1 Meet with the Director of Graduate Studies Select your Doctoral Advisor or Permanent Lab Qualifying (PharmEPO only) Select your Doctoral Committee Members Doctoral Plan of Study October, first year in residence End of 1 st year in program The first year of a student s program of study End of 2 nd year in program No later than the summer of 2 nd year in residence Meet with your program Doctoral Advisor with your list of potential members The Graduate Program Coordinator will schedule a time for you to meet with the Director of Graduate Studies Throughout your doctoral studies you will have a Doctoral Advisor to guide and support you. He/she will advise you to complete the following: Plan of Study; Annual Evaluation; Written and Oral Comprehensive ; Dissertation Proposal; Dissertation and Defense. Meet with PharmEPO Track Director Throughout your doctoral studies you will have a committee to guide and support you. They will serve as your Committee as you complete the following: Written and Oral Comprehensive ; Dissertation Proposal; and the Dissertation Defense. no later than 30 days prior to Written Exam. Annual Report Due June 1 st of each year Obtain PhD Annual Report Form from COP Complete form, have your advisor review and sign it, and return to Pharmacy no later than June 1 st. Annual Reports will be given to the Track Directors for review May 2015 Page - 1 -
2 Student Evaluation Written Comprehensive Oral Comprehensive Committee Form (3 major +1-2 minor members) Announcement of Doctoral Comprehensive Exam Measurement for the Outcomes After 2 nd year in residence and formation of committee, student evaluation is required each year At completion of minor and minor course work no later than the end of the 3 rd year. Within six weeks of passing Written Usually within six months of passing the written comprehensive examination Day of Oral Exam Proposal No later than 10 business days before Oral Exam Obtain Student Annual Committee Report Form from COP Graduate Programs Otain Comprehensive Written Exam Results Form from COP Obtain Outcomes Assessment Form prior to Oral Exam from COP Hard Copy and bounded Schedule a meeting with your committee on an annual basis. Give the form to your committee. Each member will evaluate your progress. Your advisor will give you an overall evaluation and make comments as to your progress. Make a copy and return original to the Pharmacy Graduate Coordinator no later than June 1 st. no later than 30 days prior to schedule Written Exam. Upon exam completion, collect committee signatures and submit form to Pharmacy Graduate Coordinator in Skaggs 336. When you schedule your written exam, meet with Pharmacy Graduate Coordinator to submit form in Grad Path. no later than 30 days prior to Oral Exam to submit form in GradPath, schedule a room and/or practice session(s). Each member of the Comprehensive Exam Committee will fill out the Outcomes Assessment form after the oral exam. Return forms to the Pharmacy Graduate Coordinator. Please check with any of your committee members if they prefer an electronic copy as well May 2015 Page - 2 -
3 Prospectus/Proposal Confirmation Doctoral Dissertation Committee Appointment Announcement of Final Oral At proposal meeting or Oral Exam Within six weeks of passing Oral Upon completion of the dissertation and no later than six weeks prior to Defense. Obtain Proposal Form from COP Graduate Programs Take the Report form to the Dissertation Proposal Meeting or Oral Exam. Obtain signature and return it to the Pharmacy Graduate Coordinator. Once you have finalized your dissertation committee, meet with Pharmacy Graduate Coordinator to submit form in Grad Path. At least 3 months prior to proposed defense date, send committee a Doodle for potential dates. Once you have secured a date and time, meet with Pharmacy Graduate Coordinator at least six weeks prior to Final Exam to submit form in GradPath, schedule a room and/or practice session(s). Recommendations to Students Know and observe deadlines for each step in your degree program Remember that you must be enrolled each fall and spring semester while you are working toward your degree. Failure to maintain continuous enrollment will result in delays, additional paperwork and fees. 4 Satisfactory Academic Progress Students are required to demonstrate satisfactory academic progress toward degree completion. In addition to maintaining a minimum 3.0 grade-point average, students enrolled in the graduate degree program are required to demonstrate satisfactory academic progress toward degree completion. The Program s policies on what constitutes satisfactory academic progress are listed below. Minimum Grades in a Required Course - Students must receive a grade of "B" or better in all core courses. A student who receives a grade of "C" or less in a required course must repeat that course. Students failing to obtain a B or higher in a required course that is repeated must petition the graduate program faculty to remain in the program. The decision to allow the student to continue in the program requires a majority approval of the program faculty, with two negative votes sufficient for termination. Student Evaluation The program faculty committee annually evaluates each student on the basis of accomplishments in formal courses and performance in other areas of the Program including lab research as well as attendance and participation in seminars. Satisfactory performance in courses and research are required. Failure to meet performance criteria in any of these areas is grounds for recommendation of dismissal from the Program. Annual Reports - All students will submit an annual report to the Graduate Program Coordinator, on or before June 1. The Annual Report must be approved and signed by the Track Director (first year students) or the Research Advisor (all other students) prior to submission to the Program office. Annual Reports for every year matriculating in the program are mandatory. Sponsorship - By June 15 the program faculty make a determination if each first year student will be sponsored for the following year (based on performance criteria). This determination will be assessed yearly thereafter. Poor performance in assigned duties may result in the loss of sponsorship. Sponsorship decisions after the first year are made by the student s major advisor and or the Director of Graduate Studies. Advancement to Candidacy - At the end of their third year in residence, students are evaluated for Advancement to Candidacy at the time of their comprehensive examination. If performance is substandard, the program faculty may recommend a probationary period, withdraw program sponsorship, seek dismissal, or may request the student to fulfill the requirements for a Master's degree. The program faculty will evaluate overall student performance in the Program to date. If performance has been deemed satisfactory by the faculty, approval will be granted. Dissertation/Thesis Committee Meetings - All students are required, after the formation of a dissertation/thesis committee, to have at least one committee meeting per year while in the program. 5 Completion Timeline Each College of Pharmacy graduate track will include a table of completion of degree requirements in their student handbook, which is posted on the College of Pharmacy graduate programs Website. Notification of Failure To Meet Academic Progress Guidelines When it is determined by the graduate track faculty that a student is not meeting academic progress, the student will receive written notification with a clear statement of what he/she must do and a date by which such actions must be completed. The Graduate College will receive a copy of the letters of unsatisfactory progress. Students who fail to remediate by the deadlines specified may be dismissed from the program. Students will be given an opportunity to appeal or rebut. Appeals Process If a student wishes to appeal any of the requirements mentioned above, the appeal should be made in writing to the director of the student s research track (Pharmaceutical Economics, Policy, and Outcomes; Pharmaceutics; Drug Discovery and Development; Clinical Research; Pharmacology & Toxicology). The appeal will be reviewed by the entire program faculty and may include a collective meeting with the student. A decision to accept the appeal of the program faculty will be based on a majority vote. The program faculty may place additional requirements/deadlines on the student as a prerequisite for continuing in the program. Students who wish to appeal the decision of the program faculty must submit an appeal in writing to the director of graduate studies in the College of Pharmacy. May 2015 Page - 5 -
