1 JOB DESCRIPTION: RESPONSIBLE TO: PURPOSE OF POST: Team Leader Care Manager To support residents living in registered residential care lead an independent, healthy, positive and fulfilling life. To deliver personal care and household care that is personalised for each resident. To treat each person with respect and dignity and to behave in a positive manner at all times that is not derogatory to the resident, visitors, Gateway or its representatives. To perform the line management Care Workers To undertake other activities identified by Gateway management that will assist with the operation of the service and organisation. To administrate Health and Safety protocols and report issues. 1 Policies & Procedures 1.1 To act at all times in accordance with Gateway Housing Association s (GHA) published policies and procedures. 1.2 To ensure compliance with GHA s policies and procedures. 1.3 To ensure that the requirements of the Data Protection Legislation is complied with in carrying out the duties of the post. 2 ICT Systems (Information and Communication Technology) 2.1 To develop the necessary skills in using the GHA s ICT system in the performance of your duties. 2.2 To follow good ICT housekeeping practices in relation to back-up procedures and filing of reports and documents. 1

2 3 Key Responsibilities 3.1 To work as part of a team to provide personal care and support to residents living within GHA properties. 3.2 To be available to work as and when required during a 24 hour day over 7 days a week in accordance with the needs of the service. 3.3 To provide care and support to residents who require it to: Get up from and return to bed Maintain good personal hygiene (including oral hygiene) Dress and undress as required Wash and bathe Use the toilet and maintain their continence when necessary. 3.4 To support residents who have difficulty or are unable to move around independently to transfer and move around the home and community as required. 3.5 To provide support to residents so they can use their mobility equipment. 3.6 To provide care and support to people who are at the end of life. 3.7 To promote the mental and physical activity of residents by: Communicating with them through their preferred method Supporting them to access the community Sharing with them in activities such as reading and writing and engaging with hobbies and recreational activities. 3.8 To make and change bedding, tidy bedrooms; carry out light cleaning duties and empty and clean continence aids. 3.9 To make sure the home is clean, tidy and free from clutter To encourage residents to dress appropriately each day taking into account the weather, the residents clothing preference, personal appearance and to ensure that clothing is clean, free from stains threadbare and in a good condition To set dining tables, prepare light meals and drinks, serve meals, support residents who require it to eat and drink, clear up after meals and wash crockery and utensils To undertake the supervision of care workers including: Carry our regular one to ones with care workers on a minimum of six weekly basis Identify training and development requirements of care workers Contribute to the annual appraisal of care workers 2

3 Provide motivation and leadership for care workers during shift leading Contribute to the induction of new staff Provide guidance and mentorship Give clear instructions and make considered decisions Coordinate care workers through delegation of duties to care workers during shift work Coordinate the administration of medication To facilitate fulfilling and engaging daily activities and encourage residents to participate with them while respecting the choices that they make To follow the instructions set out in the residents care plans, support plans and risk assessments To advise the care manager of any concerns or difficulties experienced with the service provided by you or others to residents To advise the care manager of any ideas that may enhance or improve the service that is delivered to residents To carry out the instructions given by the care manager and/or management team To actively talk and listen to residents and to spend time to understand their personality and preferences in order to build positive relationships with them while maintaining professional boundaries at all times To maintain accurate, concise, timely and up to date daily monitoring records in accordance with GHA policy and procedure To write, review, keep up to date and communicate to the team care plans and risk assessments for residents To report and record changes in a residents needs and abilities to ensure they can be communicated to the rest of the team, the care and support package can be updated and so other actions required actions can be taken To record and report any accident / incident that has occurred, no matter how minor, to the care manager or on-call manager as soon as possible and within 24 hours To actively encourage residents to work towards their goals which includes developing care and support plans and identifying appropriate and meaningful activities within the scheme and the local and wider community that are appropriate to their lifestyle and choices To work in a person centred way to meet the holistic needs of the 3

4 resident To monitor progress against care and support plans in conjunction with residents, other team members and related professionals To provide residents with advice and support to maintain their personal safety and security, including raising awareness about potential abuses and how to balance risk with an active and meaningful life To monitor the physical and mental health of residents, ensuring their health and social needs are met To actively encourage residents to participate in the planning and development of service provision To support residents to forward plan for End of Life and ensure that Enduring Power of Attorney, Wills, Appointee-ship and Advanced Decisions are considered To provide help to residents in the event of emergencies; to call the emergency services or a doctor; to then provide first aid or other personal care, if appropriate and; in an emergency remain with the resident until help arrives To be responsible for the implementation of GHA Health & Safety procedure within the scheme. This will include particular attention to: Policy and practice on fire precautions and action in the event of fire. Creating a safe environment for all individuals in the unit but with particular attention to those with disabilities, such as limited mobility, hearing, eyesight or mental concentration To wear uniforms provided by GHA when at work and use the Personal Protective Equipment (PPE) provided in the appropriate situation To follow infection control guidelines set out by GHA To provide a daily report during hand over to the person in charge of the next shift. 4 Building 4.1 To liaise with building maintenance and arrange access as required, calling in immediate assistance for breakdowns of key services such as lifts and heating. To ensure that light bulbs in communal areas are regularly checked and replaced when necessary. 4.2 To encourage the appropriate disposal of tenants refuse and promote re-cycling. To inform the Estate Services Manager when bulk rubbish requires removal and disposal or when fly-tipping has occurred. 4

5 4.3 To ensure the general security of the building including testing the alarm and communication system. 5 General 5.1 To attend team meetings, briefing sessions for staff and GHA staff conference and other events when required. 5.2 To attend training when requested. 5.3 To attend regular 1 to 1 s and Annual Individual Performance Planning meetings with your manager. 5.4 To promote the GHA in a positive and professional manner at all times. 5.5 To perform other duties as may be reasonably required. 5

6 PERSON SPECIFICATION: Team Leader Education and Qualifications Level 3 NVQ/QCF qualification in Health and Social Care or similar Current Emergency First Aid at Work qualification Experience Significant experience of delivering personal care Staff / team management in social housing An understanding and experience of preparing for CQC/Commissioner Inspection and assessment Experience of target setting and monitoring performance Implementing Policies and Procedures Identifying training needs of staff Observation of staff competence in delivering personal care Knowledge and Skills Knowledge of CQC Essential Standards of Quality and Safety Understanding of dignity within a care home context Good knowledge and experience of medication administration Excellent knowledge of Safeguarding Adults legislation and reporting procedures Numerate Computer Literate, able to utilise and manage complex IT systems Analytical skills Excellent time management skills, able to work under pressure Self motivated to achieve targets. Excellent communication skills required orally and written Excellent Administration skills Effective working within teams and individually.
