. This provides basic advice to treat common symptoms. Also provides excellent links to support groups and associations. Practice Manager Mrs Karen Soks The Practice Manager is responsible for all administration and staff. Suggestions for improvements are always welcome and should be directed to her, as should any complaints. Our nurses are also trained to give advice/treatment to smokers wishing to quit. Administration Staff There are six receptionists, one secretary and one data input clerk. The receptionists have an important role and endeavour to deal with your queries, appointments, repeat prescriptions etc. as efficiently as possible. They have enquiries and calls coming from every direction, so please be a patient patient. When telephoning for medical attention, the receptionists may ask for a few details. They have been trained to make these enquiries in order that we can help you in the most appropriate way. 7

They work in close liaison with the Doctors and give support to patients too ill to attend surgery. They also assist with our over 75 health checks. They also liaise with Twilight Nurses, MacMillan Nurses etc. and give advice on appliances and aids: Contact No Health Visitors Work in close liaison with the Practice, concentrating on families with children under 5 years and especially new babies. 10 Appointments Non-urgent appointments You can consult with any of the Doctors or Nurses. Appointments can be made by either calling at the surgery or telephoning reception on You have the right to express a preference to receive services from a particular practitioner. You will be given the first available appointment to see the Doctor of your choice. Children under 12 must be accompanied by an adult. You will be encouraged to see the same Doctor that you have seen previously with the same complaint. Please note that one appointment is for one person only. Urgent appointments If you feel you need to see a Doctor urgently and are not able to wait for the next available appointment, you will be offered an appointment with the Doctor who is on call. The Doctor that you see may not be the Doctor of your choice. Please try to attend the surgery if at all possible. Do not be offended by questions the Reception Staff ask; they are asked to ascertain the urgency of each case. Telephone Advice Telephone appointments are available. Please let the Receptionist know the nature of your call so that the Doctor can have the necessary information to hand. Home Visits If your illness prevents you from attending the surgery, a home visit can be arranged by telephoning Please ring before a.m. GP visit is recommended if the patient is a truly bed bound patient The Doctor will advise you whether you should be seen in your own home, the surgery, hospital or urgent care centre. GP visit is not useful: In some cases, to visit would not be an appropriate use of the GP service. Common symptoms of childhood illnesses: fevers, cough, cold, earache, headache, Diarrhoea, vomiting and most cases of abdominal pain. It is not necessarily harmful to take a child with a fever outside. These children may not be fit to travel by bus or walk, but car transport may be available from friends, relatives, taxi firms or voluntary services. Adults with common problems such as a cough, sore throat, influenza, back pain or abdominal pain are also readily transportable by car to a health centre. Common problems in the elderly, such as joint pain and general malaise, would also be best treated by consultation at a Doctor's premises. The exception would be the truly bed-bound patient. The surgery premises are equipped for a more full examination. 10

11 Out of Hours Care Please call the NHs (111) service. A Healthcare Professional can be contacted when the surgery is CLOSED by ringing 111. NHS 111 Service provides emergency care during the out of hours period -6:00pm to 8:00am on weekdays and throughout the whole of weekends and bank holidays. Access to healthcare services during the out of hours period will be via a single telephone number 111 (except in the case of 999 emergencies). The number will give you access to a range of services once your details have been taken. The location of services If you need to see a healthcare professional out of hours, you will be offered an appointment at the Urgent Care Centre (UCC) at Peterlee Community Hospital, although home visits can be provided for patients who are very unwell. Appointments to attend the UCC or to be seen at home will be allocated via the single telephone number. Patients will be asked to make their own way to and from the UCC although for patients with particular needs, transport can be arranged. If there is any delay in responding to an absolute emergency, you are instructed to dial 999 and ask for the Ambulance Service. Repeat Prescriptions In some cases, with the Doctor's consent, you may obtain repeat prescriptions from the receptionist. You may either hand in the print-out attached to your previous prescription, or there is a request slip for completion at reception; put it in the box provided in Reception or post it to us. We do not take repeat prescriptions over the telephone. Prescriptions will be ready for collection after 48 hours or 2 working days. (Please note if you hand your request in on a Friday it will not be ready until Tuesday). If you are unable to pick up your prescription, you can send a stamped addressed envelope and your prescription will be posted to you, or the local Chemists do offer a collection and delivery service, which you must arrange yourself by contacting the chemist of your choice; we do not arrange this for you. Prescriptions will not be given to children under 16 years. You can also order prescriptions online. Please see/speak to the receptionist, who will print off password and details. Once this has been done, online requests can be made via the following link: Test Results Patients are advised to ring for their test results arranged by the GP or Practice Nurse 7 days after the test by calling or telephoning the surgery after 1.30 p.m. Test results arranged by hospital doctors will be available from the hospital. 11

