Mock exams will be administered frequently to prepare student to complete the RHIT examination. COURSE RATIONALE: To prepare students to be knowledgeable of the various healthcare settings where they may work, the healthcare professionals with whom they will work, and of the healthcare accrediting and licensing agencies which will regulate their work. COURSE OBJECTIVES: Upon completion of the course, the student will be able to: 1. Identify and evaluate the level of mastery in HITT professional competency areas. 2. Demonstrate self management to improve knowledge and skill bases identified as needing improvement. 3. Recognize and define the RHIT domains/sub domain stated by AHIMA. 4. Demonstrate the knowledge and skills required for entry level RHIT professionals. 5. Successfully complete multiple, subject based RHIT mock exams. 6. Use and refer to the appropriate resources and textbooks that will assist the student to prepare for the RHIT exam. 7. Enhance test taking skills. 8. Achieve 70% or better on the Semester Final Mock Exam. 9. Demonstrate ability to apply principles of health information management previously acquired in the classroom environment to the work environment 10. Demonstrate a sense of responsibility and self management by adhering to course calendar and assigned work in study guides. 1

2 11. Demonstrate ability to work well with others through positive response to supervision, suggestions for improvement, and evaluation of skill performance. 12. Understand the knowledge and skills required for entry level RHIT professionals. 13. Recall and understand the information contained within each task and domain. 14. Perform intense review of the pertinent information required for RHIT entry level competencies. 15. Complete multiple, subject based RHIT mock exams. COURSE STUDENT LEARNING OUTCOMES: The curriculum of the Austin Community College Health Information Technology program is designed to meet or exceed the professional course content as published in the AHIMA Model Curriculum that includes the HIM Entry Level Competencies and Knowledge Clusters. This course addresses the specific Domains, Subdomains, and Competencies identified below: Domain I: Health Data Management Subdomain A: Health Data Structure, Content, and Standards 1. Collect and maintain health data (such as data elements, data sets, and databases). 2. Conduct analysis to ensure that documentation in the health record supports the diagnosis and reflects the patient s progress, clinical findings, and discharge status. 3. Apply policies and procedures to ensure the accuracy of health data. 4. Verify timeliness, completeness, accuracy, and appropriateness of data and data sources for patient care, management, billing reports, registries, and/or databases. Subdomain B: Healthcare Information Requirements and Standards 1. Monitor and apply organization wide health record documentation guidelines. 2. Apply policies and procedures to ensure organizational compliance with regulations and standards. 3. Maintain the accuracy and completeness of the patient record as defined by organizational policy and external regulations and standards. 4. Assist in preparing the organization for accreditation, licensing, and/or certification surveys. Subdomain C: Clinical Classification Systems 1. Use and maintain electronic applications and work processes to support clinical classification and coding. 2. Apply diagnosis/procedure codes according to current nomenclature. 3. Ensure accuracy of diagnostic/procedural groupings such as DRG, MSDRG, APC, and so on. 4. Adhere to current regulations and established guidelines in code assignment. 5. Validate coding accuracy using clinical information found in the health record. 2

