This checklist is to help students and parents organize the forms needed to apply to the Information Technology Program at Battlefield Senior High School. Please submit completed forms in the order below. Check off the following forms when completed Application for Admission. Be sure to check your address and indicate your base high school. Student Information Release Form. Program Requirements Contract (read and sign; return signed copy with application, keep one for your records). Copy of your semester report card (if applying early, be sure to send semester report card when you receive it in February). Student Statement of Interest Essay. Two Teacher Recommendation Forms from core academic areas. (Your teachers will send these forms). Please indicate on the application which area of Information Technology you are interested. Check only one. Transfer Request Form. (For students who do not live in the Battlefield High School attendance zone.) Transfer requests can be obtained from all schools. Fill out the top section only at this time; parents must sign. Submit with this application. Transfer students must submit new transfer request each year. Express Bus Stop Form. Locate the nearest bus stop and write the number. Transfer students only; must submit one each year. Read and sign the Specialty School Transfer Regulation. 2

Print your name on the two teacher recommendation forms and give them to two of your Math, Science, English, or Social Studies teachers. 2. An essay addressing your interest and experience in technology (1 page typed). 3. A copy of your semester report card. Students must have a 2.4 (C+ GPA) to apply. 4. If applicable, a Student Transfer Request Form with the first section completed only. Check your address! To be eligible for the program, students must have recommendations from two teachers, a 2.4 (C+) GPA or better, and no U's in conduct in any class. 6 it PROGRAM REQUIREMENTS CONTRACT Battlefield High School The Information Technology (it) Program is a rigorous and challenging course of study for academically and technology motivated students. The Information Technology Program will provide interested students the opportunity to become engaged in an intensive technological program of studies developed through a collaborative relationship with the business and academic community. 1. All students applying for the first time must have a 2.4 GPA. Once in the program it students must maintain a 2.0 (C) overall GPA and a 2.0 (C) in all core it classes in order to be scheduled for it classes the following year. If by the first semester a student does not meet the minimum requirements, they will not be scheduled for the following year. 2. All it students are expected to maintain a satisfactory (S) grade or better in conduct. If a student receives a U in any class they will not be scheduled in the it program the following year. 3. Students must be enrolled in at least one core it class each year. 4. Students must maintain a digital it Portfolio to be submitted by the end of 1st semester of their senior year. The portfolio will consist of one best practice or project for each year they are in the it program. 5. Students wishing to achieve an it Advanced Certificate must maintain a 3.0 overall GPA with no grade below a 3.0 (B) in any it core class. A total of 6 or more it core classes are required. 6. Academic expectations and grading standards are high; homework and independent study is a mainstay for success in all it courses. 7. Students must successfully complete all prerequisites before enrolling in any it class. 8. Parental involvement and support are critical to the student's success in the it Program. 9. All it students must have access to an Internet connected computer at home. 10. All students enrolled in the it program will be expected to adhere to the Prince William County Schools Code of Behavior and the Computer Ethics Policy for technology. Any deviation from these policies will be grounds for immediate removal from the it Program. Please sign and return this contract with all other application forms. It is advised that families keep one copy for their records. 11 PRINCE WILLIAM COUNTY PUBLIC SCHOOLS TRANSPORTATION DEPARTMENT SPECIALTY PROGRAM TRANSPORTATION INFORMATION FORM The Transportation Department's mission is to provide safe and on-time transportation for Prince William County students. In order to provide this service for the specialty programs, please complete the following information. Accurate and complete student information will assist the Bus Operations Section in allocating drivers, buses and establishing express bus stops for all eligible specialty program students before the school year begins. Express bus stop service differs from regular neighborhood service in that the parents have full responsibility to provide transportation to and from the express bus stops. Students living within the boundaries of the specialty program school will ride their regular bus to school. 13 Virginia High School League Eligibility Specialty School Transfer Regulation Rising ninth grade students who transfer to a school to participate in a specialty program will be eligible for VHSL sponsored activities for athletic/activity participation. Rising tenth and eleventh grade students who transfer to any specialty program MUST submit a request for eligibility if accepted into the specialty program. The transfer must occur at the beginning of the program. The student must meet full participation requirements of the program to retain eligibility. If a student withdraws or is dismissed by the program director from the specialty program, once the withdrawal procedure is completed, the student must return to his/her base school. If a student withdraws from the specialty program on or before October 15, the student will be ineligible for participation in VHSL sponsored activities for the remainder of the first semester, but eligible for participation for the second semester. If a student withdraws from the specialty program after October 15 but before the end of the first semester, the student will be ineligible for participation for the remainder of that school year. Eligibility will be restored at the base school at the beginning of the fall season of the next school year. If the student withdraws during the second semester of a school year, the student will be ineligible for the next fall semester. If a student withdraws from the specialty program after one school year, the student will return to his/her base school and will be ineligible for one year.
