Without access to safe, decent, and affordable housing, individuals are unable to focus on managing their co-occurring health conditions and often end up in the ER for their care. Many states are interested in providing housing stability to their complex care populations, but are unsure with whom to partner and how to provide access. Experts will explain what affordable housing is, who states need to engage, and how to leverage existing resources.
