Joe Parks, Director of Missouri s Medicaid authority, will discuss how Missouri, an early adopter, successfully implemented Health Homes for different populations with demonstrable improvements in health and cost savings. The presentation will be followed by an opportunity for questions and answers with the presenter. Following that, presenters will discuss specific payment methodologies and financial incentives that can support this work, with an opportunity for dialogue with the audience. Without access to safe, decent, and affordable housing, individuals are unable to focus on managing their co-occurring health conditions and often end up in the ER for their care. Many states are interested in providing housing stability to their complex care populations, but are unsure with whom to partner and how to provide access. Experts will explain what affordable housing is, who states need to engage, and how to leverage existing resources. The session will be followed by an opportunity for questions and answers from the audience.
