1 Requesting Services from Facilities Management Using the FAMIS Self Service Web Application NOTE: The following instructions are to be used by FDCB Staff. Anyone with a Western Washington University (WWU) Universal ID and password can submit a request for services by using the FAMIS Self Service Web Application. If you experience problems with this form please contact the FAMIS Help Desk at x2034 or at: FAMIS@wwu.edu The link to FAMIS Self Service can be found on the Applications page of Administrative Computing under the CUSTOMER SELF SERVICE heading. You can bookmark this page: Bookmarking the actual FAMIS Self-Service page is not recommended because there might be updates made to the link that would cause the bookmarked link to fail without the user knowing it. The link on the Applications page is always the correct updated link. Step 1. To access and log into FAMIS Self Service Go to Western s Home Page and click on mywestern located in the top right corner of the page Click on the word Administrative under the Technology heading
2 Click on Admin Application option from the menu on the Adminstrative Computing page Select FAMIS Self Service Submit Work Orders from the CUSTOMER SELF SERVICE heading.
3 Log into FAMIS Self Service Enter your WWU Universal ID and Password in the Log-in screen and then click on the Login button. Step 2. Completing the Self Service Request Once you ve successfully logged into FAMIS Self Service the first screen you see is Select a Site. Completing the Requested Work Information At the Select a Site page, click on the Service Requests button At the Display Service Requests page, click on the Create Service Request button
4 At the Create Service Request: Confirm Requestor Step 1 of 5 page o Enter your telephone extension o Under Alternate Requestor area enter the name and phone extension of the Budget Authority. o Click on the Next button when completed. At the Create Service Request: Select Service Step 2 of 5 page For PW Projects/Assists always select Maintenance Request and Construction Services Note: The Select Service Step 2 of 5 page is under construction. The next two pages will be used in the near future but in the interim please click through the two pages.
5 At the Create Service Request: Enter Service Details Step 3 of 5 page o Enter the Work Title. Always start with the PW Number then the title. This will ensure the Work Control Center routes the request to the appropriate person. o Enter the Description of Work. Please be thorough and complete in your description. Enter location information (building, floor, room, area, etc.) here. You can request that you be contacted prior to beginning of work if you d like. o Enter Earliest Start Date. Be sure to follow the mm/dd/yyyy format. o Enter the Latest Completion Date. o If there are any times or dates that FM staff would not be able to do the work, please include that here. (Example: Dates/Times class is in session if work is to be done in a classroom) o When you ve completed this step, click on the Next button.
6 At the Create Service Request: Enter Location Step 4 of 5 page Note: Please do not type into the boxes. Use the buttons next to each area. FAMIS will auto fill these areas based on the picks made from the drop down lists that appear after you ve clicked on the specific button. o Click on the Site button. At the Select Site (Description) page, click on ALL WWU LOCATIONS FAMIS returns you to the Enter Location Step 4 of 5 page. You can see that FAMIS auto-filled in the field associated with Site. FDCB Staff - Do not complete the Building, Floor or Room information. If you do, then the service request will be routed to the wrong place. Once you ve completed the Site information, click on the Next button which will take you to Step 5 of 5.
7 At the Create Service Request: Review Step 5 of 5 page you are able to review the information that you entered into each of the steps. If you need to change something, use your browser back arrow to return to the necessary step to make your changes. If you are satisfied with the information that has been entered, then click on the Finish button. I Once you ve click on the Finish button, you will receive the Create Service Request form that lists the Work Request number and the date it was submitted. Save this for your records.
8 FAMIS automatically generates an to you from FM s Work Control Center as well. You will see it in your Outlook inbox. Below is what the looks like. FAMIS will auto-generate an to you when your request/work order (or Project) is updated to the following statuses: WORK ORDERS Requested Issued Completed PROJECTS Open Design Complete