3 6. Use and maintain applications and processes to support other clinical classification and nomenclature systems (ex. DSM IV, SNOMED CT). 7. Resolve discrepancies between coded data and supporting documentation. Subdomain D: Reimbursement Methodologies 1. Apply policies and procedures for the use of clinical data required in reimbursement and prospective payment systems (PPS) in healthcare delivery. 2. Apply policies and procedures to comply with the changing regulations among various payment systems for healthcare services such as Medicare, Medicaid, managed care, and so forth. 3. Support accurate billing through coding, chargemaster, claims management, and bill reconciliation processes. 4. Use established guidelines to comply with reimbursement and reporting requirements such as the National Correct Coding Initiative. 5. Compile patient data and perform data quality reviews to validate code assignment and compliance with reporting requirements, such as outpatient prospective payment systems. 6. Ensure accuracy of diagnostic/procedural groupings such as DRG, APC, and so on. Domain II: Health Statistics, Biomedical Research, and Quality Management Subdomain A: Healthcare Statistics and Research 1. Collect, maintain, and report data for clinical indices/databases/registries to meet specific organization needs such as medical research and disease registries. 2. Collect, organize, and present data for quality management, utilization management, risk management, and other related studies. 3. Comprehend basic descriptive, institutional, and healthcare vital statistics. Subdomain B: Quality Management and Performance Improvement 1. Abstract and report data for facility wide quality management and performance improvement programs. 2. Analyze clinical data to identify trends that demonstrate quality, safety, and effectiveness of healthcare. Domain III: Health Services Organization and Delivery Subdomain A: Healthcare Delivery Systems 1. Apply current laws, accreditation, licensure, and certification standards related to health information initiatives from the national, state, local, and facility levels. 2. Differentiate the roles of various providers and disciplines throughout the continuum of healthcare and respond to their information needs. Subdomain B: Healthcare Privacy, Confidentiality, Legal, and Ethical Issues 1. Adhere to the legal and regulatory requirements related to the health information infrastructure. 2. Apply policies and procedures for access and disclosure of personal health information. 3. Release patient specific data to authorized users. 3

4 4. Maintain user access logs/systems to track access to and disclosure of identifiable patient data. 5. Apply and promote ethical standards of practice. Domain IV: Information Technology and Systems Subdomain A: Information and Communication Technologies 1. Use technology, including hardware and software, to ensure data collection, storage, analysis, and reporting of information. 2. Use common software applications such as spreadsheets, databases, word processing, graphics presentation, e mail, and so on in the execution of work processes. 3. Use specialized software in the completion of HIM processes such as record tracking, release of information, coding, grouping, registries, billing, quality improvement, and imaging. 4. Apply policies and procedures to the use of networks, including intranet and Internet applications, to facilitate the electronic health record (EHR), personal health record (PHR), public health, and other administrative applications. 5. Participate in the planning, design, selection, implementation, integration, testing, evaluation, and support for EHRs. Subdomain B: Data, Information, and File Structures 1. Apply knowledge of database architecture and design (such as data dictionary) to meet departmental needs. Subdomain C: Data Storage and Retrieval 1. Use appropriate electronic or imaging technology for data/record storage. 2. Query and generate reports to facilitate information retrieval using appropriate software. 3. Apply retention and destruction policies for health information. Subdomain D: Data Security 1. Apply confidentiality and security measures to protect electronic health information. 2. Protect data integrity and validity using software or hardware technology. 3. Apply departmental and organizational data and information system security policies. 4. Use and summarize data compiled from audit trails and data quality monitoring programs. Domain V: Organizational Resources Subdomain A: Human Resources 1. Apply the fundamentals of team leadership. 2. Participate in and work in teams and committees. 3. Conduct orientation and training programs. 4. Monitor and report staffing levels and productivity standards for health information functions. 4

5 5. Use tools and techniques to monitor, report, and improve processes. 6. Comply with local, state, and federal labor regulations. Subdomain B: Financial and Resource Management 1. Make recommendations for items to include in budgets and contracts. 2. Monitor and order supplies needed for work processes. 3. Monitor coding and revenue cycle processes. 4. Recommend cost saving and efficient means of achieving work processes and goals. 5. Contribute to work plans, policies, procedures, and resource requisitions in relation to job functions. DISCIPLINE/PROGRAM STUDENT LEARNING OUTCOMES: Upon completion of the Associate of Applied Science Degree in Health Information Technology, the student will be able to: A. Appropriately manage and use health data. B. Collect, report and interpret database information and compute related healthcare statistics. C. Apply and participate in the implementation of laws and policies and procedures within healthcare delivery systems as they relate to payment systems, healthcare provider information needs, patient privacy and disclosure and ethical standards of practice. D. Utilize technology, including specialized hardware and software applications to ensure accurate data collection, record tracking, analysis, reporting and will be able to apply and contribute to the application of electronic health records and to the maintenance and design of patient information retrieval systems, while maintaining confidentiality and security of information. E. Apply the fundamentals of team and financial resource management, including budgeting, teamwork, education, communication and interpersonal skills in order to contribute to work plans, policies and procedures, resource management and others in performance as a member of a team. SCANS Competencies In 1990, the U.S. Department of Labor established the Secretary s Commission on Achieving Necessary Skills (SCANS) to examine the demands of the workplace and whether our nation s students are capable of meeting those demands. The Commission determined that today s jobs generally require competencies in the following areas: Resources: Identifies, organizes, plans and allocates resources Interpersonal: Works with others Information: Acquires and uses information Systems: Understands complex interrelationships 5

