2 Project Description Launchpad Youth Community Inc., WEAVE Youth and Community Services and Ted Noffs Foundation are a consortium of leading youth service practitioners, who have come together to deliver the newly funded Inner City Sydney Homelessness Prevention and Support Service for Young People. The service is funded by the NSW Department of Family and Community Services (FACS) through the Specialist Homelessness Services (SHS) Program. The Homelessness Prevention and Support Service for Young People will deliver a model of service delivery, which is built upon the pre- existing expertise of the partners. As such the Casework/Counsellor position will deliver activities related to its Home agency of employment Weave, as well core work related to the consortium service which will be a centrally located service hub in the inner city of Sydney, for co- location and team management. The consortium will deliver client centred services, including early intervention, crisis response, rapid rehousing approaches and post crisis support to support young people experiencing homelessness or at risk of homelessness in the City of Sydney local government area. While the partners in the consortium, and therefore team members may have different philosophies in approaches and different roles, a shared client load, and integrated approach to service delivery is one of the under pinning principles of this model, and the employees role to uphold. This will at times require stepping away from our distinct role, to support delivery of services in other parts of the program. The consortium will employ staff individually, however they will come together as a team of multi skilled practitioners with both therapeutic and integrated casework skills to deliver ICSYHS to young people in the inner city of Sydney. The service location for ICSYHS will be centrally located, however as an employee of Weave you will have strong links to your Home agency, regular training, team building, planning and learning and development opportunities with both Home agency and across consortium services. The Home organisation of employment will have clinical oversight and management of ongoing therapeutic management work with clients, including treatment and support approaches. This includes Human Resource accountabilities and responsibilities. There is an expectation that each employee will participate in Weave training, supervision, team building and clinical development. Operational management of day to day activities and coordination of clients, client approaches and service activities remain with the ICSYHS Executive Officer. This includes allocation of crisis work provided by the organisation and may include team building activities which are separate to the Home employing organisation. There will be strong systems in place to ensure effective communication channels between partners, aligning support strategies for clients. Purpose of position To deliver client centred early intervention, prevention, counselling, case management, outreach, tenancy and support services for young people experiencing homelessness or at risk of homelessness in the City of Sydney local government area. The Counsellor/Case Manager will support the crisis team by providing ongoing emotional and social wellbeing counselling sessions. Also providing practical case management support. (please refer to the Weave How we do what we do document) 3

4 The Weave worker will be based at both Weave and the hub and be expected to be able to manage two complimentary worksites. Key accountabilities Provide family counselling, individual counselling and therapeutic support with a caseload of up to 15 clients at a time depending on complexity it may be slightly smaller. Provide AOD and other support for young people with Mental health issues Provide individual client centred casework and case management support to young people with complex trauma histories and complex needs, including mental health and AOD issues. (managing a caseload of up to 15 clients at a time depending on complexity) Provide assertive outreach to the client where required, including where they reside, recreate and access other services Provide support to the client to maintain tenancy/housing longer term in partnership with property managers from community housing providers Participate in Cross Agency Case Management Groups as required Provide intake and assessment for new and existing young people and comprehensive referral pathways to other SHS, mainstream and housing agencies. Be culturally competent and able to support Aboriginal young people Provide support for young people to return to their community of origin where appropriate Facilitate access to brokerage funds where required Provide support for young people to continue or access education, employment and training options. Liaise with mental health/aod/ and other relevant professionals to improve support provision, access and services to the identified target group. Arrange community networking for the young person to encourage positive interactions in a safe and supportive environment Prevention and early intervention of homelessness Continue to deliver counselling and case management services for young people who are re- housed after becoming homeless and support them to stay housed. Deliver Early intervention and prevention strategies Provide educational and project work that prevents young people from becoming homeless, such as school programs, recreational programs and groupwork. Identify and support young people who are at imminent risk of homelessness to remain safely in their existing housing or to secure stable housing Homelessness service responses Provide direct support and case management services to young people in crisis and respond to their immediate needs Assist young people who are homeless to be rapidly and safely rehoused Facilitate access for young people in crisis with appropriate accommodation and support to access stable housing Administration Maintain accurate written records, statistics and reports Maintain data collection and contribute to funding body reports as required. Work in a highly responsive, proactive and innovative way with young people. Participate in staff meetings, training and other forums as required Maintain your own professional development in consultation with your supervisor Perform other duties as required 4

5 General Weave Youth and Community Service Accountabilities General Weave Accountabilities Work as part of the wider Weave team displaying effective team membership by assisting with and participating in whole of service events and supporting events run by other Weave programs. Attend all Weave Staff Meetings, team building days etc. and relevant interagency meetings. Participate in training and performance appraisal as required Meet regularly for supervision with your Team Leader Attend monthly clinical supervision Perform all duties in accordance with the Weave code of conduct and the Weave policies and procedures and philosophy Perform all duties in line with EEO policy and OH&S safety standards, ethical practice principles and a commitment to the principles of cultural diversity. Selection Criteria Essential! Recognised Tertiary qualifications in counselling, social work, youth work or a relevant field Or! Equivalent experience in the field of practice and accredited counselling courses! Current NSW Working With Children Clearance! At least 3 years experience in counselling with young people and case management of clients with complex needs, experiencing homelessness including sound knowledge of integrated case management theory and best practice principles for young people.! Broad understanding and use of different counselling modalities such as Narrative Therapy, CBT and Mindfulness to name some.! Demonstrated ability to manage complex caseloads, effectively plan and prioritize work with multiple priorities.! Demonstrated working knowledge of the youth sector, Juvenile Justice, Out of Home Care system and child protection agencies and Specialist Homelessness Services.! Experience in the homelessness services field and Working knowledge of the Going Home Staying Home reform, program and guidelines.! Ability to work flexible hours and work afterhours as required.! Demonstrated ability to utilise established referral pathways and use trauma- informed and strengths based approaches to achieve client outcomes.! Demonstrated ability to work with Aboriginal young people and their familes! Excellent communication skills, including written and verbal presentation skills, diplomatic and effective liaison and interpersonal skills.! Ability to provide detailed reports and collect statistics for program evaluation.! Demonstrated planning, organisational and excellent time management skills.! A current NSW Drivers licence.! Excellent IT skills in Microsoft Office, Excel, Power Point and Access.
