2 Annual Membership Conference Dear Colleagues: On behalf of the Board of Directors of the Home Care Association of New York State, I invite you to attend our Annual Membership Conference 2011, HCA s premier signature event, to be held on May 22 to 24 in Saratoga Springs. Without a doubt, 2011 has been the home care community s most challenging year ever. Yes, the state budget cuts and Medicaid system changes are monumental; but if there is anything I know about New York s home care system, it is home care s built-in power to innovate and adapt, due in large part to the expertise and resilience of its people. In like fashion, HCA is prepared to work alongside you in this challenging new landscape, to help your organization become better and stronger, even in the face of such massive changes. This year s Annual Conference is very well timed, as it provides HCA with one of the first collective opportunities as an organization to regroup, offer you new perspectives that reflect the changing landscape of home care in New York, and continue to capitalize on the strong, sustained and energized grassroots architecture we built up this year to remind policymakers at all levels about the importance of home care in the community. In fact, our conference agenda has been redesigned in recent weeks to ensure that we are bringing you the most relevant programs for arming you with information, preparing you for the challenges ahead, and assisting you in navigating an extraordinarily complex new home care environment. Beyond policy and education, HCA s Annual Conference also provides a much-needed opportunity for us all to recharge, replenish and reinvigorate. This is why I am excited about our conference theme, Calling the Tune, and its twin notions of inspiration and leadership: two vital qualities for success amid any significant challenges. Musicians will know that Calling the Tune is a term derived from the on-stage act of a band leader calling out songs for his or her band-mates to perform. Much like the role of home care leaders especially in a new and challenging environment the literal act of calling the tune in music performance requires flexibility, control, responsiveness to one s audience, as well as a sense of command, vision, and confidence. In addition to some great musical entertainment, the top-rated presenters lined up for this year s event will help you call the tune at your organizations as you explore new ideas, plan for the future of home care, enhance your leadership skills, and celebrate the heroism of your colleagues and staff during our Annual Awards Dinner on May 22. Other great performances are scheduled for this year s event as well, including the displays of over 50 exhibitors who will be presenting at our It s Just Lunch session and in the exhibit hall, plus our HCA Members Got Talent event. 3 Annual Membership Conference Sunday, May 22 10:00am to 5:00pm Conference Registration Hotel Lobby Pre-Conference Workshop Noon to 2:00pm Using Data to Improve Your Performance In this workshop, participants will learn how to develop a solution-based roadmap to improve your organization s performance using a wide range of home health quality data. Participants will explore the three domains of quality improvement data (Outcomes, OASIS-C Process, and Satisfaction) and understand their focus, role and opportunities for performance improvement. This presentation will include a review of benchmarks, PI models, and their culture and approach. Case examples of roadmaps will include acute care hospitalization and functional outcome roadmaps. Participants will learn to develop roadmaps for their own specific initiative or area of focus through this lively presentation and group discussion. There is no charge to attend this pre-conference workshop, but registration is required. Sue A. Blockberger-Miller, MSN, RN, Director of Education, Outcome Concept Systems, Inc. (OCS). Ms. Blockberger-Miller has over 30 years of experience in clinical and operational components within home health and long term care. Her current role is dedicated to interpreting and advance analysis for improving organization performance with operational and publicly reported measures. Sue works with audiences ranging from dedicated performance improvement specialists to senior leaders of home health organizations. Sue is a participant of the Ohio Council for Home Care s Research Committee and is a frequent lecturer at regional and national conventions on clinical, operational, and financial issues facing home health agencies. Roger A. Herr, PT, MPA, COS-C, Director Program Management, Outcome Concept Systems, Inc. (OCS). Mr. Herr has worked in home health for over twenty years including operations, management, and as a site visitor. He currently works in Program Management at OCS, a division of NRC Picker. The exhibit area is open throughout the morning as well. 8:30am to 9:30am Calling The Tune: An Address to the Membership During an address to the membership, HCA President Joanne Cunningham will discuss the implications of the recent state budget battle on New York s home care community, outlining a strategy for where HCA and the home care community goes next. In addition, Ms. Cunningham will discuss HCA s federal agenda, highlighting opportunities ahead for home care providers in New York State. Hear how your continued support for HCA is necessary in Calling the Tune for 2011 and beyond during this time of enormous challenges for New York s home care community. 