1 Dear Parent/Guardian Re: Year 8 Student Resource Scheme I have attached information including a Participation Agreement Form, regarding the Student Resource Scheme operated by our school and endorsed by our Parents and Citizens Association. The last page is the Participation Agreement Form which is the only page that needs to be returned. It is a requirement of Education Queensland that the Participation Agreement Form is completed and signed, by Parent/Guardian. This should be done irrespective of whether or not you wish to join the scheme, as indicated by ticking either the Yes or No box on the form. 2 ELIGIBILITY CRITERIA FOR THE TEXTBOOK AND RESOURCE ALLOWANCE Purpose Department of Education and Training provides assistance to parents of secondary school age students attending state and approved non-state schools to contribute towards the cost of textbooks and learning resources. Eligibility The following students are eligible: Students attending approved schools in Years 7 to 12 Students in ungraded secondary school programs and who are at least 13 years of age on 1 January Students who are at least 13 years of age on 1 January and attending state special schools and non-state special schools holding funding approval (15 years for senior secondary rate) Students undertaking courses in Australian Agricultural College campuses in Queensland equating to and in lieu of Years 11 to 12. The following students are NOT eligible: Students enrolled as full fee paying overseas students (FFOS) Students enrolled in a Centre for Continuing Secondary Education (CCSE) Students undertaking part time distance education from a non-state school accredited for distance education Students enrolled AFTER the first Friday in August.

3 INDOOROOPILLY STATE HIGH SCHOOL STUDENT RESOURCE SCHEME CHARGES FOR YEAR 8, 2015 The hire charge of $ together with your consent that the Government Textbook and Resource Allowance of $112.00* (see page 2 for eligibility criteria), be paid directly to the school includes provision for: The Standard Charges Administration of the Student Resource Scheme Reproduced class workbooks and worksheets and teacher-prepared material which complement and/or substitute for textbooks Student diary Student ID card Student material used for classroom projects Student reference material for hire (e.g. books; audio/video tapes) Textbooks An additional subject charge is required to cover the costs of materials for students studying the following subjects. Hire of Instruments $ Music consumables such as drumsticks and mallets $30.00 Required for Performances $ Cost for the sourcing and purchasing of necessary resources in the target language e.g.: Spanish books, audio-visual material. - Translation of materials from English to Spanish: units, exams, worksheets. - Reproduce class workbooks and worksheets in Spanish and additional photocopies. Price to be advised by Distance Ed $50.00 $ Students are required to purchase their own writing and stationery materials. These are listed on the page overleaf. NON PARTICIPATION IN THE STUDENT RESOURCE SCHEME Students must purchase the textbooks listed on page 5 as well as other books nominated by the teacher during the year. The Standard Charges and Subject Charges (listed above) as well as the Additional Subject Charges for reprographics, consumables etc. (listed on page 5) must also be paid. In addition, students will not be entitled to the school subsidy to participate in non-curriculum specific activities; for example, end of year trips, school productions and Year 12 formal. * subject to change
