4 Summary of this inspection Why we carried out this inspection This was a routine inspection to check that essential standards of quality and safety referred to on the front page were being met. We sometimes describe this as a scheduled inspection. This was an unannounced inspection. How we carried out this inspection We looked at the personal care or treatment records of people who use the service, carried out a visit on 7 August 2013, observed how people were being cared for and checked how people were cared for at each stage of their treatment and care. We talked with people who use the service, talked with staff and talked with other authorities. What people told us and what we found There were five women living at the home. We met four women as one was in hospital. They told us they liked this home, they were happy and felt well looked after. One person said; "It's a lovely home and I'm glad to be here." People said they liked the food. One person said; "I am fed well" and another said the food was "not bad at all." We found that staff knew people's individual needs and wishes well and everybody had individual attention every day from staff. People's health needs were well looked after and they were encouraged to take part in activities in the home. People told us they enjoyed regular contact with the provider's grandchildren and pets, especially those who were unable to see their family often. The staff worked with other care providers such as community mental health nurses, GPs and psychiatrists and a palliative care team to make sure people's health needs were met. We found there were enough skilled and experienced staff to meet people's needs. People were provided with equipment they needed for their safety and to help them maintain their independence. 5 Our judgements for each standard inspected Care and welfare of people who use services People should get safe and appropriate care that meets their needs and supports their rights Our judgement The provider was meeting this standard. People experienced care, treatment and support that met their needs and protected their rights. Reasons for our judgement We were able to meet and talk to four of the five women who lived in the home. The other person was in hospital at the time of our inspection. All four women told us they liked the home, were happy there and felt well looked after. One said they had "no problems" and another told us; "it's a lovely home and I'm very glad to be here." We observed staff interacting with people throughout the morning and saw that staff had formed good relationships with each person in the home and knew their needs and wishes well. People's physical appearance showed they had good support with their personal care. We saw that staff bought culturally appropriate skin and hair products for people who were of African- Caribbean origin. We saw that people were spending their time as they wished, either in their room watching television or in the lounge chatting to staff. People said they had a range of activities to choose from. One person showed us art work she had done in the home and some embroidery. Another person said that she stayed in her room but staff visited her there and responded quickly when she called. One person told us that staff supported her every week to make a cake for everybody to share. She showed photographs of cakes she had made and said she enjoyed baking with staff. People used the garden on warm days. Other regular activities were reminiscence work, music and movement, current affairs discussion and singsongs. This home is a family run home where the provider's grandchildren and pets visit regularly. People living in the home told us they liked spending time with the dogs and children and had formed good relationships with them. One person said they missed seeing their own family and enjoyed spending time with young children in the home. Care and treatment was planned and delivered in a way that was intended to ensure people's safety and welfare. 6 manages the home. She explained each person's health needs and how these were met. People at risk of pressure sores had hospital beds and pressure relieving cushions and mattresses. The provider told us that nobody had a pressure sore. We saw that staff supported one person with restricted mobility by ensuring they spent time in different positions in bed and in a chair to avoid the risk of pressure sores developing. The majority of people had a mental illness and we found that the provider supported people well with their mental health and contacted mental health professionals regularly to give updates or ask for advice as needed. We saw evidence that staff made GP appointments whenever there was any concern about people's health. One person was waiting for a GP visit on the day of the inspection. We found that people also saw a dentist and optician regularly. Two people had diabetes and the provider showed a good knowledge of their needs and was able to tell us what health checks these people had to manage their diabetes.the provider kept records of medical appointments. One person was using the services of a palliative care team. We spoke with this person who told us that they were being looked after well by staff in the home. We found that the home managed people's physical and mental health needs well. The provider had given people a fan for their room as the weather was hot. One person said she really appreciated the fan and said she was "very fond" of the provider for looking after her needs. We saw that the provider had bought fresh flowers for people for their bedroom. She demonstrated a good knowledge about each person and commitment to providing person centred care. People's care and treatment was planned and delivered in a way that protected them from unlawful discrimination. People told us they had culturally appropriate food and toiletries. We saw that people's religious needs were addressed. The provider knew each person's religion. One person had CDs of her church services and had visitors from her church. We saw that people had a care plan which was updated monthly and changed if their needs changed. Reviews were held every three to six months to ensure the home was still meeting the person's needs. 