9:45am to 11:00am State Policy Changes Understanding Home Care s Role in the Emerging Policy Framework Cuomo Administration Officials (Invited) This year s state budget included massive and far-reaching changes to New York State s home care system. Be the Lead Conductor: Succeeding with 2011 Medicare Home Health Changes To succeed in 2011 and beyond, home health leaders must identify what management and supervisory protocols are in need of updating in order to match the new regulatory requirements in home health. Through discussion and interactive exercises, this workshop will help leaders integrate the changes in philosophy required by the latest reforms, resulting in improved clinical outcomes and productivity. 3 Arnie Cisneros, PT, President of Home Health Strategic Management, speaks nationally on utilization review for home care, best practice adaptation and clinical management for quality outcomes. 5 PROGRAM AGENDA 11:15am to 12:30pm Educational Workshops Breakout Sessions will be repeated at 2:00pm. Breakout Sessions Part 1 Avoiding the Sour Notes: Audit Best Practices The Patient Protection and Affordable Care Act (PPACA) significantly ramps up existing health care audit initiatives to identify fraud and abuse. Home health providers remain the focus of multiple efforts to ensure accurate reimbursement for care and services. Who are these auditors and what tools do they have to strengthen their target efforts, and what are your colleagues doing differently to eliminate risks inherent in the OASIS and home health documentation? In this session for leaders and patient care directors, learn how you can customize agency processes to enhance clinician audit proficiencies. Patricia (Trish) Tulloch, RN, BSN, MSN, HCS-D, is a Senior Consultant with RBC Limited and is a seasoned clinician and a certified coder with more than thirty years in the health care industry. She consults with multiple health care organizations and agencies across the country to meet regulatory and accrediting standards. Ms. Tulloch is also the author of numerous industry related articles and is a regular contributor to HCA s education newsletter, The Educator. 12:30pm to 2:00pm It s Just Lunch HCA is pleased to host over 50 vendors traveling to our Conference to show you the latest in products and services that support you, your agency and the patients you serve. In addition to running exhibit booths, vendors will move from table to table at designated times during lunch, to briefly share their information with you. It will be your opportunity to learn, ask questions, and possibly win a great prize. What could be easier? This exhibitor lunch session is new this year and is patterned after the popular dating service called It s Just Lunch: a great way to interview prospective vendors. The conference will provide plenty of time for participants to network with peers, too. 2:00pm to 3:15pm Breakout Sessions Repeat 3:30pm Making Music: A Teamwork, Communication and Leadership Skills Adventure There are only 12 tones in all of music; but to write a great song, you need to have the right combination of notes a melody along with just the right words: your lyric. The same holds true in home care. As with music, the right combination of word and tone can bring harmony to your organization. In this session, participants will learn how to enhance their skills in leadership, teamwork and communication through an adventurous exercise in music. 7:30pm HCA Members (and friends) Got Talent Can you sing? Can you play an instrument? Even if you can only tap your toes or snap your fingers, you won t want to miss this fun and exciting event modeled along the lines of American Idol. A subsidized cash bar and lots of fun are guaranteed. During this session, we ll be holding a Garage Band Sale! 6 Tuesday, May 24 PROGRAM AGENDA 7:30am to 8:30am Breakfast Buffet Begin your day with a song in your heart and a smile on your face as you take time to visit with colleagues over breakfast. 8:30am to 10:00am A Symphony of New Ideas The recently enacted state budget as well as the federal health reform law both emphasize the need for enhanced care coordination models to be utilized as an effective tool to bring down costs for the highest cost users of home and long term care services. While the state and the federal approaches vary greatly, the concept of care coordination to treat high cost, high utilizer patients is a sound one. Hear from several HCA members that have both Managed Long Term Care Plans (MLTCs) as well as Long Term Home Health Programs (LTHHCP) within their organizations to talk about the merits of both programs, outlining their differences and how they work. 10:15am to 11:15am Emotional Eloquence The Lost Language of Leadership A major part of your job as a home health agency leader is to make presentations, motivate your team, or persuade others into action. Through the art of corporate storytelling a fusion of the practical and theatrical leaders can learn how to engage, influence and inspire others with just the right story, with just the right elements, that will touch others at a deeper and more meaningful level. Doug Stevenson is president of Story Theater International, a speaking, training and consulting company that trains thousands of executives to be amazing storytellers and speakers. He is also the author of the book Doug Stevenson s Story Theater Method. Note: Several members of HCA s Board of Directors participated in this program at the Northeast Home Health Leadership Summit and came away with rave reviews.
