3 Serving a diverse community. 42% of population comprised of immigrants and visible minorities from 169 countries 51.5% do not speak English or French as first language 170 languages spoken in Toronto 12% of population are new immigrants Highest rates of low income and single parent families 20% of children live below poverty line 36% of Toronto households have low incomes 40% of homeless in Ontario live in Toronto Largest gay population in Canada TB rate is triple (3.1 times) the provincial average HIV/AIDS rate 11.3 times higher than Ontario average Highest rates of completed post-secondary education and some of the wealthiest Canadians live in Toronto 3

4 What We Do Information & Referral Services Helping callers find their way & supporting CCR website hits for information Community Independence Connecting seniors to care & supporting them to remain independent in the community Adult Supportive Care Supporting 1100 adults & their caregivers to manage their long term conditions Urban Health Supporting 1200 marginalized clients with mental health issues, or homeless to access services & be supported to live in their community Palliative Care Supporting 1600 clients to die at home with dignity Hospital Transitions Supporting patients to safely transition from hospital Child and Family Helping 5000 kids get the right support in schools; 382 children who have complex medical needs & 1000 children with post acute & rehab needs Seniors Enhanced Care Intensive support to 6400 seniors & their caregivers to remain at home with dignity Post Acute & Rehab Care Providing 13,000 clients with post acute & rehab care 4

10 Meeting the unique needs of sub-populations Many immigrant women as informal caregivers of older adults, children with medical and physically disability and other individuals that need care Designing programs and supports for subpopulations of immigrant women: o South Asian immigrant women in Thorncliffe Park Improving Diabetes management o Tamil Women in St. Jamestown Enhancing access to services and supports 10

11 Some Key Considerations In home care need to consider all immigrant women those that are formal and informal care providers and those that need care Need to address the subtle and not so subtle issues of race and discrimination Understanding the needs of sub-populations of immigrant women and co-designing programs to meet their needs Organizations need to have a strong culture, leadership and commitment to diversity & equity