7 Meeting nutritional needs Food and drink should meet people's individual dietary needs Our judgement The provider was meeting this standard. People were protected from the risks of inadequate nutrition and dehydration. They had a choice of suitable nutritious food and drinks. Reasons for our judgement We looked at food and nutrition as somebody made a negative comment about not getting enough food. The same person also made positive comments about the quality of food. When we discussed food with the provider and other residents we found that people's nutritional needs and preferences were met. People's food and drink met their religious or cultural needs. The provider told us she cooked West Indian dishes. We saw that saltfish and ackee was on the menu. The provider said everybody liked this meal. One person told us they were fed well and when asked what the food was like said; "good West Indian food." One person was involved in cooking in the home and had advised the staff on how to cook specific cultural meals that she liked. One person did not eat meat for religious reasons but staff provided a variety of fish dishes. People were supported to be able to eat and drink sufficient amounts to meet their needs. The provider told us that staff restricted certain foods for two diabetic people when their blood sugar was not stable. Both these people told us they had treats and enjoyed their food. Staff helped those who needed support to eat. One person had a soft diet provided for health reasons. People were provided with a choice of suitable and nutritious food and drink. We observed that people could choose different foods. The provider also told us that people could eat in the dining room or in their room and choose their own mealtimes if they did not want to eat with the group. We saw staff offering people snacks between meals such as ice creams. We saw staff offer a choice of hot and cold drinks and offer them ice as it was a hot day. There was evidence that staff catered for individual preferences. People could get involved in food preparation if they wanted to. One person told us she enjoyed baking every week with staff support. 8 Cooperating with other providers People should get safe and coordinated care when they move between different services Our judgement The provider was meeting this standard. People's health, safety and welfare was protected when more than one provider was involved in their care and treatment, or when they moved between different services. This was because the provider worked in co-operation with others. Reasons for our judgement The provider had records of all professionals involved with each person's care and had their contact numbers. She showed us records of contacts she had made to ask for a visit from another provider or for advice. Staff worked with GPs, psychiatrists, community mental health nurses, visiting dentists and opticians, diabetic nurses, palliative care staff and district nurses. We saw a record showing the provider attended meetings at the local health centre and kept other professionals informed of people's welfare and progress. We spoke to a representative of the placing authority who told us that they thought the home provided a very good level of care. At the time of the inspection one person from the home was in hospital. The provider had visited her regularly and communicated with other professionals to make future plans for this person. The provider was committed to allowing people to stay in the home for as long as they wanted to if staff could meet their needs, to the end of their life if they wished. 9 Safeguarding people who use services from abuse People should be protected from abuse and staff should respect their human rights Our judgement The provider was meeting this standard. People who use the service were protected from the risk of abuse, because the provider had taken reasonable steps to identify the possibility of abuse and prevent abuse from happening. Reasons for our judgement We looked at a sample of financial records to see if people were protected from risk of financial abuse. We saw that one person was in debt to the provider. The provider told us that a relative managed this person's money and would pay off the debt. Haringey Council was the appointee for one person and the provider sent invoices to Haringey for this person's purchases and received reimbursement. We saw that staff kept records of people's money and expenditures. We saw that staff bought people's clothes and toiletries for them and kept receipts. There were not individual receipts for toiletries and so to find what each person had bought we had to look at the home's receipts. The provider may find it useful to note that failure to keep individual receipts of exact expenditure for people's toiletries may not protect people fully from risk of financial errors. Staff told us that they look out for special offers to save people money on their toiletries. People said they felt safe in the home. The provider was aware of the procedures to follow in the event of any abuse. Inspection Report Stirling Park Residential Home September

10 Safety, availability and suitability of equipment People should be safe from harm from unsafe or unsuitable equipment Our judgement The provider was meeting this standard. People were protected from unsafe or unsuitable equipment. They were provided with the equipment they needed to maintain their safety and independence. Reasons for our judgement There was enough equipment to promote the independence and comfort of people who use the service. People had walking frames and pressure relieving mattresses and cushions where needed. People who needed it had a hospital bed which made it easier for them to get in and out. One person had a hoist in her room so that staff could move her more comfortably from bed to chair. We checked fire records and saw that the fire doors and call bells in bedrooms were tested monthly and the fire alarm points were checked weekly to ensure the fire alarm system was working. We saw that the provider had called the engineers out to correct a fault and that the fire system was service in June At the last inspection a compliance action was made because call bells in bedrooms were not working. We tested the call bell and it was working. We saw records showing that the call sytem was checked by an engineer immediately after our last inspection to ensure it was working properly. One person said she preferred to shout for assistance and staff heard her call. Call bells were within reach of beds so that people could use them at night if they needed help. 11 Staffing There should be enough members of staff to keep people safe and meet their health and welfare needs Our judgement The provider was meeting this standard. There were enough qualified, skilled and experienced staff to meet people's needs. Reasons for our judgement We found the provider who is also the registered manager and two other staff members on duty when we arrived at 9am and another staff member arrived at 10.30am to engage people in planned activities. This meant there were enough staff to provide individual care for everyone living in the home. The provider told us that there were always three staff on duty in the mornings and two in the evenings and at weekends. We checked the staff rota to confirm that these staffing levels were in place. All staff had worked in the home for several years and knew people's needs well.the home is a family business and the provider's mother, daughter and granddaughter were employed. We saw that people benefited from this. They said they enjoyed the activities organised by the young staff member and the reminiscence activities were run by a staff member their own age.there were two staff on duty at night. One staff member was awake at night to respond to people's needs.the other staff member was asleep in the home and available for any accidents and emergencies.the provider said that one member of staff could look after people's needs at night. Staff have attended training relevant to their role and all were experienced at providing care for older people with mental and physical health needs. 