12 Newly Registered Patients Patients will be asked to bring in their medical card if possible as it contains your NHS number. If you do not have your card, fill out a Patient Registration Form and Health Questionnaire, putting as many details on as possible. An appointment for a health check with a Nurse needs to be made for patients over 6 years. A Doctor may need to see them before they can be registered. A urine bottle will be given to the patient to bring a sample when attending the health check. You may choose to register with any doctor in the practice. A practice booklet will also be available to the patient. Durham Dales, Easington and Sedgefield Clinical Commissioning Group is a party to the contract and from whom details of medical services in the area may be obtained.. Also, if you are waiting for a hospital appointment, please inform the hospital as well. Violent or Abusive Patients The practice has the right to remove a violent patient to safeguard all those who might have reasonable fears for their safety. These include members of the practice, other patients and any other bystander present where the act of violence is committed or the behaviour took place. Violence includes actual or threatened physical violence or verbal abuse leading to a concern for a person's safety. In this situation, we are obliged to notify the patient in writing of their removal from the list and record the facts of the removal and circumstances leading to it. Access to Medical Records New Seaham Medical Group has an Access to Medical Records Policy; please ask to speak to the Practice Manager for details. 12

13 Services Provided (ALL SERVICES PROVIDED BY THE PRACTICE ARE CONFIDENTIAL) Family Planning The Practice does provide full contraceptive services. A full range of contraceptive methods is available: barrier methods, diaphragms, caps, IUD, the Pill, injectable contraceptives, emergency 'morning after' contraception and natural family planning. There is a confidential counselling service for all patients, including young people. Pregnancy tests, pregnancy counselling and referral for termination can be arranged if appropriate. Maternity Care If you are planning a pregnancy, taking Folic Acid 400mcg per day helps to prevent spina bifida in babies. If you suspect that you may be pregnant, please make an appointment with your Doctor in the usual way. If the pregnancy is confirmed, you will be invited to attend the antenatal clinics held at the surgery every Wednesday at 1.30 p.m. in conjunction with the midwives. You will need to make an appointment for a check-up six weeks after the birth of your baby. It is a good idea to arrange this check to coincide with your child's six-week check-up. Child Health Services Baby Clinic Held Every Wednesday between 9:30am 11:30am, Adelaide Row by appointment. The Doctor and Health Visitor are able to discuss any of your baby's problems. You will need to make an appointment for a check-up six weeks after the birth of your baby. It is a good idea to arrange this check to coincide with your child's six-week check-up. A normal appointment is necessary for an ill child. Immunisation Clinic Held Every other Wednesday Between 9:00am 11:00am by appointment. Elderly Health Checks If you are aged over 75 years we offer an annual consultation with the District Nurse. This can be done at the surgery if you are well enough to attend or at your home. Minor Operations Dr R K Dusad has been accredited to do minor surgery. A minor op clinic will be held on the first Friday of every month. We have cryo-cautery for removal of warts on the third Thursday of every month. Asthma / COPD Clinic Seen by appointment with the Practice Nurse 13

14 Diabetic Clinic Every third Tuesday of the month between 9:00am 12noon by Dr Usha Dusad and the Practice Nurse Well Woman Clinic By appointment with either the Practice Nurse or the Doctor. CHD Clinic By appointment with the Practice Nurse. Non NHS Medical Examinations Please ask at Reception for details of charges for examinations not covered by the NHS. Immunisations Travel Vaccinations Please make an appointment, at least eight weeks in advance of your holiday, to ensure adequate cover. You may be required to collect a prescription for your vaccinations prior to attending your appointment with the practice nurse. A fee may be charged for certain vaccinations that are not covered by the NHS. Flu Vaccinations An influenza vaccination is particularly recommended for patients with heart, lung or kidney disease, diabetes and residents of nursing or rest homes. Please contact the Reception staff during October for details of the vaccination dates and to make an appointment. If you are unable to attend the surgery, we can arrange a home visit. See also 'Child Health Services' Smoking Cessation By appointment with the Smoking Cessation Advisor. The surgery also provides other Enhanced Services; further information regarding these services will be available from your Doctor or Nurse. Please speak to our Practice Manager, or a Receptionist, who will provide you with further information. It is our aim to improve our services and resolve any problems promptly. Dental Emergencies If you need dental care, you should call your own dentist. However, if you do not have a dentist or have a dental emergency, call NHS Direct. 15

