4 WHAT IS AN HSA? Health Savings Account A bank account to use for medical expenses Money saved in an HSA is tax-free* Unused dollars remain in the account year after year Can be used as a medical 401(k) a way to save for retirement Yearly contributions are permitted up to the established IRS maximum contribution limit It s portable you own the account funds even after changing employers, retirements, etc. You are in control of your account- You decide how to spend your money Allows you to pay for medical expenses that may not be covered under your health plan *Tax references are for federal taxes only. State taxes vary. Consult your tax advisor. 4
5 HDHP + HSA = HEALTHY BENEFITS A High Deductible Health Plan (HDHP) and HSA work together to create your Healthy Benefits product. You must have an IRS-qualified HDHP in order to open an HSA Medical and prescription drug expenses accumulate to the same combined deductible The PPO medical plan covers medical expenses after the deductible has been met The Prescription Drug Plan covers drug costs after the deductible has been met Funds in the HSA can be used to pay for any IRS eligible expenses 5
6 DESIGN DIFFERENCES HSA VS. PPO The IRS requires design differences for HDHPs compared to standard PPO plans Deductible HDHP Medical and Rx are combined The deductible applies to ALL services (except preventive) Qualified plans only have two tiers of deductible coverage single or family (contracts of two or more) The deductible amount must be met before any family member is eligible for covered benefits Standard PPO Medical and Rx are not combined The deductible does not apply to services that take a copayment (such as OV, ER etc.) Plans can have various tier structures The family deductible amount may not need to be met to be eligible for benefits if an individual met the single deductible amount Out-of-Pocket Medical and Rx are combined Medical and Rx are not combined in large group products 6
7 WHO IS ELIGIBLE FOR AN HSA? HSAs are available to any individual that: Is covered by an IRS-qualified HDHP Is not enrolled in Medicare Can t be claimed as a dependent on someone else s tax return Is not covered by other health insurance, some examples of permitted/non-permitted coverage are: Permitted Limited FSAs and HRAs Auto, life, workers comp, or disability coverage Dental care, vision care or long-term care Specific disease insurance, and per diem hospitalization insurance Not Permitted General Purpose FSA or HRA Other Rx coverage Medical coverage under spouse plan 7
8 HSA CONTRIBUTIONS The IRS annually adjusts minimum deductible amounts and maximum contribution amounts based on Cost of Living (COLA) Minimum Deductible Maximum Contribution Maximum Out-of-Pocket 2014 Amount $1,250 single $2,500 family $3,300 single $6,550 family $6,350 single $12,700 family 55+ Catch up Contributions $1,000 8

10 YOUR RESPONSIBILITIES HSAs are an IRS program, and as such, require tax filing every year If funds are withdrawn or used for a nonqualified expense before age 65, taxes and a 20% penalty will apply Save receipts in the event of an audit Do not exceed contribution limits with personal and employer contributions Keep account in an open status to receive all applicable contributions Reconcile account regularly to ensure adequate funds are available to cover qualified expenses Members should contact their tax professional for more information on tax implications and filings. 10

Will the hospital be less expensive or a free standing facility? How many procedures has each facility performed? 11

13 HSA PROCESS FLOW You go to the doctor s office and pay a copay at time of service Provider submits a claim to Capital BlueCross Capital BlueCross determines if deductible is met Deductible is met: Capital BlueCross processes claim Capital BlueCross mails check to provider and EOB to you No further action is needed Deductible is not met: Claim is processed and applied to deductible Capital BlueCross mails EOB to you, and SOR to provider Provider bills you for the service Payment options: HSA funds debit card, check, or online bill pay OR Non- HSA money - thus saving HSA funds for future use EOB = Explanation of Benefits SOR = Statement of Remittance 13
