How does escreen process previous drug and alcohol testing history requests for employees who have moved into DOT covered positions? The MyeScreen.com application incorporates an online ordering tool for managers to submit previous employer history. Additionally, escreen has the ability to receive employer history data in an electronic format for processing. Upon receipt of the employer information, escreen electronically places the order to its sourcing partner(s) for processing. Each employer is contacted to verify testing history, and the fi nal report abstract is posted to the manager via MyeScreen.com or via another method as designated by the employer. How does escreen notify employers of any DOT violations obtained via the previous drug and alcohol testing history requests? The fi nal report abstract is posted to the designated manager via MyeScreen.com or via another method as designated by the employer. How does escreen ensure that all the required documentation for post accident and reasonable cause testing will be obtained? All documentation related to a drug test is scanned and attached to the electronic record for easy retrieval. 4 How does escreen ensure that an employee who has violated one of the federal drug and alcohol regulations at a previous employer is taken out of service until the appropriate SAP information is obtained? escreen will contact the respective management via phone, fax, , or electronic notifi cation via MyeScreen.com regarding the violation to ensure that the individual remains out of service until the documentation regarding SAP has been provided. This information will be appended to the record for easy retrieval. How does escreen manage the DOT physical exams? DOT physical management is housed in escreen s Medical Review department (emro). Once a DOT physical is scheduled using MyeScreen, and conducted at an escreen partner clinic, the clinic is prompted to generate and print a fax cover sheet with which to transmit the physical form to emro. Upon receipt of a faxed physical, escreen transmits the data directly to the employer management. Does escreen use hard copy or electronic forms for DOT physicals? Standard hard copy DOT forms are most widely used at the clinic level today. These reports are forwarded to escreen, scanned and added to the electronic record. Full copies are available on MyeScreen.com. escreen is in the process of expanding its services to include the electronic physical form via escreen123 to all the clinic network partners. escreen envisions a process available across all technology-enabled clinics that would include the use of this electronic form, coupled with the standard hard copy for clinics that do not have immediate access to the escreen platform. What kind of tracking system does escreen utilize for DOT physicals? emro runs an automated report of missing physicals based on MyeScreen scheduling information. These missing physicals are researched and the clinics contacted by escreen, who either obtains the physical documentation from the clinic for entry into the database, or releases a no-show report to the employer. Who will review the DOT physicals for correctness and issues? Will they have an occupational health background? escreen s emro administrative staff will conduct an initial clerical review of the physical to ensure the appropriate sections of the document are complete. Medical review of the document is conducted by one of emro s licensed medical staff members, including one of the certifi ed MROs or licensed nurses all of whom have an occupational health background.

5 What database does escreen use for DOT physicals to include all numeric elements in the DOT physical? escreen s electronic physicals solution utilizes its current collection software (escreen123) architecture. Physicals data elements are captured through the end user application interface designed specifi cally for DOT and employer-specifi c requirements. Data is transmitted through the escreen systems and stored in the production Microsoft SQL 2005 database. Explain escreen s blind performance testing coordination service. escreen s emro department sends the minimum number of blind performance specimens required by federal regulations to the appropriate laboratories for analysis each quarter. emro will maintain the MRO copy of the CCF form for each blind specimen, the specimen demographic information citing the expected result from the vendor, and the certifi ed laboratory copy of the CCF in the database using escreen s proprietary enterprise MRO software. Under the supervision of the Medical Review Offi cer, an emro administrative staff member will oversee the blind specimen process and ensure that the appropriate documentation has been received prior to MRO verifi cation of the result. Any specimen results that do not match the expected outcome will be referred back to the testing laboratory for further investigation; any false positives will be referred to both the laboratory and the Offi ce of Drug and Alcohol Policy Compliance (ODAPC).
