7 CIHI s PHC Data and Information Program CIHI is leading several initiatives to improve primary health care data and information - Work with multiple stakeholders to strengthen and improve PHC data available to clinicians and health system decision makers - Program of work includes both data and information solutions 7

8 PHC Indicators - To identify two sets of priority indicators - for Policy Makers - for PHC Providers - To ensure the measures for the priority sets of indicators are standardized, aligned with evidence-based guidelines, and are compatible with existing and developing data sources - To include broad stakeholder input in the updating process to ensure the indicators meet the needs of endusers - Pan-Canadian PHC Indicators available for uptake and use in the Fall

9 PHC Survey Tools - Supplement to EMR and administrative data sources - Jurisdictions and clinics conducting surveys to understand patient experience, provider experience and models of care-results not comparable Patient Experience Survey Provider Experience Survey - Validated, standard set for clinic, regional, provincial or national use Practice Based Survey - Core set of survey questions and supporting tools will be available for update and use in the Spring

10 PHC EMR Content Standard Developed Pan-Canadian PHC EMR Content Standard to support improved PHC data and information Developed Pan-Canadian PHC EMR Data Extract Specification to support the standardization of EMR data for clinical quality improvement and health system use Continue to provide adoption and implementation advice, documents and support to jurisdictions, Infoway and EMR vendors 10

12 PHC Voluntary Reporting System CIHI s PHC VRS is an EMR data source available for health system analysis & quality improvement for clinicians who use an EMR Participation is voluntary (>500,000 patients) Clinicians inform the design of the PHC VRS quality feedback reports Clinicians do not need to collect new data to participate, so it s easy to receive the benefits of the QI tools CIHI only collects a minimum amount of EMR data in a privacy sensitive manner to support PHC clinicians with quality improvement 12
