2 Premera Long Term Care We ve taken the complexity out of long term care coverage with clear language, great flexibility, and amazingly few rules. Premera Long Term Care (LTC) is designed to be a tax-qualified cash product delivering highquality coverage and exceptional peace of mind. 1
3 You don t have to complicate your lifestyle to maintain it. Life s simple joys. Home. Hobbies. Sharing everyday experiences with friends and family. Isn t that what it s all about? But what does it take to protect these basic pleasures? How would your life change if you needed long term care? The high cost of care could make it harder to preserve the life you ve built and connections you cherish. How can you know today what it will take to provide for your lifestyle tomorrow? Now there s long term care coverage that doesn t make you predict the future to protect it. Traditional insurance only covers specific services. Premera LTC from Premera Blue Cross doesn t define the services you can receive. It gives you the means to obtain the services you need. With Premera LTC, once you re eligible, you automatically receive a monthly cash benefit. And no one tells you which services you can access or limits the duration of them. It s that simple. 2
4 You just send me a check? Yes. You receive the monthly cash benefit you selected for as long as you need care; up to your lifetime limit. Use the money as you choose for the care you want, from whomever you want, wherever you want, anywhere in the world. Receive care from a nurse or from your niece. With Premera LTC, there are no restrictions, third-party bills to submit for reimbursement, or limits. You decide what s best for you. 5 Premera LTC for individuals how does it work? With such confusing long term care policies out there, you may be shocked by the plain language of ours. But you really can get the care you want and need on your own terms. To be as clear as possible, we ve answered the most common questions about Premera LTC right here: How is this different from other long term care insurance? Traditional long term care insurance plans can be complicated. They may require you to submit provider bills, endure a lengthy approval process, and accept limits on the types of services you can receive or the amount you can receive each day. Premera LTC is different. Whether you want to stay in your home or need the care of a nursing facility, as long as you are eligible and need care, you will receive a monthly cash benefit. Simple as that. What happens when I need to use my benefits? After you are determined eligible for long term care benefits and have completed your Elimination Period, you need only submit one simple request for payment each month. Then you will begin receiving your monthly cash benefit. What can I use the benefits for? You can get whatever you need to balance your life with your need for long term care. That means every service covered under traditional reimbursement policies and more including nursing home, assisted living, home care, respite care, bed reservation, alternate care, caregiver training, or equipment. What if I need help or advice? We re here. When you need assistance, our Personal Care Advisors can be a source of comfort as well as information. Because we pay benefits regardless of the services you use, our Personal Care Advisors are only concerned with your needs. You can get this help any time your contract is in force free of charge. How is Premera LTC priced? Premera LTC is highly competitive with traditional long term care insurance, which can be complicated. 7 riders Inflation If you choose to add Inflation Protection to your contract, you can increase your Maximum Monthly Benefit to protect against the anticipated increases in the cost of long term care. Our three optional riders include: 5% Compound No Maximum Annual increase in your Maximum Monthly Benefit equal to 5% of your prior year s Maximum Monthly Benefit. 5% Compound 2x Maximum Annual increases in your Maximum Monthly Benefit equal to 5% of your prior year s Maximum Monthly Benefit until it is twice the original amount. 5% Simple Annual increase in your Maximum Monthly Benefit equal to 5% of your original Maximum Monthly Benefit. Nonforfeiture With these riders, you can add a contract feature that returns at least part of the premiums you paid if you cancel your contract or let it lapse: Contingent Nonforfeiture (included in base) or Shortened Benefit Period Continued coverage equal to premiums you have paid if your contract has been in force for three years and lapses. Full Return of Premium* If you die while the contract is in force, we will refund all premiums you have paid for the contract and riders. Available only to enrollees age 65 and under. Return of Premium* The same protection as Full Return of Premium except any benefits paid or payable are deducted. Available only to enrollees age 75 and under. Restoration of Benefits Whenever a period of 180 days elapses in which you were not chronically ill, we will restore your Lifetime Maximum to what it would have been had no benefits been paid under your contract. Not available with Unlimited Lifetime Maximum. Not available in conjunction with the Shared Care Rider. * You may not select both Return of Premium and Full Return of Premium. Joint Benefits Premera LTC allows you and another person, referred to as a Care Partner, to purchase together and share benefits on each other s contract. You each must purchase these riders to share the protection: Survivor Benefit** If one of you dies after 10 years, the other pays no further premiums. Must be purchased by both Care Partners and have the same effective date. Shared Care If you use your own Lifetime Maximum you can, with your Care Partner s consent, use the Lifetime Maximum of the other contract. Partners must always reserve at least 24 months of benefits on the other s contract. If one Care Partner dies, the surviving Care Partner can assume the deceased Care Partner s remaining Life Maximum. Not available with selection of 24 months or Unlimited Lifetime Maximum. Not available in conjuction with the Restoration of Benefits Rider. Shared Waiver** When the premiums are waived for one of you, they are waived for the other as well. ** Not available if Partners age difference is more than 15 years. Additional premium required. Refer to your Outline of Coverage. The benefits under this contract are paid without regard to the type and amount of actual long term care expenses. You should consult your tax advisor with respect to the potential tax implications of purchasing this coverage. 6