16 Self-Treatment of Common Illnesses NHS Many common aches and pains can be simply treated at home without the need to consult a doctor. Back Pain Back pain causes 13 million working days to be lost in Britain each year. The spine supports the whole weight of the upper body so it is understandable that it sometimes goes wrong. Because of the complex nature of the spine it is advisable to consult your doctor if back pain persists for more than a few days. If, as is usual, the pain has been caused by abuse, i.e. lifting too heavy wrights, etc., be sensible and take things easy. Take care to sit as upright as possible with a support for the small of the back. Take aspirin or paracetamol which will not only relieve the pain but will help to relieve inflammation. Your doctor may well prescribe stronger drugs, heat treatment, gentle exercise or some kind of supportive corset. Bed Sores Bedsores are far easier to prevent than cure. They are caused by prolonged pressure to certain parts of the body when lying in bed for long periods. They can be prevented by encouraging the patient to shift position as often as possible. Take care to smooth out creases in the bottom sheet to avoid irritation. If red marks appear at the pressure points such as heels, elbows, buttocks and hips, inform the doctor before they get worse. Burns Apply large quantities of cold water to the affected area as soon as possible and maintain this until the pain subsides. This may take as long as 15 minutes! If the skin is unbroken but blistered, apply a loose, dry dressing. Colds Even in this day and age there is still no magic cure for the common cold. Go to bed, take plenty of drinks. If you have a headache or are feverish, take aspirin or paracetamol. Do not bother to take antibiotics, as these will have no effect! Sprains Treat with a cold compress, containing ice if possible, for 15 to 30 minutes to reduce the swelling. Then apply, firmly, a crepe bandage and give the sprain plenty of rest until all discomfort has subsided. Further strain will inevitably lead to further swelling and a longer recovery period. 16

17 Stomach Pain Most attacks are not serious and are usually caused by indigestion or wind. A hot water bottle will often relieve the symptoms and, in the case of indigestion, a teaspoon of bicarbonate of soda in half a glass of water will help. If the pain lasts for longer than eight hours or increases in intensity you should consult your doctor. Diarrhoea In adults, diarrhoea is usually caused by a virus infection and is therefore unable to be treated directly. The symptoms can usually be eased by the traditional kaolin and morphine mixture or by medicines containing codeine. Holiday diarrhoea is often due to bacteria. Again, kaolin and morphine can be taken. Consult your doctor if the symptoms persist for more than a few days. Diarrhoea in very young children and babies needs careful attention. Most babies have loose bowel action during their first six months due to their predominantly liquid diet. Sudden bouts of unusually watery diarrhoea should be treated by taking the baby off solids and feeding them a cooled solution of boiled water with a teaspoon of sugar and half a teaspoon of salt to the pint. If the symptoms persist for more than 24 hours, or are accompanied by vomiting or weakness, consult your doctor. Nosebleeds Sit in a chair, lean forward with your mouth open, and pinch your nose just below the bone for approximately ten minutes, by which time the bleeding should have stopped. Avoid hot drinks or hot food for 24 hours. If symptoms persist, consult your doctor. Minor Cuts and Grazes Wash the wound thoroughly with water and a little soap. To stop bleeding apply a clean handkerchief or dressing firmly to the wound for about five minutes. Cover with a clean dry dressing. Sunburn Treat as for other burns with cold water to remove the heat. Calamine lotion will relieve the irritation whilst paracetamol will also help. Children are particularly susceptible to sunburn and great care should be taken to avoid overexposure to the harmful effects of the sun. Insect Bites and Stings Antihistamine tables can be obtained from the chemist without prescription and will usually relieve most symptoms. NOTE: Bee stings should be scraped away rather than plucked in order to avoid squeezing the contents of the venom sac into the wound. 17

18 Head Lice These creatures, contrary to popular belief, prefer clean hair and are, therefore, not a sign of poor personal hygiene. Medicated head lotion can be obtained from the chemist without prescription. Chickenpox On the first day a rash appears as small red patches about 3-4mm across. Within a few hours of these developing, small blisters appear in the centre of these patches. During the next 3 or 4 days further patches will appear and the earlier ones will turn crusty and fall off. Calamine lotion may be applied to soothe the often severe itching. Cool baths may also help. The most infectious period is from 2-3 days before the rash appears and up to 5 days after this date. Children may return to school as soon as the last crusts have dropped off. German Measles (Rubella) The rash appears during the first day and usually covers the body, arms and legs in small pink patches about 2-4mm, and doesn t itch. No other symptoms are usually present apart from occasional aching joints. It is infectious from 2 days before the rash appears until the rash disappears in about 4 or 5 days from that day. The only danger is to unborn babies and, therefore, it is important that all contacts are informed in order that anyone who may be pregnant can contact their Doctor. Measles The rash is blotchy and red and appears on the face and body around the 4 th day of illness. It is most infectious from 2 or 3 days before the rash appears until 8 or 10 days after that date. IMMUNISATION CAN PREVENT THIS DESEASE. Mumps Symptoms are swelling of the glands in front of one or other ear, often followed, after a couple of days, by swelling in front of the other ear. It is infectious from 2 or 3 days before the swelling appears until 8 to 10 days after that date. If the pain is severe you should consult your Doctor. IMMUNISATION CAN PREVENT THIS DESEASE. 18
