1 Preamble The purpose of this inter organizational survey is to gather data regarding transplant center practices for preparing cord blood units for administration, including characterizing and confirming the identity of cord blood units and thawing, diluting, and washing the units. You will be asked 34 questions; some questions relate to four different scenarios regarding cord blood units. After using this version to compile answers to the questions, submit your responses online at The time to complete the survey is estimated to be about 20 minutes after compiling all information for your transplant center. 1.
