2 PROVIDER ROLE DEFINITION: The nurse, using the nursing process, helps the client, patient or family to identify and meet basic health needs in a wide variety of settings. The nurse coordinates care of the client, patient, family and groups using a multidisciplinary approach. ASSESSMENT ANALYSIS PLANNING IMPLEMENTATION EVALUATION Assists in the identification of a data base for the client, patient or family based on a holistic assessment of health needs. Identifies from the data base common needs and problems and assists with the formulation of short and long term goals with the client, patient or family. Assists in the development of individualized nursing care plans with the client, patient, family and other health care providers utilizing the data base. Performs therapeutic and preventive nursing measures based upon fundamental knowledge of biological, psychological, social, spiritual, cultural, developmental and economic concepts in providing individualized care in structured settings. Shares responsibility for the care of the client, patient or family in structured settings. Utilizes basic communication techniques n structured care settings. Promotes a therapeutic relationship with the client, patient or family. Participates in the evaluation of nursing care given and in implementing the necessary changes. Establishes a data base for the client, patient or family based on a holistic assessment of health needs. Utilizes established data base to identify actual and potential needs and problems in order to formulate nursing diagnoses and establish short and long term goals for the client, patient or family. Develops individualized nursing care plans through utilization of the data base in consultation with the client, patient or family and other health care providers. Uses nursing knowledge, skills and established protocols to perform dependent, interdependent nursing measures according to the needs demonstrated by the client, patient or family. Assumes responsibility for care of the client, patient or family in structured settings. Applies theories of communication to therapeutic relationships in a variety of settings. Establishes and maintains a therapeutic relationship with the client, patient or family. Uses established criteria for evaluation of individualized nursing care. Identifies alternate methods of meeting the client, patient or family needs and modifies plan of care as necessary, documenting changes. Participates with the client, patient or family and other health care providers in the evaluation of established short and long term goals.

3 TEACHER ROLE DEFINITION: The nurse provides client, patient or family education in a variety of settings to promote health, using a wide range of teaching activities. ASSESSMENT ANALYSIS PLANNING IMPLEMENTATION EVALUATION Identifies situations in which the client, patient or family need information, education and support to promote, maintain and restore health. Identifies actual and potential needs of the client, patient or family for information. Responds appropriately to questions of the client, patient or family. Assists in the formulation of a teaching plan. Utilizes a specific teaching plan during routine care. Supports and reinforces the teaching plans of other health professionals. Assembles content in a way that is conducive to learning. Utilizes available and appropriate instructional materials. Participates in the evaluation of the client, patient or family by identifying outcomes and reporting to appropriate person. Participates in modification of the teaching plan. Identifies and assesses situations in which the client, patient or family need information, education and support to promote, maintain and restore health. Formulates teaching plans based upon short and long term goals with consideration given to biological, psychological, social, spiritual, cultural, developmental and economic factors. Identifies individualized selected teaching strategies applicable to the situation. Formulates teaching plans based upon short and long term goals with the client, patient or family. Selects an appropriate teaching methodology for the specific situation or problem. Utilizes the nursing process, implements and documents appropriate teaching of the client, patient or family. Uses appropriate content and teaching strategies for the instruction of the client, patient or family. Provides the client, patient or family with an environment conducive to learning; provides opportunities to practice the expected behaviors and receive feedback. Utilizes appropriate instructional materials. Evaluates the learning of the client, patient or family through the use of established criteria. Modifies the teaching plan as indicated.

The nurse intervenes when clients, patients or families are unable to act in their own interest.