12 About CQC inspections We are the regulator of health and social care in England. All providers of regulated health and social care services have a legal responsibility to make sure they are meeting essential standards of quality and safety. These are the standards everyone should be able to expect when they receive care. The essential standards are described in the Health and Social Care Act 2008 (Regulated Activities) Regulations 2010 and the Care Quality Commission (Registration) Regulations We regulate against these standards, which we sometimes describe as "government standards". We carry out unannounced inspections of all care homes, acute hospitals and domiciliary care services in England at least once a year to judge whether or not the essential standards are being met. We carry out inspections of other services less often. All of our inspections are unannounced unless there is a good reason to let the provider know we are coming. There are 16 essential standards that relate most directly to the quality and safety of care and these are grouped into five key areas. When we inspect we could check all or part of any of the 16 standards at any time depending on the individual circumstances of the service. Because of this we often check different standards at different times. When we inspect, we always visit and we do things like observe how people are cared for, and we talk to people who use the service, to their carers and to staff. We also review information we have gathered about the provider, check the service's records and check whether the right systems and processes are in place. We focus on whether or not the provider is meeting the standards and we are guided by whether people are experiencing the outcomes they should be able to expect when the standards are being met. By outcomes we mean the impact care has on the health, safety and welfare of people who use the service, and the experience they have whilst receiving it. Our inspectors judge if any action is required by the provider of the service to improve the standard of care being provided. Where providers are non-compliant with the regulations, we take enforcement action against them. If we require a service to take action, or if we take enforcement action, we re-inspect it before its next routine inspection was due. This could mean we re-inspect a service several times in one year. We also might decide to reinspect a service if new concerns emerge about it before the next routine inspection. In between inspections we continually monitor information we have about providers. The information comes from the public, the provider, other organisations, and from care workers. 13 How we define our judgements The following pages show our findings and regulatory judgement for each essential standard or part of the standard that we inspected. Our judgements are based on the ongoing review and analysis of the information gathered by CQC about this provider and the evidence collected during this inspection. We reach one of the following judgements for each essential standard inspected. This means that the standard was being met in that the provider was compliant with the regulation. If we find that standards were met, we take no regulatory action but we may make comments that may be useful to the provider and to the public about minor improvements that could be made. Action needed This means that the standard was not being met in that the provider was non-compliant with the regulation. We may have set a compliance action requiring the provider to produce a report setting out how and by when changes will be made to make sure they comply with the standard. We monitor the implementation of action plans in these reports and, if necessary, take further action. We may have identified a breach of a regulation which is more serious, and we will make sure action is taken. We will report on this when it is complete. Enforcement action taken If the breach of the regulation was more serious, or there have been several or continual breaches, we have a range of actions we take using the criminal and/or civil procedures in the Health and Social Care Act 2008 and relevant regulations. These enforcement powers include issuing a warning notice; restricting or suspending the services a provider can offer, or the number of people it can care for; issuing fines and formal cautions; in extreme cases, cancelling a provider or managers registration or prosecuting a manager or provider. 14 How we define our judgements (continued) Where we find non-compliance with a regulation (or part of a regulation), we state which part of the regulation has been breached. Only where there is non compliance with one or more of Regulations 9-24 of the Regulated Activity Regulations, will our report include a judgement about the level of impact on people who use the service (and others, if appropriate to the regulation). This could be a minor, moderate or major impact. Minor impact - people who use the service experienced poor care that had an impact on their health, safety or welfare or there was a risk of this happening. The impact was not significant and the matter could be managed or resolved quickly. Moderate impact - people who use the service experienced poor care that had a significant effect on their health, safety or welfare or there was a risk of this happening. The matter may need to be resolved quickly. Major impact - people who use the service experienced poor care that had a serious current or long term impact on their health, safety and welfare, or there was a risk of this happening. The matter needs to be resolved quickly We decide the most appropriate action to take to ensure that the necessary changes are made.