6 Technology: Works with a variety of technologies The Texas Higher Education Coordinating Board requires that all degree plans in institutions of higher education incorporate these competencies and identify to the student how these competencies are achieved in course objectives. HITT 2249 COMPETENCE Resources Interpersonal Information EXAMPLE OF LEVEL Identifies resources used in course and allocates time for studying. Shares experiences and knowledge with classmates, works as a member of a team for any assigned activities. Participates in weekly discussions. Identifies systems to use such as excel, vizio or other statistical software. Identifies classification systems for inpatient, outpatient and procedures. Identifies statistical formulas for determining length of stay, daily census, death rates, newborn deaths, and other statistical formulas. Discusses electronic health record with classmates and instructor. Reads assigned pages. Identifies and prepares for tests, quizzes and research activities. GRADING SYSTEM: The Health Information Technology courses use the following scale for determination of final grades: A = % B = 80 89% C = 70 79% D = 60 69% F = 59% and below A grade of 70% or above is required for passing any subject area. METHOD OF EVALUATION: Achievement of objectives will be evaluated by: Examinations 50% (There will be at least 3 written exams and/or computer delivered examinations including recognition and recall as well as analysis and discrimination.) Final for Course 50% COURSE POLICIES: Attendance/Class Participation Regular and punctual class and laboratory attendance is expected of all students. If attendance or compliance with other course policies is unsatisfactory, the instructor may withdraw students from the class. Withdrawal Policy It is the responsibility of each student to ensure that his or her name is removed from the roll should he or she decide to withdraw from the class. The instructor does, however, reserve the 7

8 right to drop a student should he or she feel it is necessary. If a student decides to withdraw, he or she should also verify that the withdrawal is submitted before the Final Withdrawal Date. The student is also strongly encouraged to retain their copy of the withdrawal form for their records. The student is required to turn in their program student ID and any equipment or items that belong to the department. Failure to do so may compromise their standing at ACC. Students who enroll for the third or subsequent time in a course taken since Fall, 2002, may be charged a higher tuition rate, for that course. State law permits students to withdraw from no more than six courses during their entire undergraduate career at Texas public colleges or universities. With certain exceptions, all course withdrawals automatically count towards this limit. Details regarding this policy can be found in the ACC college catalog. Incompletes An instructor may award a grade of I (Incomplete) if a student was unable to complete all of the objectives for the passing grade in a course. An incomplete grade cannot be carried beyond the established date in the following semester. The completion date is determined by the instructor but may not be later than the final deadline for withdrawal in the subsequent semester. Requests for incompletes must be submitted by the following dates: Deadline for Fall Semester: Second Friday in November Deadline for Spring Semester: Friday following Spring Break Deadline for Summer Semester: Friday following the 4th of July In all cases an incomplete cannot be requested unless 1/2 of the required coursework has been completed. Students must follow all instructions given by faculty or designated college representatives when taking examinations, placement assessments, tests, quizzes, and evaluations. Actions constituting scholastic dishonesty include, but are not limited to, plagiarism, cheating, fabrication, collusion, and falsifying documents. Penalties for scholastic dishonesty will depend upon the nature of the violation and may range from lowering a grade on one assignment to an F in the course and/or expulsion from the college. These rights carry with them the responsibility to accord the same rights to others in the college community and not to interfere with or disrupt the educational process. Opportunity for students to examine and question pertinent data and assumptions of a given discipline, guided by the evidence of scholarly research, is appropriate in a learning environment. This concept is accompanied by an equally demanding concept of responsibility on the part of the student. As willing partners in learning, students must comply with college rules and procedures.
