1 FMS Service Model Specification Updated April 2015 Group Accommodation Model MDS Code: 1.04 Service model objectives: To provide high level supported accommodation for an individual or small group of people in a community setting. The Group Accommodation model applies to a range of accommodation types, such as one bedroom single storey villas and apartments, 2-3 bedroom townhouses, five bedroom single storey homes, and co-located homes and/or apartment groups, for not more than 10 individuals on a site.. The model covers a range of accommodation options which support people with a disability in jointly occupied or solely occupied households of up to ten people in premises attached to, or in close proximity to other similar premises, with each able to be operated independently, with or without shared staffing. Organisations funded to provide the Group Accommodation model are required to comply with the ADHC sector-wide system for managing vacancies in supported accommodation services. To support the primary objective, the Group Accommodation Model will: 1. use a flexible person-centred planning approach to support the individual; 2. provide a tailored program of support and care; 3. assist people with essential activities of daily living (ADL and IADL); 4. assist people to live, access and participate in the community. Group Accommodation services will operate according to the NSW Disability Inclusion Act 2014 and related service standards. The service model will provide innovative, appropriate, and efficient and effective 24 hour care and support for people to live in a community setting. Each individual s needs and functional abilities will determine the intensity of the support provided, and be used to guide the identification and placement of an individual into the most appropriate accommodation model. The support needs of individuals served by the Group Accommodation model may increase (through deteriorating function), or decrease (through increasing skills) over time but must remain within the parameters of the allocated support hours. People may be able to move into less intensive accommodation models with skills development. The Group Accommodation model therefore needs to be flexible and responsive to the individual s changing needs. Whilst not all accommodation services under the model are suitable for children under the age of 16 years (e.g. co-located services), this model may be provided to children, young people and adults of all ages with an intellectual or other disability, who have high support needs and require fulltime daily support, and who may also have additional health care and behavioural support needs. 2 Priority of access groups: The Group Accommodation model is designed for eligible people with a disability with high support needs requiring 24 hour support, and who are unable to live independently. Potentially suitable people will be identified and allocated through District allocation processes as per the Accommodation Support Register (ASR) Guidelines. Services and Activities: The descriptors for the Group Accommodation include: 1. the service provides high level 24/7 support to a group of up to ten (not more than 5 in a dwelling) people with a disability; 2. each person resides independently or with other people with a disability in their own or leased accommodation on the same site or in close geographic proximity; 3. each person receives a range of support services relating to personal care and daily living, that are individualised, planned and based on assessed support need; 4. the delivery of support services through a person-centred approach which assists the person to reside in their living arrangement. Accommodation Model Group Accommodation may be provided to children, young people and adults of all ages who: have an intellectual or other disability, high support needs and require full-time daily support. and may also have additional health care and behavioural support needs. The intensity of support required to adequately meet assessed need is expressed by the number of hours of staff input required to ensure appropriate outcomes for each person. The service activities of the Group Activities include, but are not limited to: 1. assessing risk and developing appropriate support plans, other health care plans, behaviour intervention plans, etc. where appropriate; 2. delivering the individual s person-centred support plan for each resident, which includes supporting tenancy obligations if applicable; 3. providing support and assistance in personal care (toileting, bathing, grooming, menstrual care, dressing, etc.); 4. providing support and assistance with access to health and medical care, domiciliary services, support to undertake other life activities, (household management, household routines, meal preparation, budgeting, health management, etc.); 5. providing skills development in line with person-centred planning principles; 6. assist and support with decision making and choice; Group Home Service Specifications pg. 2

3 7. maintaining and developing communication and social skills including facilitating family and neighbour relations 8. assist and support access for participation in community life, including access to community services The specific details of the services to be provided would be set out in the resident's individual support plan. This then informs the planning, organisation and coordination of services necessary to implement the plan. The service will provide each resident assistance with: Personal care completing or monitoring the completion of tasks such as: eating and drinking hygiene dressing and grooming bathing and toileting Communication developing communication skills developing interpersonal skills Daily living care completing or monitoring the completion of tasks such as: preparing meals and cooking clothes washing, drying and care shopping for food and household goods required for everyday life household cleaning, maintenance of hygienic and ordered living environment Social skills and relationship: developing and maintaining contact with significant others (family etc.) developing and maintaining social contacts and friendships developing and maintaining good neighbour relations Community access: travelling independently using public transport using community facilities e.g. library, swimming pool, banks Leisure and recreation: participating in fitness, sport, recreational and leisure groups participating in adult education, employments skills and training Day activities: accessing day activities, training, employment, volunteer and other day time activities, but not providing them Group Home Service Specifications pg. 3

4 Health care: attending fitness, nutrition, sexual health and relationship programs attending to health and dental care as prescribed and documented by qualified medical and allied health practitioners Managing behaviour: support the development of positive behaviours minimising behaviours that restrict participation in community activities or pose risk to self or others developing a contingency after hours plan for emergencies Skills development: maximising personal independence through skills building minimising risk behaviours by learning citizenship and social norms building and encouraging employment related skills and activities Personal and financial accountability skill building, mentoring and monitoring the completion of tasks such as: budgeting, managing finances and payment of bills handling money paying rent on time Other Professional Support People in Group Accommodation will be referred to clinical services and/or case management from government (including Medicare funded), nongovernment and/or other providers as required, by referral. People may also receive a separate day program or employment service from another service provider. Outcomes and measures: The Group Accommodation model enables people to: 1. receive appropriate levels of support that manage risks associated with health and well-being, and meet individual choice and need; 2. reside in their own home within a community setting, access meaningful activities and community services; 3. receive individually appropriate support to complete activities of daily living and develop and maintain social connection, which are consistent with their life stage and functional needs; 4. have suitable opportunities for decision making and choice; 5. develop skills to meet their goals and access less intensive services in the future or delay possible requirement for more intensive services. 5 Outputs and reporting requirements: Performance indicators measure those features of the model that demonstrate operational efficiency and effectiveness. Performance indicators can measure features at both the activity/service model level and the outlet level. The performance indicators for the Group Accommodation model are: 1. Number of people receiving a service 2. Average staff hours per person, per week 3. Average expenditure per outlet 4. Average expenditure per person 5. Number of group homes 6. Number of villas/apartments 7. Number of co-located arrangements 8. Number of service clusters The Accommodation Type (e.g. group home, villa/apartment, co-located arrangement) relevant to a service will be specified in the Accommodation Description(s) set out in the Special Conditions in the Service Description Schedule. The Accommodation Type (as specified) must be reported in the above output measures. Additional output measures specific to this model may also be included in the applicable Program Guidelines. These must be reported on separately as required by the Program Guidelines.
