11 ELIGIBILITY REQUIRMENTS AZ 501(c)(3) organization, government agency, & tribal entity operating with fiscal responsibility Religious organizations may apply for non-sectarian programs Must demonstrate non-discrimination policy All organizations and Fiscal Agents must have visible profile on ACF website and also be 501(c)(3) organization
12 PROPOSALS ARE ENCOURAGED TO: Leverage matching funds Engage strategic, mission-driven collaborations Build capacity of organization Support direct service projects Support capital and/or program projects Support one-year and/or renewable funding proposals
13 Letter of Interest Notifies ACF of Cochise, DACF, NCF, and LFSAZ that an organization is interested in submitting a full grant application in the upcoming 2015 Open Competitive Grant Cycle. An organization fully understands that the proposed project may not be selected to submit a complete grant application. This is not a LETTER this is an application online!

Funding request range: Minimum $250 Maximum $5,000 ACF allows for multiple applications under certain circumstances Please review the What if... section of the grant guidelines for further explanation Organizational profile must be fully completed before accessing an application.

Application Questions: Applicants will select the field of fund(s) for which the organization will be applying ALL organizations will select - Cochise Community Foundation General Endowment Fund Organizations serving the geographic areas of Douglas, Double Adobe, Elfrida, and McNeal will select Douglas Area Community Fund Organizations serving the geographic areas of Willcox, Bonita, Bowie, San Simon, Cochise, Dos Cabezas, Dragoon, and Pearce/Sunsites will select Northern Cochise Fund
24 Field of Interest Funds Bisbee 1000 Health and Wellness Fund: Supports healthy, active living programs in Bisbee and Cochise County. Bob Holcombe Fund for Educational and Vocational Success: Supports at-risk students pursuing education in Cochise County. Consideration will be given to any program or project which provides support for the educational and/or vocational success of Cochise County residents who may be at risk of not graduating from high school. Preference may be shown to programs or projects which provide in-person assistance to students in grades 7-12 and college level vocational education students in their first year, in their efforts to obtain a vocational education and a high school diploma, with an emphasis on support for students who may be at risk of not graduating from high school.
25 Field of Interest Funds Borderlands Fund: Supports improving the quality of life in Cochise County by investing in projects that may also provide benefit in Mexico. Cochise Arts and Culture Fund: Supports arts, culture and historic preservation in Cochise County.
26 Field of Interest Funds Cochise Community Foundation General Endowment Fund: Supports programs and services for the residents located in the geographic area of Cochise County in Southeastern Arizona. Cochise Environmental Fund: Benefits organizations whose purpose it is to address environmental issues and environmental education in Cochise County. Grants from this fund will be used to support, nurture and initiate programs which further environmental causes and education.
27 Field of Interest Funds Cochise Health and Wellness Fund: Supports health and wellness programs that address physical and mental health needs. Cochise Lifelong Learning and Literacy Fund: Benefits organizations whose purpose it is to promote lifelong learning and literacy including, but not limited to, support of programs encouraging adult/parent computer literacy; Spanish/English language programs; mentoring programs; skill development programs; early childhood development programs and other programs that support border issues in Cochise County.
28 Field of Interest Funds Cochise Veterans Fund: Provides a permanent source of funding dedicated to the needs of Cochise veterans and their families. Douglas Area Community Fund: Supports programs and services for residents located in Douglas, Double Adobe and McNeal.
29 Field of Interest Funds John Lawrence Kurdeka Fund for the Environment: Supports environmental education and outreach; preservation and restoration of habitat and wildlife in Cochise County. Northern Cochise Fund: Serves the Sulphur Springs and San Simon Valleys in Northern Cochise County; specifically the communities of: Bowie, Cochise, Dos Cabezas, Dragoon, Pearce, San Simon, Sunsites, and Willcox.
30 Field of Interest Funds Science, Math and Technology Innovation Fund: Benefits public school teachers throughout Cochise County whose purpose is to teach, to organize classroom projects or to continue their own education in Science, Mathematics and Technology by using or learning innovative methods in these curriculum. The Legacy Foundation of Southeast Arizona Fund: Supports capacity building and collaboration, the Foundation's Mission is to Promote Population Health and Community Wellness Throughout Southeast Arizona.
