1 Program Specialist Bellingham (Whatcom County) & Burlington (Skagit County) JOB ANNOUNCEMENT Preference given to applications received by: Feb. 5, 2016 Open until filled REPORTS TO: Community Programs Program Supervisor TEN-STEP HOURLY RANGE: $ $26.48, DOQ JOB SUMMARY: The rthwest Regional Council, as an Area Agency on Aging, funds and provides community-based programs to help people live in their own homes and communities for as long as possible, postponing or eliminating the need for institutional care. Program Specialists provide outreach, information, assistance, and options counseling on the services, products, and/or resources available to clients and their families through social service agencies, medical providers, and community partners who face challenges related to aging and/or disabilities. Functions include screening and assessing to identify needs, making referrals and\or authorizing appropriate resources and services, providing ongoing education, consultation, and general assistance to clients and family caregivers. NWRC is currently recruiting for 1 FTE each for Bellingham (Whatcom County) and Burlington (Skagit County) offices ESSENTIAL JOB DUTIES include but are not limited to: 1. Develops and implements effective outreach strategies. Provides clients and their families with pertinent information about aging, disability, and caregiving resources. 2. Provides outreach and information to difficult-to-serve populations including individuals with limited-english proficiency, rural/isolated populations, and populations of minority older adults. 3. Provides effective public relations, advocacy, and information to other agencies, businesses, churches, and community members to ensure that rthwest Regional Council programs are respected and known to be an important source of resources for the community. 4. Screens clients for services by telephone intake interview, office visits, and home visits when necessary. 5. Completes automated assessment of client or caregiver needs that evaluates client s cognitive, behavioral, and functional abilities. Determines need and level of care for respite services and authorizes resources for support 6. Determines eligibility for and authorizes such Family Caregiver Support Program (FCSP) services as Respite Care, Caregiver Training, and Caregiver Consultants. 7. Works with the client or caregiver using service planning and supportive counseling to identify services and supports that address individual circumstances and preferences. Refers clients to appropriate services and evaluates success of referral. Provides timely follow up. Monitors progress of service delivery as needed. 8. Works closely with internal and external partners to maintain awareness of available services to meet the needs of clients, caregivers, and their families to assure partners are knowledgeable and can make appropriate referrals for program services. 9. Provides appropriate and accurate information on the services, products, and/or resources available to clients and their families. 10. Provides crisis/mental health referrals for clients. Makes Adult Protective Services (APS) referrals as necessary. 11. Gathers and/or develops and maintains resource files and/or database(s) of programs and services. Manages resource files and database(s) using Alliance for Information and Referral Systems (AIRS) taxonomy. Provides information to designated agency webmaster to update program web pages.

2 12. Facilitates community presentations and trainings for agencies and businesses as requested. 13. Develops and maintains client files which document activities performed for the client and contain all other records required for the ADR or FCSP program. Compiles reports as necessary to meet reporting and statistical requirements for funding purposes. As necessary, tracks authorized spending against actual spending and stays within allocated budget guidelines QUALIFICATIONS: Knowledge, Skills and Abilities: Demonstrated skills in interpersonal communication, interviewing, client assessment, problemsolving, organization, time management, and documentation of activities. Ability to manage several tasks at one time with repeated interruptions, in a shared office space, and meet regular deadlines. Extensive knowledge of the formal and informal support networks available to the residents of the appropriate service area. Demonstrated ability to triage client needs, assess client s strengths and weaknesses, and match clients with services in the region. Knowledge of the assigned geographical area. Knowledge of the long term care process, issues related to aging and disability, service delivery systems, and family caregiver issues. Demonstrated ability to work with computers and various computer software programs including Microsoft Office, Access database, internet, and . Ability to operate standard office equipment. Ability to track authorization costs and stay within a budget. Ability to communicate effectively in both oral and written format. Ability to develop and present clear information to groups of people. Ability to locate resources using resource database and files to match the needs identified by clients with the eligibility requirements of the program. Ability to interpret the eligibility requirements of Medicare, Medicaid and other public programs. Ability to maintain confidentiality of information, including client information, and financial records. Ability to establish and maintain effective working relationships with co-workers, other members of government service, community agencies, appropriate community businesses, and members of the general public using courtesy, tact, and good judgment. Sensitivity to the needs and values that are present in working with diverse groups of people. Knowledge of Alzheimer and dementing disease processes preferred. Skill and/or fluency in speaking Russian or Spanish is desired. Education/Experience: Bachelor's Degree in social services or related field; AND Two years of social service experience; OR An equivalent combination of education and experience which provides the skills, knowledge and ability to perform the work. Licenses/Certificates: Must have a valid WA State driver's license, a vehicle available for work-related travel, and appropriate liability insurance. 3 WORKING CONDITIONS\PHYSICAL REQUIREMENTS:. Work is performed in an office environment with client home visits and outside assignments. The office environment is a shared workspace and may be busy due to constant interruptions. Potential hazards include working with emotionally unstable, non-compliant or aggressive clients and complex family dynamics; significant local travel; driving in inclement weather; second-hand tobacco smoke; aggressive animals; exposure to contagious diseases; and exposure to repetitive stresses due to prolonged use of computers or laptops. Sufficient mobility is required for the use of office equipment such as computers or laptops, telephones, files and copiers, as well as for performing in-home assessments of clients, which may have limited accessibility. Sitting for extended periods of time as well as lifting a maximum of 30 pounds may be required. The ability to hear and communicate at a level sufficient to perform the essential functions of the position is required.
