COMPANY APPLICATION, ELECTRICAL INSTALLATIONS AREA: Tick off: Electrical contractor The authorisation entitles your company to carry out heavy current electrical installations and installation of non-heavy current control and regulation systems used to control heavy current functions and the servicing of these installations and systems. Partial authorisation residential installation contractor The authorisation entitles your company to carry out electrical installations in permanent residence housing with individual settlement of electricity and holiday houses approved for overnight stays. The work includes the installation in the place of residence and in rooms connected with and supplied from the place of residence but not part of the actual living area, the supply line, outdoor installations and any photovoltaic system connected to the place of residence in question. Partial authorisation photovoltaic installation contractor The authorisation entitles your company to carry out the installation and connection of photovoltaic systems to an already designated connection point in the fixed electrical installation. On the DC side the installation and connection includes the wiring system from the photovoltaic panels to the photovoltaic inverter and associated safeguard equipment. On the AC side the installation and connection includes the wiring system from the AC terminal on the photovoltaic inverter to the connection point in the fixed electrical installation. For residential installations the installation and connection also includes the installation of associated overcurrent- and fault protection equipment on the AC side when placed in the residence to which the photovoltaic system belongs. 2. 2 3. QUALITY MANAGEMENT SYSTEM: (CONFIRMATION FROM A CERTIFYING BODY) Tick off: Quality management system documentation is enclosed. Confirmation from an approved certifying body that the company s quality management system has been found to be satisfactory following a provisional review. A foreign company seeking authorisation to carry out - on temporary or occasional basis - work requiring authorisation in Denmark must submit documentation that the company already uses a quality management system for production and end control. A company set up in another EU or EEA country seeking authorisation as electrical contractor in a role of temporary service provider can meet the approved quality management system requirements if the company submits documentation to the Danish Safety Technology Authority containing proof of the current use of a quality management system for production and end control approved by a recognised certification agency on the basis of joint European or national standards. The company must at the same time confirm to the Danish Safety Technology Authority in writing that the company s quality management system for production and end control also covers the work requiring authorisation that the company seeks to carry out in Denmark, and that work within these areas will be carried out in accordance with Danish rules and regulations. A foreign company seeking authorisation as electrical contractor in preparation for working permanently in Denmark must present documentation that the company has a quality management system relating to work covered by the law s section 2 and the quality management system must be approvable by the rules in the Act on quality management systems for authorised companies within the electricity, plumbing and sewage installation industries and for companies with company authorisation in the gas area No. 545 of 30 May If a foreign company already uses a quality management system for production and end control approved by a recognised certification agency on the basis of joint European or national standards, the approved certifying body assesses the system on the basis of the received documentation from the company. On this basis, the approved certifying body assesses whether or not to approve the system. On the basis of reports from the recognised certification agency, the approved certifying body checks whether the company uses and actually acts in accordance with its quality management system. 4. INFORMATION ABOUT THE TECHNICALLY RESPONSIBLE PERSON: (A SEPARATE APPLICATION MUST BE SUBMITTED BY THE TECHNICALLY RESPONSIBLE PERSON) Your enquiry will be processed according to two different procedures, depending on whether you wish to pursue your profession on a temporary and occasional basis or whether you wish to work permanently in Denmark. When applying for working permanently or pursuing your profession on a temporary and occasional basis, the duration, frequency and continuity of the pursuit of your profession are very important factors. Please read the following guidelines carefully before completing the form. If the form is not completed correctly or if the necessary documentation is not attached to this, the case processing time will be prolonged. You can read more about the Act, the associated Executive Orders and the structure of a simple quality management system on the website, of the Danish Safety Technology Authority. The company management is responsible for setting up a quality management system and for its use and continual testing by an approved certifying body. The objective of the quality management system is to ensure that electrical installations are carried out and serviced correctly due to safety. The company must have affiliated one technically responsible person with the necessary technical skills within the area of business. Setting up of the quality management system In the quality management system the company must describe: 1) The company s data Central Business Register information etc. 2) The company s organisation and distribution of tasks 3) The company s responsibility and skills situation 4) The business area within the authorisation area The skills of the technically responsible person and the company In its quality management system the company must describe: 1) The technically responsible person(s) tasks, authority and responsibilities in connection with the employees carrying out the work. 2) The qualifications of the management, the technically responsible person(s) and the employees. Company procedures In its quality management system the company must describe: 1) Procedures for manning tasks, instruction of employees, supervision and end control. 2) Procedures for management s internal evaluation of the quality management system. 3) Procedures for the processing of mistakes in the completed work requiring authorisation or work that demands a company authorisation within the gas area. 4) Access to all relevant and updated legislation and instruction material from authorities covering the business area and required for the safe setup, operation and service/maintenance of installations. If the business area includes the production of electrical installations reports as part of the Danish Huseftersynsordning, the company must also describe: 1) Procedure for ensuring that the authorised company is impartial in the production of electrical installations reports, see section 7, No. 4, in the Act on electrical installations reports, part of the Danish Huseftersynsordning. 2) Procedure for ensuring that committed mistakes in connection with the production of electrical installations reports are not repeated. Document management including testing and end control In the quality management system the company must hold: 1) Documentation and traceability for performed end control and copy of the inspection and control reports including checklists for internal end controls of all work requiring authorisation.

AFFILIATION REQUIREMENT FOR THE TECHNICALLY RESPONSIBLE PERSON: The technically responsible person must be affiliated with the company for a minimum of 30 hours per week within the company s normal business hours. This affiliation ensures, among other things, that the technically responsible person has a real presence in the company and is able to ensure the correct manning of tasks, that the employees are instructed sufficiently and that sufficient supervision is carried out. 5. THE MANAGEMENT OF THE COMPANY DECLARES: That the company, the company owner or the company CEO has not been convicted (fine/caution) of infringement of authorisation legislation including the technical rule set within the past three years. 6. FEE: A fee must be paid through the website of the Danish Safety Technology Authority, in conjunction with applying for authorisation. 7. SIGNATURE: Company owner/ceo confirms by signature that the above information is correct, and that the company is currently not undergoing bankruptcy proceedings or in liquidation. The name, address and telephone number of the company are published here. Personal data, including civil registration (CPR) No., is not published. It is essential that all parts of the form are completed so that it can be assessed whether the requirements for obtaining authorisation are met. The requirements are laid down in the Consolidation Act authorisation of companies in the electrical-, plumbing- and sewer installation field (No. 401 of 28 April 2014), as subsequently amended, with associated Executive Orders, which can be consulted on the website of the Danish Safety Technology Authority. Note that the Danish Safety Technology Authority may check the accuracy of your declarations, for example by contacting the Danish Customs and Tax Administration to ask whether you are indebted to the State.
